PRIORITY SERVICES

The Brazilian Registry of Clinical Trials (ReBEC) presents the new fast tracks for clinical research and observational studies on INDIGENOUS PEOPLE AND TRADITIONAL COMMUNITIES and on NEGLECTED TROPICAL DISEASES (NTDs). Fast-track can reduce the normal approval period for clinical trials to less than 48 hours, if the documentation, information and any mandatory changes requested are provided by the registrant.

The already existing fast-tracks and the attendance to other studies will not be interrupted. Priority care is granted with a view to accelerating clinical research in epidemiological emergency situations and/or studies with the potential to reduce socioeconomic inequalities, keeping the perspective aligned with the forefront of ethics and transparency, diplomacy in health, scientific collaboration and diversity and specificities of recruitment and health care.

FAST-TRACKS IN OPERATION:

I - Indigenous Peoples, Traditional Communities, Human Migration and Refugee Movements – clinical research and observational studies dedicated to or preferably addressing traditional communities (quilombola, caiçara, faxinalense, among others), Indigenous peoples (within or outside legally demarcated territories), and refugee groups, taking into account the specificities established by regulation, on a case-by-case basis.

II - Neglected Tropical Diseases – Leprosy, Tuberculosis, Leishmaniasis, Schistosomiasis, Scabies, Chagas disease, Intestinal parasitoses, Viral hepatitis, Trachoma, Onchocerciasis, Human rabies transmitted by dogs.

III - National and International Public Health Emergencies – Mpox, Covid-19.

IV - Arboviruses – Zika, Dengue, Chikungunya, Malaria, Yellow fever.

VII - Studies on the impacts of climate change, the effects of scientific denialism, and health misinformation -The physical, mental, and social consequences of environmental and climate changes on human populations; The impact of extreme weather events and environmental degradation on infectious, chronic, or emerging diseases; The influence of denialist narratives and misinformation in Health on adherence to preventive measures, vaccination, treatments, and risk behaviors, as well as the physical and mental outcomes for human beings and populations.

In February, the Government published the National Cancer Plan, the first dedicated cancer strategy for England in a decade. With a new Government now responsible for the Plan’s implementation, it is vital that the Plan remains a priority and that its ambitions are delivered at pace. In this series, we offer our perspective on a few key features within the Plan and what actions are needed to translate its commitments into genuine improvements for people affected by cancer.

In this article, Policy Manager Emily Eagles focuses on clinical research, exploring whether the National Cancer Plan’s proposals on clinical trials are enough to create a truly research-powered NHS.

In February, the Government published its long-awaited National Cancer Plan for England, which aims to boost cancer survival to be among the best in the world by 2035.

We were pleased that alongside measures to improve cancer care (which we’ll discuss in future articles), the Plan set out concrete steps to boost research, development and innovation across the NHS.

This is vital. Future tests and treatments won’t reach patients by chance; they need to be built on long-term scientific discovery and a strong clinical trials system.

But in recent years, following shocks like the COVID-19 pandemic, the clinical research environment in England has struggled. In 2023, an influential government-commissioned review by Lord O’Shaughnessy found that the UK had fallen behind other comparable countries on several criteria.

Moderna, Inc. (NASDAQ:MRNA) today announced that following Health Canada's authorization of the study, the first participants have been vaccinated in a Phase 1 clinical study in Canada evaluating mRNA-1469, the Company's investigational mRNA vaccine being evaluated to prevent Bundibugyo ebolavirus, or BDBV, disease.

The vaccine candidate is being developed using Moderna's mRNA platform, the same technology that demonstrated rapid development, scalability, and global deployment capabilities during the COVID-19 pandemic. The program also builds on Moderna's existing research and development efforts in filoviruses, including Ebola-related viruses.

Mayo Clinic researchers have developed an experimental nanotherapy that delivers two cancer drugs directly to brain tumors, according to a study published in *Nature Communications Medicine*. The strategy extended survival in preclinical models of glioblastoma, the most aggressive form of brain cancer.

The nanotechnology-based approach encapsulates two existing cancer drugs within tiny particles designed to cross the brain's protective blood-brain barrier and reach tumor cells. In preclinical models using patient-derived tissue, combining the treatment with radiation therapy more than doubled survival compared to untreated controls.

Glioblastoma is notoriously difficult to treat. Patients typically survive about 15 months after diagnosis, even with state-of-the-art therapies such as surgery, radiation, and chemotherapy. A major challenge is that many drugs cannot effectively reach brain tumors, and those that do often lose their effectiveness as tumors develop resistance.

The new approach uses small lipid-based particles, known as liposomes, to transport and deliver a drug combination—everolimus or rapamycin and vinorelbine—directly to cancer cells, employing a novel tumor-targeting strategy. By ensuring that both drugs reach the same cells simultaneously, researchers aim to enhance the anti-tumor effect while reducing the toxic side effects associated with higher doses.

A complete COVID-19 vaccination series, including booster doses, induces an immune response in 99.8% of institutionalized older adults in Belo Horizonte

Completing the vaccination series and receiving booster doses are essential strategies for ensuring safety and controlling viral infections. This is demonstrated by a study conducted by the Graduate Program in Applied Sciences for Adult Health at the UFMG School of Medicine. The research, led by geriatrician and researcher Flávia Lanna, found that the complete COVID-19 vaccination series, along with booster doses, is essential for an immune response, even in vulnerable, institutionalized older adults.

The researcher became involved in the topic as a participant in a care project at Long-Term Care Facilities for the Elderly (ILPIs) carried out in partnership with the Hospital das Clínicas (HC), which was invited by the Department of Health to assist in controlling the spread of COVID-19 in ILPIs during the pandemic.

The evaluation of the participants was conducted in three phases following the collection of biological samples from the participants. The first sample collection took place 45 days after completion of the primary vaccination series (two doses); the second, approximately five and a half months after the second dose; and the third, after the booster doses.

The rapid development of blood-based biomarkers has continued to reshape the Alzheimer disease (AD) landscape, offering clinicians increasingly accessible tools to aid in the detection of underlying pathology. Among the most closely studied biomarkers is phosphorylated tau 217 (p-tau217), which has demonstrated strong performance in identifying AD pathology and has increasingly moved toward clinical implementation. As these tests become more widely available, questions surrounding their appropriate use, interpretation, and integration into existing diagnostic pathways have become increasingly important.
