This is a prospective, randomized, controlled, single-blind study. The sample will consist of 30 patients with chronic tinnitus. There will be three groups A, B, and C, which will be block-randomized through the website randomization.com, thus determining which group they will belong to and the duration of treatment with transcutaneous auricular vagal electrostimulation that is most effective for the treatment of tinnitus will be evaluated, 60, 30, or 10 minutes. Each group will have 10 individuals with chronic tinnitus. Individuals in group A on the first day of intervention will undergo evaluation, answer questionnaires, and then will be referred to the physiotherapy group where they will receive transcutaneous auricular vagal nerve stimulation for 60 minutes and a detailed explanation of how to perform transcutaneous auricular vagal nerve stimulation at home. They will be introduced to the equipment and the sensations generated by the electrical waves, at which time they can also ask any questions regarding the handling of the equipment. Individuals in group B on the first day of intervention will undergo evaluation, answer questionnaires, and then will be referred to the physiotherapy group where they will receive transcutaneous auricular vagal nerve stimulation for 30 minutes and an explanation of how to perform transcutaneous auricular vagal nerve stimulation at home. They will be presented with the equipment and the sensations generated by the electrical waves, at which time they will also be able to ask any questions regarding the handling of the equipment. Individuals in group C on the first day of intervention will undergo evaluation, answer questionnaires, and then will be referred to the physiotherapy group where they will receive transcutaneous auricular vagal electrostimulation for 10 minutes and a detailed explanation of how to self-administer transcutaneous auricular vagal electrostimulation at home. They will be presented with the equipment and the sensations generated by the electrical waves, at which time they will also be able to ask any questions regarding the handling of the equipment. All groups will take home a complete set of equipment; portable Neurodin TENS, 2 clips for auricular application, 1 30g conductive gel, 1 charger for the device, a written manual, and an explanatory video sent to the patient's cellphone, explaining how to use and adjust the parameters according to the protocol described in this research, and they will be instructed to use the application time according to each group. Individuals in group A, 60 minutes per session. Individuals in group B, 30 minutes per session, and individuals in group C, 10 minutes per session. They will also receive a diary in which they should record the day, time, and intensity at which they performed the session. Each individual will perform one electrostimulation session three times per week, for three weeks, with three sessions per week at home, totaling 10 sessions. Only the first session will be at the tinnitus research group of the Hospital das Clínicas of the Faculty of Medicine at the University of São Paulo. Weekly, a researcher will contact the patients via telecall, at a pre-scheduled time, to follow up on the treatment. The principal researcher's WhatsApp will be provided so that patients can get in touch if any additional guidance is needed for managing the equipment at home. During the entire treatment period, they will not receive instructions Regarding symptom management, only concerning the treatment protocol. At the end of the third week, they will return to the tinnitus research group for final data collection and equipment return. For groups A, B, and C, an electrode will be attached to the cymba concha region of the ear, as it is the most suitable area for the application of this therapy since it is richly innervated by the auricular branch of the vagus nerve, and on the left ear because the vagal afferent fibers that innervate the heart are generally located on the right side; the other clip will be applied to the concha cavity to close the circuit. They will remain in the supine position and will be stimulated with electrostimulation until they perceive a tingling at the stimulation site, the sensitivity threshold. Each group will have an application with different durations. For group A, the stimulation will last 60 minutes. For group B, the stimulation will last 30 minutes, and for patients in group C, the application duration will be 10 minutes. The patients will be instructed oriented to increase the intensity if the tingling sensitivity disappears during the treatment. During home applications, they will be instructed not to go more than 3 days without application. In all groups, the electrodes will be coupled with gel on the patients' skin for better transduction of the electrical waves. The electrical stimulus will be biphasic, rectangular, with a frequency of 25 Hertz, and a pulse width of 250 μs with stimuli slightly above the patients' sensitivity threshold as it is the way to activate the myelinated A-beta fibers of the vagus nerve. At the beginning of each intervention, the two clips will be placed on the patients' left ear, with the parameters adjusted according to the protocol, frequency of 25 Hz, and pulse of 250 μs. Before the first session, the individuals will go through an initial evaluation and after the last day of the sessions, the patients will go through a final evaluation. There will be 5 researchers. 4 researchers will not have access to which intervention will be conducted, they will be blinded and will be responsible for carrying out the assessments, which consist of the Tinnitus Handicap Inventory (THI) questionnaire validated for the Portuguese language to assess the degree of tinnitus discomfort, the Visual Analog Scale (VAS) to evaluate the degree of discomfort and the volume of tinnitus, the General Anxiety Disorder (GAD-7) questionnaire to assess the level of anxiety, the Personal Health Questionnaire Depression Scale (PHQ-9), the Insomnia Severity Index (ISI), and the Somatosensory Assessment with Brain Gauge (SA), before the first intervention and after the last intervention. Only one researcher, the principal, will have access to which intervention will be conducted, applying 60 minutes, 30 minutes, or 10 minutes of transcutaneous auricular vagus nerve stimulation and will not have access to the questionnaire and scale results. Therefore, they will be blinded to the assessment results. Patients will be informed that they can stop and leave the study at any time.

Os indivíduos do grupo A no primeiro dia de intervenção passarão por avaliação, responderão questionários e depois serão encaminhados ao grupo de fisioterapia onde receberão aplicação de eletroestimulação auricular vagal transcutânea durante 60 minutos e explicação detalhada de como autorrealizar a eletroestimulação auricular vagal transcutânea em domicílio. Serão apresentados ao equipamento e às sensações geradas pelas ondas elétricas, momento em que também poderão fazer qualquer pergunta quanto ao manejo do equipamento. Os indivíduos do grupo B no primeiro dia de intervenção passarão por avaliação, responderão questionários e depois serão encaminhados ao grupo de fisioterapia onde receberão aplicação de eletroestimulação auricular vagal transcutânea durante 30 minutos e explicação detalhada de como autorrealizar a eletroestimulação auricular vagal transcutânea em domicílio. Serão apresentados ao equipamento e às sensações geradas pelas ondas elétricas, momento em que também poderão fazer qualquer pergunta quanto ao manejo do equipamento. Os indivíduos do grupo C no primeiro dia de intervenção passarão por avaliação, responderão questionários e depois serão encaminhados ao grupo de fisioterapia onde receberão aplicação de eletroestimulação auricular vagal transcutânea durante 10 minutos e explicação detalhada de como autorrealizar a eletroestimulação auricular vagal transcutânea em domicílio. Serão apresentados ao equipamento e às sensações geradas pelas ondas elétricas, momento em que também poderão fazer qualquer pergunta quanto ao manejo do equipamento. Todos os indivíduos dos grupos levarão para casa um equipamento completo; neurodin portable tens, 2 clips para aplicação auricular, 1 gel condutor de 30grs, 1 carregador para o aparelho, um manual escrito e um vídeo explicativo que enviado ao celular do paciente, com explicação de como utilizar e ajustar os parâmetros segundo o protocolo descrito nesta pesquisa, serão orientados a utilizar o tempo de aplicação de acordo com cada grupo. Os indivíduos do grupo A, 60 minutos cada aplicação. Os indivíduos do grupo B, 30 minutos cada aplicação e os indivíduos do grupo C, 10 minutos cada aplicação. Também receberão um diário onde deverão anotar o dia, horário e intensidade que realizaram a aplicação. Cada indivíduo realizará uma aplicação da eletroestimulação três vezes por semana, durante três semanas, sendo três aplicações em domicílio semanal, totalizando 10 aplicações. Somente a primeira aplicação será no grupo de pesquisa em zumbido do Hospital das Clínicas da faculdade de medicina da Universidade de São Paulo. Semanalmente um pesquisador entrará em contato com os pacientes, via tele chamada, em horário pré-determinado para fazer o acompanhamento do tratamento. Será fornecido o whatsApp do pesquisador principal para que os pacientes possam entrar em contato caso haja alguma orientação adicional necessária para o manejo do equipamento em domicílio. Durante todo o período de tratamento não receberão orientações quanto ao manejo do sintoma, somente referente ao protocolo de tratamento. No final da terceira semana retornarão ao grupo de pesquisa em zumbido para coleta final de dados, e devolução do equipamento. Para o grupo A, B e C um eletrodo será acoplado na região da cimba concha da orelha pois é a região mais indicada para aplicação dessa terapia por ser ricamente inervada pelo ramo auricular do nervo vago, e na orelha esquerda pois as fibras aferentes vagais que inervam o coração geralmente são localizadas no lado direito o outro clip será aplicado na cavidade da concha, para fechar o circuito. Permanecerão em decúbito supino e serão estimulados com a eletroestimulação até que percebam um formigamento no local do estímulo, limiar de sensibilidade. Cada grupo fará uma aplicação com tempos diferentes. Para o grupo A, o estímulo durará 60 minutos. Para o grupo B o estímulo durará 30 minutos e para os pacientes do grupo C, a duração da aplicação será de 10 minutos. Os pacientes serão orientados a aumentar a intensidade caso a sensibilidade de formigamento desapareça durante o tratamento. Durante as aplicações domiciliares receberão a orientação de não passarem de 3 dias sem aplicação. Em todos os grupos os eletrodos serão acoplados com gel na pele dos pacientes para melhor transdução das ondas elétricas. O estímulo elétrico será bifásico, quadrangular, com frequência de 25 Hertz, e largura de pulso de onda de 250us com estímulos pouco acima do limiar de sensibilidade dos pacientes por ser a forma de ativar as fibras mielinizadas A-beta do nervo vago. No início de cada intervenção será colocado os dois clips na orelha esquerda dos pacientes, com os parâmetros ajustados de acordo com o protocolo, frequência de 25 Hz, e pulso de 250us. Antes do primeiro atendimento os indivíduos passarão por avaliação inicial e após o último dia de atendimento os pacientes passarão por avaliação final. Haverá 5 pesquisadores. 4 pesquisadores não terão acesso a qual intervenção será realizada, estarão cegados, serão responsáveis por realizar as avaliações que são compostas dos questionários Tinnitus Handcap Inventory (THI) validado para a língua portuguesa, para avaliar o grau de incômodo do zumbido, (EVA) escala visual analógica para avaliar o grau de incômodo e volume do zumbido, o questionário General Anxiety Disorder (GAD-7) para avaliar o grau de ansiedade, o Personal Health Questionnaire Depression Scale (PHQ-9), Indice de Gravidade de Insônia (IGI) e Avaliação Somatossensorial com Brain Gauge (AS), antes da primeira intervenção e após a última intervenção. Somente um pesquisador, o principal, terá acesso a qual intervenção será realizada, aplicação de 60 minutos, 30 minutos ou 10 minutos de eletroestimulação auricular vagal transcutânea e não terá acesso aos resultados dos questionários e escalas. Portanto estará cegado para os resultados das avalições. Os pacientes serão informados que podem interromper e sair do estudo a qualquer momento.
