PRIORITY SERVICES

The Brazilian Registry of Clinical Trials (ReBEC) presents the new fast tracks for clinical research and observational studies on INDIGENOUS PEOPLE AND TRADITIONAL COMMUNITIES and on NEGLECTED TROPICAL DISEASES (NTDs). Fast-track can reduce the normal approval period for clinical trials to less than 48 hours, if the documentation, information and any mandatory changes requested are provided by the registrant.

The already existing fast-tracks and the attendance to other studies will not be interrupted. Priority care is granted with a view to accelerating clinical research in epidemiological emergency situations and/or studies with the potential to reduce socioeconomic inequalities, keeping the perspective aligned with the forefront of ethics and transparency, diplomacy in health, scientific collaboration and diversity and specificities of recruitment and health care.

FAST-TRACKS IN OPERATION:

I - Indigenous Peoples, Traditional Communities, Human Migration and Refugee Movements – clinical research and observational studies dedicated to or preferably addressing traditional communities (quilombola, caiçara, faxinalense, among others), Indigenous peoples (within or outside legally demarcated territories), and refugee groups, taking into account the specificities established by regulation, on a case-by-case basis.

A single algorithmic correction — removing one variable from one equation — is now demonstrably responsible for thousands of Black patients gaining access to kidney transplants they were previously denied. If that doesn’t reframe how the clinical trial industry thinks about the tools embedded in its own infrastructure, nothing will.

According to STAT News, removing race as a variable from the estimated glomerular filtration rate (eGFR) calculation — the standard measure used to assess kidney function — produced a 5.3 increase in transplants per 1,000 Black candidates. That is a structural correction to a system that was actively and measurably disadvantaging patients based on a biological assumption with no sound scientific foundation. The race-adjusted eGFR had been artificially inflating kidney function estimates for Black patients, making them appear healthier than they were, and therefore less urgently in need of a transplant. The algorithm wasn’t neutral. It was a gatekeeping mechanism dressed in clinical clothing.

The implications for clinical trials are the story. Because the same class of algorithmic thinking — race-adjusted, historically derived, assumption-embedded — is woven throughout the trial ecosystem. It lives in the eligibility criteria. It lives in biomarker stratification. It lives in the composite endpoints that define whether a drug works. And almost nobody is auditing it.

European regulators want to streamline study authorization when health crises hit, learning from pandemic shortcomings that hampered research efforts

Clinical trials could be green lit faster during public health emergencies under new draft guidelines issued by the European Medicines Agency (EMA).

The proposals include regulations intended to accelerate the approval of studies “relevant to the emergency itself” as well as those “addressing other serious or life-threatening medical needs.”

The draft provides guidance on when trial participants can be transferred across investigational sites and when adapted procedures or alternative approaches may be required due to the specific challenges posed by public health emergencies.

It also encourages sponsors to contact the EMA’s Emergency Task Force (ETF) to ensure trials are efficient and capable of delivering meaningful data.

Trial design
All trials—particularly those addressing public health emergencies—need to be well-planned and inclusive, according to the authors, who say studies during the pandemic were often less effective than they could have been because of design issues.

Pfizer is hoping to challenge other biologics in the competitive AD market, which is currently dominated by blockbuster sellers, Dupixent and Rinvoq.

American pharmaceutical giant Pfizer is planning to take its trispecific antibody, tilrekimig, to Phase III in atopic dermatitis (AD) as the company looks to grab a slice of this fast-growing market.

The company will progress tilrekimig following the positive outcome of a Phase II study (NCT02780167), in which 269 patients with moderate-to-severe AD were given either a low, medium or high once-monthly dose of tilrekimig or placebo over the course of 16 weeks.

Within the treatment period in the second stage of this study, tilrekimig triggered a significant increase in the percentage of patients achieving an eczema area and severity index (EASI)-75 score compared with placebo. Across the low, medium and high-dose cohorts, 38.7%, 51.9% and 49.4% of patients met this clinical milestone compared to placebo – meeting the trial’s primary endpoint. Pfizer is yet to disclose the percentage of patients that achieved EASI-75 in the placebo group.

Rates of treatment-emergent adverse events (TEAEs) across the tilrekimig cohorts were also comparable to placebo, with patients given the drug most commonly experiencing infections, skin disorders and injection site reactions. While three serious events were recorded in this study, all were considered unrelated to treatment.

DocTr uses patient encounter data and historical records to identify optimal trial sites before recruitment.

Finding the right physician for a clinical trial can be as difficult as finding the right patient, according to researchers who claim that a new AI-based technology matches doctors—and sites—to studies more effectively than current methods.

The technology—called DocTr—uses patient encounter data, information from unstructured trial documents, and historical records to find the investigators and sites that are the best fit for each study.

And the approach works, according to the authors, who shared details in a study in Nature Health last month.

“Tested rigorously on a large-scale dataset using regular and temporal splits, DocTr demonstrated superior performance. It achieved 58% higher similarity in clinician recommendations compared with the best baseline when evaluated on unseen, newly proposed trials. DocTr also showed robustness and versatility across all trial phases and disease categories,” the authors wrote.

However, according to the study, the primary benefit is that the technology is applied earlier in the recruitment process than traditional site-focused methods.

Datafolha survey investigated perceptions of four diseases, including Parkinson's and AIDS

A Datafolha survey commissioned by pharmaceutical company Eli Lilly and released on Monday (9) shows that Alzheimer's is the second most feared disease among Brazilians, behind only cancer and ahead of Parkinson's and AIDS. The data also shows that 4 in 10 Brazilians know someone with Alzheimer's.

The survey interviewed 2,002 people over the age of 16 in December last year, throughout Brazil. When asked which diagnosis they most feared would affect a relative or friend - between Alzheimer's, Parkinson's, AIDS, and cancer - 75% of respondents put cancer in first place.

Alzheimer's disease comes next, cited by 13% as the most feared; AIDS comes after that (9%), and Parkinson's disease is mentioned by 1% as their greatest fear.

According to experts interviewed by Folha, the fear of Alzheimer's disease is strongly linked to a lack of knowledge about the disease. Geriatrician Celene Pinheiro, president of Abraz (Brazilian Alzheimer's Association), says that stigma keeps people from seeking diagnosis and treatment.

“Fear of the disease causes many to avoid seeking help. There is still the idea that dementia is a natural part of aging, and that is not true. Whenever there are cognitive changes, it is necessary to investigate.”
