A new sarcoma clinical trial bridges adolescent and adult care at the MUHC

Highlighting the MUHC’s unique ability to bridge pediatric and adult cancer care, the trial evaluates a new antibody-drug conjugate for the treatment of osteosarcoma and soft tissue sarcomas in adolescents and adults.

Adolescents and adults with difficult-to-treat sarcomas can now access a new investigational treatment at the McGill University Health Centre (MUHC), through the Phase Ib/II EMBOLD Sarcoma clinical trial. The study brings together the MUHC’s pediatric and adult oncology teams to offer the same trial to patients aged 12 and older, an approach made possible by the MUHC’s unique continuum of cancer care. The trial is locally coordinated by the Research Institute of the MUHC (The Institute).

Sarcoma is a rare cancer that primarily affects adolescents and young adults. Approximately 2,000 new cases are diagnosed each year in Canada and despite advances made in recent decades, treatment options remain limited.

“Clinical trials in sarcoma are particularly valuable. These cancers account for only about one per cent of cancers in adults and encompass many subtypes that affect both adolescents and adults, making participant recruitment and the generation of robust evidence particularly challenging. Modifiable health behaviours account for nearly half of deaths in the United States, with the burden of modifiable chronic disease being especially significant among those aged 60 years or older. Despite the importance of modifying health behaviours to improve health and reduce mortality in older adults, large-scale, stable health behaviour change has been difficult to achieve. Well-designed behavioural interventions are critical to supporting consistent health behaviour change.

The goal of the National Institute of Health’s (NIH) National Institute on Aging (NIA) is to support innovative research that fosters the health and independence of older adults while studying the conditions that uniquely affect the most vulnerable among America’s ageing population. The Edward R. Roybal Centers for Translational Research in the Behavioural and Social Sciences of Aging are a NIA-supported consortium of 15 Centers created to help achieve this goal. In 2020, the average phase 3 protocol collected approximately 3.56 million data points. By 2025, that figure had reached about 5.96 million, a five-year increase of 67% and 6.4 times the 2012 average of 929,203, according to collaborative research from the nonprofit industry group TransCelerate BioPharma and the Tufts Center for the Study of Drug Development. The study found potential room for greater discipline as nearly one-third of phase 3 procedures and associated data were classified as non-core or non-essential. The peer-reviewed paper also noted that AI and ML processing power “may be serving as a disincentive to reduce data volume.”

Nearly eight months later, that observation about AI looks prescient. “I don’t think [sponsors] are as worried about collecting too much data,” said Venu Mallarapu, chief transformation and AI officer at eClinical Solutions. He argued that AI has made expanding datasets easier to manage and analyze, while sensors and wearables could drive volumes higher still. “Of course, you still want to make sure that you’re collecting the right data,” Mallarapu said. “But crunching that data and generating the insights that would be required, AI has certainly made that a lot easier with the powerful capability of some of the models.”

new Huntington’s disease (HD) clinical trial has officially begun, marking another important step in the search for treatments that could slow the progression of the disease. Prilenia Therapeutics and Ferrer have announced via a Linkedin post that the first participant has been dosed in the PRECISE-HD Phase 3 clinical trial, which is testing the investigational drug pridopidine.

Another chance for pridopidine
Long-time HDBuzz readers may recognise pridopidine. Pridopidine has had one of the longest and most winding development journeys of any experimental HD therapy. Originally developed as a treatment for movement symptoms, early clinical trials failed to meet their primary goals despite nuanced hints of benefit on some tests.

In 2023, the large Phase 3 PROOF-HD study did not meet its primary goal of slowing disease progression across all participants. However, a planned analysis of the data suggested that people who were not taking antidopaminergic medications (ADMs) appeared to have greater benefit from pridopidine than those who were taking these medications. ADMs are drugs commonly prescribed to treat chorea or psychiatric symptoms.

African Bundibugyo ebolavirus vaccine candidate to be advanced to clinical trials

Leading African biopharmaceutical company Minapharm Group, headquartered in Egypt with subsidiaries in Berlin, will receive up to US$16.5 million from the Coalition for Epidemic Preparedness Innovations (CEPI) to advance the development of a Bundibugyo ebolavirus vaccine candidate, strengthening efforts to develop vaccines to combat the fastest-growing and second-largest Ebola outbreak on record affecting the Democratic Republic of the Congo (DRC).

Minapharm’s Bundibugyo vaccine is designed by ProBioGen, a Berlin-based specialist within Minapharm Group, based on their proprietary modified vaccinia Ankara (MVA) vaccine platform, MVA-CR19. A similar technology underpins a licensed vaccine against smallpox and mpox, and has elicited long-term immunity in a vaccine targeting the related Zaire ebolavirus.

CEPI’s funding will advance the Bundibugyo vaccine candidate through preclinical development and into an early-stage clinical trial to assess the immunogenicity and safety of the vaccine candidate. The study in humans, known as a Phase 1 trial, is expected to take place in Africa.

Minapharm’s investigational vaccine becomes the fifth Bundibugyo-specific virus vaccine candidate supported by CEPI in response to the escalating epidemic. It joins a portfolio of promising CEPI-backed candidates* built on different vaccine technologies to maximise the likelihood of developing a safe and effective shot that could help bring this devastating crisis under control.

It is the only Bundibugyo vaccine candidate CEPI is funding that is designed to induce both B- and T-cell responses for comprehensive and long-lasting immunity.
