PRIORITY SERVICES

The Brazilian Registry of Clinical Trials (ReBEC) presents the new fast tracks for clinical research and observational studies on INDIGENOUS PEOPLE AND TRADITIONAL COMMUNITIES and on NEGLECTED TROPICAL DISEASES (NTDs). Fast-track can reduce the normal approval period for clinical trials to less than 48 hours, if the documentation, information and any mandatory changes requested are provided by the registrant.

The already existing fast-tracks and the attendance to other studies will not be interrupted. Priority care is granted with a view to accelerating clinical research in epidemiological emergency situations and/or studies with the potential to reduce socioeconomic inequalities, keeping the perspective aligned with the forefront of ethics and transparency, diplomacy in health, scientific collaboration and diversity and specificities of recruitment and health care.

FAST-TRACKS IN OPERATION:

I - Indigenous Peoples, Traditional Communities, Human Migration and Refugee Movements – clinical research and observational studies dedicated to or preferably addressing traditional communities (quilombola, caiçara, faxinalense, among others), Indigenous peoples (within or outside legally demarcated territories), and refugee groups, taking into account the specificities established by regulation, on a case-by-case basis.

II - Neglected Tropical Diseases – Leprosy, Tuberculosis, Leishmaniasis, Schistosomiasis, Scabies, Chagas disease, Intestinal parasitoses, Viral hepatitis, Trachoma, Onchocerciasis, Human rabies transmitted by dogs.

III - National and International Public Health Emergencies – Mpox, Covid-19.

IV - Arboviruses – Zika, Dengue, Chikungunya, Malaria, Yellow fever.

V - WHO Prioritized Pathogens for Latin America – Studies dealing with World Health Organization prioritized pathogens for Latin America, as listed in the document "PATHOGENS PRIORITIZATION - A SCIENTIFIC FRAMEWORK FOR EPIDEMIC AND PANDEMIC RESEARCH PREPAREDNESS" (Health Emergencies Programme, 2024) – Lassa fever, Cholera, Black Death (Plague), Bacillary dysentery or Shigellosis, Salmonella, Pneumonia, Urinary tract infection, MERS-CoV, SARS, Ebola virus (EVE), Marburg virus (MVE), Ebola virus (EVE) Sudan subtype, Hantavirus pulmonary syndrome (HPS), Hemorrhagic fever with renal syndrome (HFRS), Crimean-Congo hemorrhagic fever, Influenza A H1, Influenza A H2, Influenza A H3, Avian influenza (Influenza A H5N1), Avian influenza (Influenza A H6), Avian influenza H7 (Avian influenza H7), Avian influenza H10 or Avian influenza H10, Nipah disease, Viral hemorrhagic disease, Poliomyelitis, Hand-foot-and-mouth disease, Herpangina, Viral meningitis, Smallpox, Mpox, Human Immunodeficiency Virus (HIV), Venezuelan equine encephalitis (VEE).

VII - Studies on the impacts of climate change, the effects of scientific denialism, and health misinformation -The physical, mental, and social consequences of environmental and climate changes on human populations; The impact of extreme weather events and environmental degradation on infectious, chronic, or emerging diseases; The influence of denialist narratives and misinformation in Health on adherence to preventive measures, vaccination, treatments, and risk behaviors, as well as the physical and mental outcomes for human beings and populations.

1. Register or log in and normally fill out the forms in Portuguese and English. For English, in case of difficulty, use the Google Translate tool (or another free translation tool).
2. Attach the mandatory ethics committee documentation and submit the form.
3. Register the service number on your Whatsapp. Send a message with your name and login. IT IS NOT NECESSARY NOR RECOMMENDED TO SEND YOUR PASSWORD.
4. A reviewer will respond to you. Choose the service modality: chat, audio recordings or voice call by appointment.
5. Your form will be proofread in both languages. Follow the remaining guidelines to finalize the approval.

When someone wants to participate in a clinical trial, finding the right opportunity shouldn't feel like a research project of its own. Thanks to the work of Kiley Vander Wyst, PhD, MPH, and a cross-campus team, it's becoming much easier for community members to discover clinical research opportunities at CU Anschutz.

As assistant director of clinical trials operations, Vander Wyst oversees the Clinical Research Recruitment Program, which supports research teams across CU Anschutz, UCHealth and Children's Hospital Colorado. The program helps investigators connect with potential study participants through services like the research studies website, social media campaigns, the Study Spotlight newsletter and educational workshops focused on recruitment best practices.

"The biggest challenge for many clinical trials isn't the science – it's recruitment," Vander Wyst said. "We have incredible research happening across campus, but helping the right people learn about those opportunities can be one of the most time-consuming and costly parts of the process."

Grove AI’s technology uses voice agents to recruit eligible patients for clinical trials. Formed in 2024, the startup’s rapid growth led to its acquisition by Hippocratic AI earlier this year.

Physicians may be able to suggest a trial or two, but it’s hard for them to keep up with all of the drugs in clinical development, said Tran Le, co-founder of Grove AI. More often than not, patients are directed to conduct their own searches on a clunky federal database of clinical trials.

Grove AI gives patients an easier and more intuitive alternative: Just pick up the phone and talk.

“When we started two years ago and we pitched our simple voice call agent to the industry, people told us we were dreaming and we were crazy and that it would never happen,” Le said. “Now, we get trial protocols from [clinical trial] sponsors. And in these trial protocols, they write down exact tasks that the [AI] agent should do and that the humans should do.”

Grove’s technology uses AI agents to automate many clinical trial recruitment tasks that have historically been handled by a human. These agents interact with patients, helping them find the clinical trial that’s a fit for their disease. Whereas a human recruiter recruits for one trial at a time, a Grove agent has information about many trials and can recruit for multiple studies. Clinical trials now generate 5.9 million datapoints per Phase III protocol — and most sponsors are still managing them manually.

Real-time data (RTD) offers exciting opportunities to empower clinical trial teams with the trusted, actionable insight that is vital for successful and meaningful research. It allows sponsors to react quickly to bottlenecks and potential issues like deviation from study protocol––before they jeopardize trial integrity.

But, with teams already grappling with data management and analysis challenges, there are significant hurdles which need to be overcome––including analysis capacity, data integration and consistency––if the true potential of RTD is unlocked.

On Thursday, Portugal authorized its first clinical trial as a Reporting Member State under the European pilot project "Facilitating and Accelerating Strategic Clinical Trials in the EU/EEA" (FAST-EU). This move meets the new, shortened 70-day deadline and reinforces the country's position in implementing this European initiative.

FAST-EU is a joint initiative by the Heads of Medicines Agencies (HMA), the Clinical Trials Coordination Group (CTCG), and MedEthics EU. Developed within the scope of the "Accelerating Clinical Trials in the European Union" (ACT EU) project, it tests a new assessment model designed to accelerate the authorization of multinational clinical trials in the European Union.

Portugal is participating in eight of the 23 clinical trials selected for submission via this European mechanism, highlighting Infarmed's strong involvement in the pilot project's implementation. To date, five clinical trials have been authorized, and three are currently under assessment; the new 70-day deadline is expected to be met for these, representing a significant reduction compared to the current regulatory timeframe of 106 days.

The FAST-EU pilot project anticipates some of the changes to the European Clinical Trials Regulation planned under the future European legislative initiative for biotechnology (the "Biotech Act").
