This study is a randomized controlled clinical trial conducted with patients admitted to the Adult Intensive Care Unit of the University Hospital of the Federal University of Maranhão, Presidente Dutra unit, from January to December 2025. One hundred participants will be selected through non probabilistic sampling and randomly allocated into two groups, intervention (50 participants) and control (50 participants), using simple randomization generated by an electronically produced numerical sequence. The researcher responsible for allocation will be different from the professional applying the interventions, ensuring partial masking of the process. The intervention group will begin the protocol after twenty four hours of invasive mechanical ventilation, following the recommendations of the Brazilian Guidelines for Early Mobilization in Intensive Care Units. Passive joint mobilization will consist of repeated movements within the limits of the joint range performed by a physiotherapist in both upper and lower limbs, with two daily sessions including ten repetitions per selected joint in each session, lasting approximately fifteen to twenty minutes. The control group will receive only the conventional physiotherapy routinely provided in the unit, without the additional application of passive joint mobilization. The total number of participants will be evenly distributed between the two groups according to the randomization sequence, ensuring comparability between the interventions studied. The following will be analyzed: duration of mechanical ventilation, length of stay in the Intensive Care Unit and hospital, as well as peripheral muscle strength and mobility status

Este estudo é um ensaio clínico controlado randomizado conduzido com pacientes internados na Unidade de Terapia Intensiva Adulto do Hospital Universitário da Universidade Federal do Maranhão, unidade Presidente Dutra no período de janeiro a dezembro de 2025. Cem participantes serão selecionados por amostragem não probabilística e alocados aleatoriamente em dois grupos, intervenção (50 participantes) e controle (50 participantes) , por meio de randomização simples realizada através de sequência numérica gerada eletronicamente sendo o pesquisador responsável pela alocação distinto do profissional aplicador das intervenções garantindo mascaramento parcial do processo. O grupo intervenção iniciará o protocolo após 24 horas de ventilação mecânica invasiva seguindo as recomendações da Diretriz Brasileira de Mobilização Precoce em Unidade de Terapia Intensiva. A mobilização articular passiva consistirá em movimentos repetidos dentro dos limites articulares realizados pelo fisioterapeuta em membros superiores e inferiores com frequência de duas sessões diárias sendo aplicadas dez repetições por articulação selecionada em cada sessão com duração aproximada de quinze a vinte minutos. O grupo controle receberá exclusivamente o atendimento fisioterapêutico convencional disponibilizado rotineiramente na unidade sem a aplicação adicional da mobilização articular passiva. O número total de participantes será distribuído de forma equilibrada entre os dois grupos conforme a ordem gerada pela randomização assegurando comparabilidade entre as intervenções estudadas. Serão analisados tempo de ventilação mecânica, de internação na Unidade de Terapia Intensiva e hospitalar, além de força muscular periférica e status de mobilidade

Patients aged eighteen years or older will be included in the study; patients of both sexes; patients on invasive mechanical ventilation for more than twenty four hours; patients presenting a sedation level between minus three and minus five according to the Richmond Agitation Sedation Scale; patients whose legal guardian agrees to participation through signing the Free and Informed Consent Form

Patients who remain under sedation with a Richmond Agitation Sedation Scale score between minus three and minus five will be excluded; patients who are unable to perform active activities will be excluded; patients who upon awakening do not agree to participate in the study will be excluded; patients whose legal guardian decides to withdraw consent will be excluded
