PRIORITY SERVICES

The Brazilian Registry of Clinical Trials (ReBEC) presents the new fast tracks for clinical research and observational studies on INDIGENOUS PEOPLE AND TRADITIONAL COMMUNITIES and on NEGLECTED TROPICAL DISEASES (NTDs). Fast-track can reduce the normal approval period for clinical trials to less than 48 hours, if the documentation, information and any mandatory changes requested are provided by the registrant.

The already existing fast-tracks and the attendance to other studies will not be interrupted. Priority care is granted with a view to accelerating clinical research in epidemiological emergency situations and/or studies with the potential to reduce socioeconomic inequalities, keeping the perspective aligned with the forefront of ethics and transparency, diplomacy in health, scientific collaboration and diversity and specificities of recruitment and health care.

FAST-TRACKS IN OPERATION:

I - Indigenous Peoples, Traditional Communities, Human Migration and Refugee Movements – clinical research and observational studies dedicated to or preferably addressing traditional communities (quilombola, caiçara, faxinalense, among others), Indigenous peoples (within or outside legally demarcated territories), and refugee groups, taking into account the specificities established by regulation, on a case-by-case basis.

II - Neglected Tropical Diseases – Leprosy, Tuberculosis, Leishmaniasis, Schistosomiasis, Scabies, Chagas disease, Intestinal parasitoses, Viral hepatitis, Trachoma, Onchocerciasis, Human rabies transmitted by dogs.

III - National and International Public Health Emergencies – Mpox, Covid-19.

The NIHR-funded trial will be one of the first and largest nutritional emergency surgery trials of its kind.

A major new UK-wide clinical trial led by the University of Birmingham will investigate whether early nutritional support can improve recovery and reduce complications in patients undergoing emergency abdominal surgery.

The TONIC Trial (Timing Of Nutritional Intervention after emergency laparotomy/laparoscopy) will evaluate whether providing early Parenteral Nutrition (PN), or nutritional supplementation delivered directly into the bloodstream, within the first 48 hours after surgery can lead to better outcomes than current standard care.

Each year, approximately 21,000 patients in England and Wales undergo emergency abdominal operations such as laparotomy to treat life‑threatening conditions. Although survival rates have improved over the past decade, national data show that 90‑day mortality has plateaued at around 9.6% since 2021, highlighting the need for innovative approaches to support recovery.

Why nutrition matters after emergency surgery
Many patients struggle to eat in the days following major abdominal surgery, putting them at significant risk of malnutrition, which can slow wound healing, increase infection risk, and prolong hospital stay.

A new analysis of a large international clinical trial finds semaglutide, a compound found in weight loss medications Wegovy and Ozempic, reduces major heart events and lowers fatty liver disease risk in adults with both obesity and heart disease. 

People with obesity face a higher risk of heart attacks and strokes. Many also have fatty liver disease that can lead to liver scarring. A common liver disease, metabolic dysfunction-associated steatotic liver disease (MASLD), affects about 4 out of 10 adults and is closely linked to obesity, type 2 diabetes and high blood pressure. 

Researchers at Virginia Commonwealth University and around the world are studying the effects of semaglutide on both heart and liver diseases. VCU hepatologist Arun Sanyal, M.D., and colleagues last year published results from the “Effect of Semaglutide in Subjects with Non-cirrhotic Non-alcoholic Steatohepatitis” (ESSENCE) clinical trial, demonstrating that it improves fatty liver disease and lowers the risk factors associated with heart disease.

A new analysis of a separate clinical trial, “Semaglutide Effects on Cardiovascular Outcomes in People with Overweight or Obesity” (SELECT), shows that semaglutide indeed lowers cardiac risks in those who also have fatty liver disease, even those who have advanced liver scarring. The results were recently published in the journal Nature Medicine; Sebastian M. Meyhöfer of Novo Nordisk Pharma GmbH, in Germany, is the paper’s corresponding author and Sanyal is a co-author.

Clinical trials are essential in bringing new, improved treatment options to patients. They support our hospitals, bringing new infrastructure and skills, and anchor research in Europe.

The pharmaceutical sector is the engine room that drives clinical trial activity and the benefits that go way beyond patient care. Clinical research delivers significant economic growth, and the European Union has the potential to deliver so much more.

That’s why it’s good news that the European Medicines Agency (EMA), alongside the European Commission and Heads of Medicines Agencies (HMA), have set a new target for increasing clinical trials in the EU. The 11.1 percent target should bring an additional 500 multinational trials over five years.

A new report by Frontier Economics, published in February by EFPIA, analyzed this target. It will produce an extra 35,000 clinical trial places for people in Europe, who will be able to access treatments that may otherwise have been unavailable. The economic impact is estimated to be around €4 billion. It’s certainly a good first step toward turning around a decade of decline.

Spain, where Almirall is based, has shown what is possible when the right conditions are in place. In 2023 it overtook Germany as the country with the most clinical trial starts across Europe. Sleep data captured by a wearable device may help doctors better personalize care by identifying patients with chronic obstructive pulmonary disease (COPD) who may need additional support to participate in pulmonary rehabilitation, according to new research published in Mayo Clinic Proceedings: Digital Health.

COPD is a chronic lung disease that makes breathing difficult after the airways become inflamed and narrowed, leading to mucus buildup. COPD can also make it difficult to sleep, affecting the patient’s energy levels and overall health. These factors can influence participation in pulmonary rehabilitation, which includes a combination of exercise, education, and support.

The researchers sought to understand whether a patient’s sleep quality could help predict their level of participation in remote rehabilitation activities.

“As a scientist and engineer, I wanted to explore how data from wearable devices could improve dropout rates from remote pulmonary rehabilitation programs. By better understanding a patient’s daily life, we can develop recommendations for more personalized and potentially more effective care plans,” says Stephanie Zawada, Ph.D., M.S., a research associate at Mayo Clinic and the study’s first author. Experts highlight the country’s potential to conduct studies from the earliest stages and strengthen Brazilian science in the sector

Brazil has been gaining prominence on the global medical cannabis scene by advancing clinical research with its own leadership. Unlike the traditional model of the pharmaceutical industry, in which the country typically participates only in advanced phases of international studies, cannabis opens the door for research to be developed from the outset within the country.

In an interview with Sechat’s Deusa Cast podcast, experts Helena Joaquim, medical and scientific manager, and Alana Costa, scientific coordinator at Green Care, analyzed the sector’s current state and the opportunities emerging for Brazilian science.

According to Helena, a solid academic background—with master’s and doctoral degrees—remains a key differentiator within the industry. “We came from academia and brought that essence to the market. Cannabis is a field where we can apply everything we’ve built, including our scientific philosophy,” she stated.

Science from the Ground Up
One of the key points highlighted is the possibility of conducting full clinical trials in Brazil, starting from the initial phases. This development represents a paradigm shift in the national pharmaceutical sector.
