PRIORITY SERVICES

The Brazilian Registry of Clinical Trials (ReBEC) presents the new fast tracks for clinical research and observational studies on INDIGENOUS PEOPLE AND TRADITIONAL COMMUNITIES and on NEGLECTED TROPICAL DISEASES (NTDs). Fast-track can reduce the normal approval period for clinical trials to less than 48 hours, if the documentation, information and any mandatory changes requested are provided by the registrant.

The already existing fast-tracks and the attendance to other studies will not be interrupted. Priority care is granted with a view to accelerating clinical research in epidemiological emergency situations and/or studies with the potential to reduce socioeconomic inequalities, keeping the perspective aligned with the forefront of ethics and transparency, diplomacy in health, scientific collaboration and diversity and specificities of recruitment and health care.

FAST-TRACKS IN OPERATION:

I - Indigenous Peoples, Traditional Communities, Human Migration and Refugee Movements – clinical research and observational studies dedicated to or preferably addressing traditional communities (quilombola, caiçara, faxinalense, among others), Indigenous peoples (within or outside legally demarcated territories), and refugee groups, taking into account the specificities established by regulation, on a case-by-case basis.

II - Neglected Tropical Diseases – Leprosy, Tuberculosis, Leishmaniasis, Schistosomiasis, Scabies, Chagas disease, Intestinal parasitoses, Viral hepatitis, Trachoma, Onchocerciasis, Human rabies transmitted by dogs.

III - National and International Public Health Emergencies – Mpox, Covid-19.

IV - Arboviruses – Zika, Dengue, Chikungunya, Malaria, Yellow fever.

Moderna (Nasdaq: MRNA) has dosed the first participants in the first clinical trial of a vaccine specifically targeting Bundibugyo ebolavirus (BDBV). The mRNA candidate enters development as the company and the Coalition for Epidemic Preparedness Innovations (CEPI) prepare for potential accelerated advancement during the ongoing outbreak. The trial comes as the BDBV outbreak has surpassed 3,000 confirmed cases and 1,400 deaths — already the second-largest filovirus outbreak on record.

The Phase I study, authorized by Health Canada and running across three Canadian sites, will enroll approximately 80 healthy adult participants. Primary objectives are safety, tolerability, and immunogenicity. CEPI has committed up to USD 50 million to support preclinical work and the Phase I trial under an expanded collaboration with Moderna, and the company said it is manufacturing additional clinical doses in parallel with early-stage testing to accelerate potential Phase II and Phase III initiation if Phase I results support advancement.

mRNA-1469 uses Moderna's lipid nanoparticle-delivered mRNA platform to direct host cells to express the BDBV surface glycoprotein, the primary antigen for neutralizing antibody induction. The speed at which clinical trials can be activated in an outbreak is essential to an efficient outbreak R&D response. That’s why, together with the Africa Centres for Disease Control and Prevention (Africa CDC), the Coalition for Epidemic Preparedness Innovations (CEPI) has published a database of comprehensive information on 45 African clinical trial units across 17 African countries' capability to conduct Phase 1 trials to accelerate development of safe, effective Bundibugyo ebolavirus vaccines.

The publicly accessible resource will help to connect leading African research institutions with Bundibugyo vaccine developers seeking to conduct early-stage clinical trials on the continent. Data were gathered through a Request for Information, which invited institutions across the continent to submit information.

Details on each site, including self-reported prior experience, facilities, regulatory track record and operational capabilities, are available here.

Legal challenge to King’s College London research study claimed it could harm participants but was rejected

A clinical trial of drugs that can delay puberty can go ahead after a high court judge threw out a legal challenge on Friday.

Campaigners had launched the attempt to halt the planned research into puberty blockers, to be run by researchers at King’s College London (KCL), on the grounds that they believed it could harm the children taking part.

But Mr Justice Chamberlain refused the application for a judicial review, saying none of their claims had a realistic chance of success in court. He said each patient involved in the Pathways trial would have been assessed over “many months” by specialist doctors who would have “formed the opinion that a child has a reasonable prospect of receiving a clinical benefit from the treatment”.

The university welcomed the court’s decision on Friday, saying particular care would be taken over the safety of participants of the trial, which will start recruiting more than 220 children aged between 11 and 16 years old from 1 August.

But opponents, including the psychotherapist James Esses and Keira Bell, who began taking puberty blockers as a teenager before later detransitioning, expressed sadness at the ruling.

Bell told the BBC: “It’s a tragedy that the court has refused permission for this challenge to proceed. The medical profession and morality do not always walk hand in hand. If they did, this trial would be out of the question.”

In February, the Government published the National Cancer Plan, the first dedicated cancer strategy for England in a decade. With a new Government now responsible for the Plan’s implementation, it is vital that the Plan remains a priority and that its ambitions are delivered at pace. In this series, we offer our perspective on a few key features within the Plan and what actions are needed to translate its commitments into genuine improvements for people affected by cancer.

In this article, Policy Manager Emily Eagles focuses on clinical research, exploring whether the National Cancer Plan’s proposals on clinical trials are enough to create a truly research-powered NHS.

In February, the Government published its long-awaited National Cancer Plan for England, which aims to boost cancer survival to be among the best in the world by 2035.

We were pleased that alongside measures to improve cancer care (which we’ll discuss in future articles), the Plan set out concrete steps to boost research, development and innovation across the NHS.

This is vital. Future tests and treatments won’t reach patients by chance; they need to be built on long-term scientific discovery and a strong clinical trials system.

But in recent years, following shocks like the COVID-19 pandemic, the clinical research environment in England has struggled. In 2023, an influential government-commissioned review by Lord O’Shaughnessy found that the UK had fallen behind other comparable countries on several criteria.
