PRIORITY SERVICES

The Brazilian Registry of Clinical Trials (ReBEC) presents the new fast tracks for clinical research and observational studies on INDIGENOUS PEOPLE AND TRADITIONAL COMMUNITIES and on NEGLECTED TROPICAL DISEASES (NTDs). Fast-track can reduce the normal approval period for clinical trials to less than 48 hours, if the documentation, information and any mandatory changes requested are provided by the registrant.

The already existing fast-tracks and the attendance to other studies will not be interrupted. Priority care is granted with a view to accelerating clinical research in epidemiological emergency situations and/or studies with the potential to reduce socioeconomic inequalities, keeping the perspective aligned with the forefront of ethics and transparency, diplomacy in health, scientific collaboration and diversity and specificities of recruitment and health care.

FAST-TRACKS IN OPERATION:

I - Indigenous Peoples, Traditional Communities, Human Migration and Refugee Movements – clinical research and observational studies dedicated to or preferably addressing traditional communities (quilombola, caiçara, faxinalense, among others), Indigenous peoples (within or outside legally demarcated territories), and refugee groups, taking into account the specificities established by regulation, on a case-by-case basis.

II - Neglected Tropical Diseases – Leprosy, Tuberculosis, Leishmaniasis, Schistosomiasis, Scabies, Chagas disease, Intestinal parasitoses, Viral hepatitis, Trachoma, Onchocerciasis, Human rabies transmitted by dogs.

III - National and International Public Health Emergencies – Mpox, Covid-19.

Cochrane’s research looked at 17 clinical trials of anti-amyloid drugs in Alzheimer’s disease.

Eli Lilly and Eisai have backed their Alzheimer’s disease drugs despite research finding the mechanism of action may be less clinically meaningful than suggested.

A review by Cochrane found that the absolute effects of anti-amyloid drugs on cognitive decline and dementia severity were “absent or trivial,” falling “well below established thresholds for the minimum clinically important difference”.

Lead author Francesco Nonino, neurologist and epidemiologist at the IRCCS Institute of Neurological Sciences of Bologna, Italy, said: “There is now a convincing body of evidence converging on the conclusion that there is no clinically meaningful effect. While early trials showed results that were statistically significant, it is important to distinguish between this and clinical relevance. It is common for trials to find statistically significant results that do not translate into a meaningful clinical difference for patients.”

The review also found that anti-amyloid drugs likely increase the risk of swelling and bleeding in the brain, known as amyloid-related imaging abnormalities (ARIA). This was observed in brain scans without any apparent symptoms for most patients, although any long-term effects remain unclear since reporting of symptoms was inconsistent across trials. Funded by Yorkshire Cancer Research, EPIC-GB, a £6.9 million programme of clinical trials led by experts at the University of Sheffield and the University of Glasgow, aim to transform how cancer drug treatments are tested for people with recurrent glioblastoma, so they can access new and potentially life-extending treatment options.

New ground-breaking brain cancer clinical trials designed to improve survival and quality of life will be available in Yorkshire for people with glioblastoma, the most common type of brain cancer thanks to an innovative programme which will be co-led by Mr Ola Rominiyi, Neurological Resident at Sheffield Teaching Hospitals NHS Trust.

How the new trials will work
Clinical trials involving people with glioblastoma are urgently needed to find out if different cancer drugs are likely to get into the brain and work.

The EPIC-GB trials would enable people to start trial treatments before surgery. Tumour tissue removed during the operation can then be studied, creating a valuable 'window of opportunity' to quickly understand which drugs reach the tumour and are likely to be effective.

By identifying promising treatments as early as possible, people with recurrent glioblastoma can continue those treatments, or stop them early if they don't get into the brain.…

The UK has made progress in its efforts to slash set-up times for clinical trials, one of the factors blamed for a decline in industry-backed studies over the last few years.

New figures from the National Institute for Health and Care Research (NIHR) show an average set-up time of 122 days in the six months to March, down from 169 days in the same period of the previous year.

The finding suggests the government has met its target of getting UK clinical trials up and running within an average of 150 days by March 2026, from an earlier level of around 250 days, a feat which the NIHR has attributed to cutting "red tape and unnecessary paperwork."

Measures include standardising and mandating commercial contracting processes across the NHS, and the launch of the NIHR Life Sciences Industry Hub, which acts as a single front door for companies running trials in England.

The 150-day metric is intended to make the UK a faster, more predictable and easier place for life sciences companies to run high-quality clinical trials. However, long set-up times are just one factor that has contributed to the UK slipping behind some other countries as a hub for clinical research, and others – like low patient recruitment rates and rising costs – remain a drag on the sector, according to the pharma industry.

In addition, while regulatory approvals may be coming in more quickly, there are still time lags in opening studies for recruitment.

A new clinical trial at the University of Chicago is testing whether a smaller, symptom-focused surgery can help pleural mesothelioma patients maintain quality of life when full tumor removal isn’t possible. This study looks at surgery differently than most mesothelioma trials. Rather than focusing first on complete tumor removal, it asks whether a smaller operation can safely help patients feel better and function better.

The study is enrolling 30 patients with the epithelioid subtype of pleural mesothelioma. To be eligible, participants have tumors that can’t be fully removed surgically. Patients must also have already completed chemotherapy or immunotherapy. Instead of aiming for a full resection, surgeons will perform a limited partial pleurectomy and decortication designed to ease symptoms such as pain, fluid buildup and shortness of breath.

The regulator requires Lilly to evaluate the “unexpected serious” risks associated with Foundayo.

li Lilly has released more data for its obesity pill, Foundayo (orforglipron) in a rapid response to US regulators voicing certain reservations on the therapy’s safety.

In a new drug application (NDA) approval letter sent to Lilly by the US Food and Drug Administration (FDA), the agency requested that the pharma conduct clinical studies evaluating Foundayo’s safety profile across the general population, while homing in on the drug’s impact on younger paediatric and pregnant patients.

The FDA’s request comes as the agency airs concerns around early safety signals exhibited by Foundayo that cannot be dissipated through nonclinical or observational studies – including the alleged capacity for the drug to enhance a patient’s risk of major adverse cardiovascular events (MACE), drug-induced liver injury (DILI) and a type of thyroid cancer.

Since the FDA debuted this letter, Lilly has shared results of the Phase III ACHIEVE-4 trial (NCT05803421), which found that patients taking Foundayo were at a non-inferior level of risk for MACEs – including heart attack, stroke, cardiovascular death or sudden chest pain – compared with insulin glargine. There was also no safety signal around liver toxicity detected in this study.

Based on these results, the pharma is teeing up approval for Foundayo in type 2 diabetes, with plans to submit a new drug application (NDA) for the therapy to the FDA by the end of Q2 2026.
