The first participant has now been dosed in the Phase 3 PRECISE-HD trial of pridopidine for HD. The study will test whether the drug can slow disease progression in people most likely to benefit

new Huntington’s disease (HD) clinical trial has officially begun, marking another important step in the search for treatments that could slow the progression of the disease. Prilenia Therapeutics and Ferrer have announced via a Linkedin post that the first participant has been dosed in the PRECISE-HD Phase 3 clinical trial, which is testing the investigational drug pridopidine.

Another chance for pridopidine
Long-time HDBuzz readers may recognise pridopidine. Pridopidine has had one of the longest and most winding development journeys of any experimental HD therapy. Originally developed as a treatment for movement symptoms, early clinical trials failed to meet their primary goals despite nuanced hints of benefit on some tests.

In 2023, the large Phase 3 PROOF-HD study did not meet its primary goal of slowing disease progression across all participants. However, a planned analysis of the data suggested that people who were not taking antidopaminergic medications (ADMs) appeared to have greater benefit from pridopidine than those who were taking these medications. ADMs are drugs commonly prescribed to treat chorea or psychiatric symptoms.

African Bundibugyo ebolavirus vaccine candidate to be advanced to clinical trials

Leading African biopharmaceutical company Minapharm Group, headquartered in Egypt with subsidiaries in Berlin, will receive up to US$16.5 million from the Coalition for Epidemic Preparedness Innovations (CEPI) to advance the development of a Bundibugyo ebolavirus vaccine candidate, strengthening efforts to develop vaccines to combat the fastest-growing and second-largest Ebola outbreak on record affecting the Democratic Republic of the Congo (DRC).

Minapharm’s Bundibugyo vaccine is designed by ProBioGen, a Berlin-based specialist within Minapharm Group, based on their proprietary modified vaccinia Ankara (MVA) vaccine platform, MVA-CR19. A similar technology underpins a licensed vaccine against smallpox and mpox, and has elicited long-term immunity in a vaccine targeting the related Zaire ebolavirus.

CEPI’s funding will advance the Bundibugyo vaccine candidate through preclinical development and into an early-stage clinical trial to assess the immunogenicity and safety of the vaccine candidate. The study in humans, known as a Phase 1 trial, is expected to take place in Africa.

Minapharm’s investigational vaccine becomes the fifth Bundibugyo-specific virus vaccine candidate supported by CEPI in response to the escalating epidemic. It joins a portfolio of promising CEPI-backed candidates* built on different vaccine technologies to maximise the likelihood of developing a safe and effective shot that could help bring this devastating crisis under control.

It is the only Bundibugyo vaccine candidate CEPI is funding that is designed to induce both B- and T-cell responses for comprehensive and long-lasting immunity.

Presented by the Union for International Cancer Control (UICC) and produced for us by BBC StoryWorks Commercial Productions as part of the global Rewriting Cancer series, the feature tells the story of JaTaun Williams, a 68-year-old Chicago-based special education teacher diagnosed with lung cancer after seeking care for COVID-19 like symptoms. Upon her diagnosis, Williams chose to enroll in a clinical trial as part of her treatment. She shares her experience to help others understand the importance of clinical trials and how individual participation contributes to progress in cancer research.

Collecting superfluous clinical trial information unrelated to key endpoints creates unnecessary burdens on patients, sites and sponsors, say experts.

Growing complexity is often cited as a challenge for the trials sector, partly driven by biopharma’s shift toward hard-to-treat diseases. However, another factor making trials harder is the practice of gathering non-core data, researchers say.

Estimates vary, but most observers agree it costs between $2 billion and $2.6 billion and takes 10 to 15 years to develop a biopharmaceutical, a considerable proportion of which is spent on clinical trials.

Trials are expensive, in part, because they are technically challenging, according to recent analysis by investment bank Bourne Partners, which suggests that the growing focus on difficult diseases is a root cause.

Head of research Donald Hooker told BioXconomy, “Sponsors continue to struggle with increasing clinical trial complexity and rising drug development costs.

“A larger driver of the increasing complexity is therapeutic mix. For instance, oncology, central nervous system disorders, and infectious diseases are increasingly dominating the clinical trial pipeline. Each of these therapeutic categories brings with it significant complexity and data management challenges,” he said.

In the US, recent legislative measures – such as the One Big Beautiful Bill Act and the Inflation Reduction Act – are also impacting the types of diseases developers target, according to Hooker.
