Outcome measure:
en
The mean pain intensity on the visual analog scale (VAS) was similar between the groups at the initial assessment: 5.25 ± 1.48 in the control group and 5.20 ± 1.44 in the intervention group. At the second assessment, the means were 4.30 ± 1.72 and 4.60 ± 1.57, respectively; at the third assessment, they were 4.05 ± 1.64 for the control group and 3.90 ± 1.25 for the intervention group. In the within-group comparison between the first and second assessments, the control group showed a mean reduction of 0.95 points (95% CI: −1.77 to −0.13; p = 0.026), while the intervention group showed a median change of 0.00 [−1.00; 0.00] (p = 0.046). After the Holm adjustment for the four paired VAS comparisons, the adjusted p-values were 0.052 for both groups. Between the first and third assessments, a mean reduction of 1.20 points was observed in the control group (95% CI: −2.04 to −0.36; p = 0.008; adjusted p = 0.023), and a median reduction of 1.00 point was observed in the intervention group [−2.25; 0.00] (p = 0.003; adjusted p = 0.012). In the between-group comparison, the adjusted difference in VAS scores at the second assessment was 0.33 points (intervention minus control; 95% CI: −0.62 to 1.28; p = 0.488). At AV3, the primary outcome, the adjusted difference was −0.13 points (95% CI: −1.04 to 0.78; p = 0.773). Therefore, no statistically significant difference was identified between the groups, either in the immediate response or during follow-up. The negative estimate at assessment 3 (AV3) numerically favors the intervention, but its confidence interval includes the possibility of no difference. Regarding range-of-motion measurements, there was an intra-group increase in flexion between assessment 1 (AV1) and AV3 in both the control group (mean change: 6.05°; raw p = 0.049) and the intervention group (6.90°; raw p = 0.022). At assessment 2 (AV2), the intervention group also showed a mean increase in flexion (8.60°; raw p = 0.006). These comparisons did not remain statistically significant after the Holm adjustment. No between-group differences were observed—following adjustment for multiplicity—in comparisons of flexion, extension, lateral flexion, or rotation. The mean Oswestry Disability Index (ODI) percentage decreased from 14.0% to 12.3% in the control group and from 14.7% to 13.1% in the intervention group between AV1 and AV3. The between-group difference in these changes was 0.18 percentage points (95% bootstrap CI: −2.70 to 3.25; p = 0.925; Holm-adjusted p = 1.000). Intra-group comparisons also failed to remain significant after the adjustment. In algometry, pressure pain thresholds were recorded in gf/cm² across the ten sites evaluated. Between-group comparisons of the changes from AV1–AV2 and AV1–AV3 were performed, alongside the corresponding intra-group comparisons. While some contrasts yielded a raw p-value of less than 0.05, none remained significant following the Holm adjustment. The lowest adjusted p-value among the 33 between-group comparisons was 0.430; among the 66 paired intra-group comparisons, no differences remained significant after the adjustment. At AV3, the proportion of positive neurodynamic tests did not differ between the groups. The right straight leg raise (SLR) was positive in 4/20 participants (20%) in each group (p = 1.000); the left SLR was positive in 4/20 (20%) in the control group and 3/20 (15%) in the intervention group (p = 1.000); and the Slump Test was positive in 7/20 (35%) in the control group and 11/20 (55%) in the intervention group (p = 0.341; Fisher’s exact test)

Results summary:

Chronic low back pain (CLBP) is a common condition that may impair the functional capacity and quality of life of university students. High-velocity, low-amplitude (HVLA) spinal manipulation is a technique used in the management of low back pain; however, its short-term effects in this population remain to be investigated. The objective of this study was to analyze the short-term effects of HVLA on pain intensity, range of motion and functional disability in university students with CLBP. This was a randomized clinical trial involving 40 participants allocated into two groups: intervention group (n=20), which received HVLA spinal manipulation, and control group (n=20), which received the simulated technique. Three assessments were conducted: before the interventions (A1), after the second intervention (A2) and three days after A2 (A3). Pain intensity was assessed using the Visual Analog Scale (VAS), functional capacity using the Oswestry Disability Index (ODI) and range of motion using goniometry. Pressure pain threshold, cutaneous sensitivity and neurodynamic tests were also assessed. The data were analyzed using descriptive and inferential statistics, with Holm adjustments for multiple comparisons. The mean VAS score for the total sample was 5.28 ± 1,50 points at A1, 4.00 ± 1.63 points at A2 and 3.98 ± 1.45 points at A3, indicating a descriptive reduction in pain. However, the adjusted analyses did not identify statistically significant differences between the groups in pain intensity, range of motion, functional disability or any of the other outcomes assessed. In conclusion, there is no statistically significant additional benefit of HVLA demonstrated compared with the simulated technique in this sample over the study period. These findings should be interpreted considering the sample size and the short follow-up period, and further studies with larger samples and longer follow-up periods are needed
