PRIORITY SERVICES

The Brazilian Registry of Clinical Trials (ReBEC) presents the new fast tracks for clinical research and observational studies on INDIGENOUS PEOPLE AND TRADITIONAL COMMUNITIES and on NEGLECTED TROPICAL DISEASES (NTDs). Fast-track can reduce the normal approval period for clinical trials to less than 48 hours, if the documentation, information and any mandatory changes requested are provided by the registrant.

The already existing fast-tracks and the attendance to other studies will not be interrupted. Priority care is granted with a view to accelerating clinical research in epidemiological emergency situations and/or studies with the potential to reduce socioeconomic inequalities, keeping the perspective aligned with the forefront of ethics and transparency, diplomacy in health, scientific collaboration and diversity and specificities of recruitment and health care.

FAST-TRACKS IN OPERATION:

I - Indigenous Peoples, Traditional Communities, Human Migration and Refugee Movements – clinical research and observational studies dedicated to or preferably addressing traditional communities (quilombola, caiçara, faxinalense, among others), Indigenous peoples (within or outside legally demarcated territories), and refugee groups, taking into account the specificities established by regulation, on a case-by-case basis.

II - Neglected Tropical Diseases – Leprosy, Tuberculosis, Leishmaniasis, Schistosomiasis, Scabies, Chagas disease, Intestinal parasitoses, Viral hepatitis, Trachoma, Onchocerciasis, Human rabies transmitted by dogs.

III - National and International Public Health Emergencies – Mpox, Covid-19.

IV - Arboviruses – Zika, Dengue, Chikungunya, Malaria, Yellow fever.

Innovation strengthens China's role in nuclear medicine and improves the precision of tumor diagnosis

China recently approved an innovative nuclear medicine image composite, developed by a research team led by the University of Pequim. According to information from China Daily, along with BRICS TV, this is the world's first radioactive medicine specifically created to identify and generate images of tumors.

Specialists explain that the innovation is based on the compound's ability to identify a protein highly expressed in tumor cells and new blood vessels that feed the growth of tumors. According to researchers, the imaging agent is relatively simple to prepare and is suitable for routine clinical use.

This new development can identify a significant gap in the use of two Single Photon Emission Tomography (SPECT) systems for cancer diagnosis. Although molecular imaging is widely used in modern medicine, the lack of effective drugs for tumor-targeted imaging still limits the use of these systems. The approval of this new image agent represents an important step forward to overcome this limitation.

In recent decades, nuclear medicine doctors in China have carried out various studies using the drug, publishing almost 30 clinical research articles. In addition, we have developed a consensus among specialists on the effectiveness of compost in the treatment of lung, breast, esophageal and thyroid cancers.

By partnering with physicians and streamlining enrollment, the company is reshaping how patients access research opportunities.

Cedar Health Research launched in 2019 with a simple focus: to offer clinical trials as a legitimate healthcare option.

“Doctors were very interested in offering clinical research to their patients, but they just did not have the time,” Amy Walls, VP of engagement for Cedar, said. “We essentially became a concierge service for clinical research, both for the patient as well as for the doctors who wanted to offer these services for their patients.”

Upon its 2019 founding, Cedar linked with Arbor Diagnostics, a women’s health-focused medical lab. It had access to extensive lab data, but no chance to apply it at the time. That has changed: today, the organization works with nearly 200 physicians at 80 North Texas locations and 46 healthcare organizations. Most importantly, it reaches over 1.8 million patients in DFW through its physician network.

“Historically, the trials have been happening on the coasts in your big academic medical centers,” CEO Mimi Fenton explained. “When they have been in the community, they haven’t really focused on partnership instead of transaction. We’ve had situations in which the populations in clinical trials don’t reflect those who actually experience the disease. To be able to provide equitable access to clinical trials, you really need to be in the communities.”

The Dementia Trials Accelerator has taken a significant step towards transforming dementia clinical trials in the UK by welcoming its first participants into clinics for simple tests that will ultimately get the right people into clinical trials quicker, speeding up vital research.

Led by Health Data Research UK (HDR UK) and the UK Dementia Research Institute (UK DRI), the Dementia Trials Accelerator aims to address one of the biggest barriers to progress in dementia research: the chronic under-recruitment to clinical trials. In some cases, it can take up to three years to recruit enough participants to run an 18-month dementia trial – while the average cancer trial takes 2.3 years from start to completion, including the recruitment phase.

Over 15,500 participants aged 65-75 from REACT – one of the UK's largest cohort studies involving 2.7 million adults and led by Imperial College London – have been invited to the first clinics run by the Dementia Trials Accelerator's delivery partner, Inuvi. Those that come forward undertake cognitive tests and provide a blood sample, as well as having their height, weight, and blood pressure measured. Blood samples are then tested for biomarkers that have been linked to dementia risk. More than 800 people have taken part in just the first few weeks.

By early 2027, the Dementia Trials Accelerator aims to have over 10,000 participants who want to take part in future research to combat this devastating disease. Artificial intelligence has become a familiar topic in clinical trial discussions. Sponsors increasingly ask about automation, predictive insights, and smarter systems as a signal of progress. AI represents modernity and momentum, especially in an environment where timelines compress and complexity grows. Yet, in those same conversations, minimization, a proven adaptive randomization method used successfully for decades, often receives hesitation or rejection. That contrast exposes a quiet but important paradox in how clinical trials approach intelligence.

Minimization, particularly when based on the Pocock and Simon algorithm, has supported balanced treatment allocation since the 1970s. It prioritizes balance across multiple, often prognostic, factors while retaining randomness to protect against selection bias. Unlike many AI-driven concepts, minimization is fully explainable, statistically transparent, and regulator accepted. It does not rely on opaque models or post hoc interpretation. Still, many trials default to permuted block designs, even as enrollment patterns and stratification demands continue to challenge their effectiveness.

This hesitation rarely stems from a lack of evidence. It comes from familiarity and perceived risk. Permuted block designs feel safe because teams understand them well, even when those designs strain under real-world conditions. AI, by contrast, feels aspirational.
