PRIORITY SERVICES

The Brazilian Registry of Clinical Trials (ReBEC) presents the new fast tracks for clinical research and observational studies on INDIGENOUS PEOPLE AND TRADITIONAL COMMUNITIES and on NEGLECTED TROPICAL DISEASES (NTDs). Fast-track can reduce the normal approval period for clinical trials to less than 48 hours, if the documentation, information and any mandatory changes requested are provided by the registrant.

The already existing fast-tracks and the attendance to other studies will not be interrupted. Priority care is granted with a view to accelerating clinical research in epidemiological emergency situations and/or studies with the potential to reduce socioeconomic inequalities, keeping the perspective aligned with the forefront of ethics and transparency, diplomacy in health, scientific collaboration and diversity and specificities of recruitment and health care.

FAST-TRACKS IN OPERATION:

I - Indigenous Peoples, Traditional Communities, Human Migration and Refugee Movements – clinical research and observational studies dedicated to or preferably addressing traditional communities (quilombola, caiçara, faxinalense, among others), Indigenous peoples (within or outside legally demarcated territories), and refugee groups, taking into account the specificities established by regulation, on a case-by-case basis.

II - Neglected Tropical Diseases – Leprosy, Tuberculosis, Leishmaniasis, Schistosomiasis, Scabies, Chagas disease, Intestinal parasitoses, Viral hepatitis, Trachoma, Onchocerciasis, Human rabies transmitted by dogs.

III - National and International Public Health Emergencies – Mpox, Covid-19.

IV - Arboviruses – Zika, Dengue, Chikungunya, Malaria, Yellow fever.

In 1747, James Lind conducted one of the earliest comparative trials, testing oranges and lemons to treat scurvy in sailors. In 1948, Austin Bradford Hill designed the first modern randomised controlled trial (RCT), testing streptomycin and bedrest compared to bedrest alone for treating pulmonary tuberculosis.

Eighty years later, the RCT – more evolved, but with its fundamental characteristics (randomisation, control groups, and blinding) intact – remains the highest level of evidence to establish causal relationships in science.

For evaluating efficacy and safety in pharmaceutical drugs and medical devices, the RCT is considered the gold standard. And yet, it’s not perfect.

In addition to the associated high costs, potential feasibility issues, and ethical restrictions for certain interventions, RCTs often have a problem with generalisability.

Despite efforts to improve patient diversity, gaps still exist between the participant pool for clinical trials and the population at large. Patients outside of trials often have characteristics, experiences, and treatment protocols that differ from the controlled environment of RCTs, meaning the results may not apply to broader, more diverse patient populations.

UT Health San Antonio Multispecialty and Research Hospital has enrolled and treated its first clinical trial participant in a novel cancer study, marking a pivotal moment for science and patient-centered cancer care in San Antonio and South Texas.

The hospital, which opened its doors on Dec. 10, 2024, is part of UT Health San Antonio, the academic health center of The University of Texas at San Antonio, bringing a unique integration of patient care, research and education as the region’s only academic health center.

“Treating our first clinical trial participant at the UT Health San Antonio Multispecialty and Research Hospital is a defining milestone. It demonstrates that we are not only a hospital that delivers high-quality care, but one that actively advances it,” said W. Allen Fink, DO, MHA, chief medical officer of the hospital. “One important tenet of our mission is to transform healthcare through healing discovery. Researchers from the University of Oslo (UiO), Akershus University Hospital (Achus), and international collaborators have published a new specialized review on Nature's Aging. The article brings together more than 25 scientists, including physicians and leading experts in the biology of aging and metabolism, with a strong focus on nicotinamide adenine dinucleotide (NAD+). Their combined understanding underscores a growing global effort to understand how this small molecule can support healthier aging and help protect against diseases such as Alzheimer's and Parkinson's.

NAD+ is often called the cell's "fuel regulator" because it plays a central role in energy production, DNA repair, and maintaining normal cellular function. However, NAD+ levels naturally decline as people age. This decline has been associated with memory problems, decreased muscle strength, and an increased risk of age-related diseases.

"Fine-tuning NAD+ metabolism shows promise in slowing age-related health decline, as well as diseases like premature aging," says Dr. Jianing Zhang, one of the lead authors. “But to truly unlock its potential, we need a better understanding of the correct dosages, long-term safety, and interindividual variability in response to NAD+ enhancement strategies.”

The House of Lords Science and Technology Committee will investigate how artificial intelligence can transform personalized medicine delivery.

The UK government announced it will examine how artificial intelligence (AI) can aid health innovation—including in clinical trials—in an enquiry launched this month.

The House of Lords Science and Technology Committee revealed the initiative as part of a wider investigation into the role tech plays in the UK’s public health system, explaining that personalized medicine and AI would be a focus.

Committee chair Lord Mair said, “As the NHS plans to harness new developments in genomics, AI, and personalized medicine, our inquiry will seek to establish the state of the science and technology in this area and understand where patients might benefit from near-term developments. 

“We will examine the gap between early-stage research, clinical trials and NHS-wide delivery, looking at blockages in the system slowing progress, including procurement processes, clinical pathways, and the role of regulators and professional bodies.”

The inquiry is part of the Labour government’s AI for science strategy, one of whose aims is to use the technology “to accelerate drug discovery to develop trial-ready drugs within 100 days by 2030 and contribute to deploying new treatments faster.”

Human oversight remains crucial as the technology is leveraged in safety monitoring, boosting clinical trial diversity and more.

When a drug fails, it’s not always because it doesn’t work. In many cases, the problem is linked to a limitation in trial design. This is especially true in mental health research, where studies of conditions like depression, and post-traumatic stress disorder, rely on subjective symptom ratings, not objective measures such as tumor size or blood markers — a challenge that can introduce bias.

Symptom ratings are sometimes inflated by investigators who may be eager to help a patient meet study criteria, according to research published in Journal for Clinical Studies. Tweaking these numbers may boost enrollment, but could lessen the chances a drug will succeed. When baseline scores are artificially high, it becomes harder to see the actual difference between the drug and the control group, said Gary Zammit, founder of Clinilabs, a central nervous system-focused clinical research organization.  

This long-held hurdle to improving outcomes is one of the many issues in clinical trials AI is being tapped to fix. 

“AI models not only seem to be more accurate in their ratings, but they help to eliminate the biases that investigators might bring to assessment,” Zammit said. 

AI is making its mark in drug development in other ways as well.

Promoting safety and expanding diversity

AI holds substantial potential in safety monitoring as well and is being used to mine trial data to detect safety signals before patients experience an adverse event, Zammit said.
