This is a randomized, controlled, blind, three-arm clinical study, each arm being defined based on the previous renal function of each patient Previous renal function was calculated using electronic software, the e-GFR application and the CKD-EPI Creatinine formula Patients with glomerular filtration rate >60mL/min/1.73m2 were allocated to the control group; patients with glomerular filtration rate <30mL/min/1.73m2 allocated in group 1; and patients with glomerular filtration rate between 30-59mL/min/1.73m2 were allocated in group 2 The expected study sample size is 100 patients. Trata-se de um estudo clinico randomizado controlado , cego, de três braços, sendo cada braço definido a partir da função renal prévia de cada paciente A função renal prévia foi calculada por software eletrônico, aplicativo e GFR e a fórmula CKD EPI Creatinine Os pacientes com taxa de filtração glomerular maior que 60mL/min/1,73m2 foram alocados no grupo controle; os pacientes com taxa de filtração glomerular menor que 30mL/min/1,73m2 alocados no grupo 1; e os pacientes com taxa de filtração glomerular entre 30 e 59mL/min/1.73m2 foram alocados no grupo 2 O tamanho amostral do estudo previsto são 100 pacientes. Em cada braço ( grupo controle; grupo 1 e grupo 2 ) foram randomizados controladamente 40 pacientes nos grupos controle e grupo 2 , e 20 pacientes no grupo 1 A randomização foi realizada de modo eletrônico, em planilha do excel, de forma estratificada, e o responsável pela realização dos exames contrastados, no caso o pesquisador principal, desconhece qual grupo o paciente do procedimento pertence. Hospitalized patients with peripheral arterial occlusive disease requiring contrast-enhanced imaging procedures (diagnostic and/or therapeutic arteriography only). Signed informed consent (ICF) was obtained. Patients of any age. Any gender were eligible. Individuals with normal renal function, defined by an estimated glomerular filtration rate (eGFR)>60mL/min/1.73m², were included in the control group. Patients with impaired renal function consistent with moderate chronic kidney disease (eGFR between 30–59 mL/min/1.73m²) .Severe chronic kidney disease (eGFR < 30 mL/min/1.73m²) were also included

A total of 76 patients diagnosed with peripheral arterial disease were included in the study, stratified by renal function status (normal, intermediate, and end-stage) and allocated into intervention and non-intervention groups
Age: The majority of patients were aged 60 years or older, with distributions as follows:
81.0% in the non-intervention group with normal renal function;
100% among all patients with intermediate renal function;
66.7% and 80.0% in end-stage renal function patients without and with intervention, respectively.
Sex:
There was a predominance of male participants across all groups, particularly:
76.2% in the control group without intervention;
66.7% in those with intermediate renal function and intervention;
80.0% in the end-stage renal function group receiving intervention.
Ethnicity:
White individuals were the majority in the normal renal function groups (47.6% without intervention and 62.5% with intervention).
Mixed-race (Pardo) participants were more frequent in intermediate and end-stage renal dysfunction subgroups.
Black individuals represented a minority across all cohorts (<20%).
Comorbidities:
Hypertension was highly prevalent, especially in end-stage renal dysfunction groups (100%).
Diabetes mellitus was present in up to 42.9% of patients with normal renal function and no intervention.
Dyslipidemia ranged between 13–33%, with similar distribution across groups.
Smoking was a frequent risk factor, present in up to 83.3% of patients with intermediate renal function and intervention.
Procedures Performed:
Arteriography was the most frequently performed procedure in all subgroups, with frequencies ranging from 58.3% to 81.8%.
Angioplasties and aortographies were less common and varied among groups.

Faixa etária: A maioria dos participantes tinha 60 anos ou mais, representando:

Material and Method This is a prospective, randomized, controlled, single-blind clinical trial conducted in the vascular surgery department of the Hospital Geral de Goiânia Dr. Alberto Rassi between February 2022 and March 2024 Patients with peripheral arterial disease undergoing contrast-enhanced angiographic procedures (diagnostic and/or therapeutic) in the hemodynamics unit were included, provided they signed the informed consent form The study was approved by the Research Ethics Committee (Approval No. 5.307.711) and registered in the Brazilian Clinical Trials Registry (ReBEC: U1111-1277-3145) Exclusion criteria: patients with acute kidney injury, dialysis-dependent chronic kidney disease, congestive heart failure, septic or cardiogenic shock, recent use (within 72h) of iodinated contrast or nephrotoxic drugs, and those who refused to participate Sample size and randomization: Sample size was calculated using GPower 3.1®, with a significance level of α=0.05, power of 80%, and effect size (d) of 0.5, resulting in 52 participants Accounting for 15% losses, the final sample was 60 However, 76 patients completed the protocol; 12 were excluded due to hemolysis of blood samples. Stratified randomization was performed using Microsoft Excel®, with allocation based on the =RAND() function Groups and intervention: Patients were divided based on glomerular filtration rate (GFR), estimated via CKD-EPI 2021 (creatinine) and CKD-EPI 2012 (cystatin C) equations: Control Group: GFR >60 ml/min/1.73m² Group 1: GFR <30 ml/min/1.73m² Group 2: GFR 30–59 ml/min/1.73m² Each group was randomized into two subgroups (intervention and non-intervention). All patients received isotonic saline (0.9% NaCl) at 1 mL/kg/h, 12h before and 12h after the procedure. The intervention subgroup additionally received oral N-acetylcysteine 600 mg every 12h for 48h (EMS®) and atorvastatin 80 mg before and 40 mg after the procedure (Eurofarma®) Primary outcomes: Serum creatinine levels were measured 24h before, and 24h and 72h after the procedure. CI-AKI was defined as an absolute increase ≥0.5 mg/dL or relative increase ≥25% from baseline Serum cystatin C levels (measured by turbidimetry) were assessed 24h before and 24h after the procedure. A ≥10% increase was considered indicative of CI-AKI Statistical analysis: Data were organized in Microsoft Excel® and analyzed using SPSS version 26.0 (IBM®). Material e Método Trata-se de um ensaio clínico prospectivo, controlado, randomizado com cegamento unico, conduzido no serviço de cirurgia vascular do Hospital Geral de Goiânia Dr Alberto Rassi, entre fevereiro de 2022 e março de 2024 Foram incluídos pacientes com diagnóstico de doença arterial periférica, que necessitavam de exames ou procedimentos com contraste iodado realizados na hemodinâmica (angiografias diagnósticas e/ou terapêuticas), e que concordaram com a participação mediante assinatura do Termo de Consentimento Livre e Esclarecido O estudo foi aprovado pelo Comitê de Ética em Pesquisa (Parecer nº 5.307.711) e registrado na plataforma ReBEC (U1111-1277-3145 ) Critérios de exclusão: pacientes com doença renal aguda, nefropatia crônica dialítica, insuficiência cardíaca congestiva, choque séptico ou cardiogênico, uso prévio de contraste iodado nas últimas 72 horas ou de medicações nefrotóxicas, e pacientes que não aceitaram participar do estudo Amostragem e randomização: O cálculo amostral foi realizado com o software GPower 3.1®, adotando-se um α=0,05, poder estatístico de 80% e tamanho de efeito (d) de 0,5, resultando em 52 participantes Acrescido 15% para perdas, obteve-se n=60. Contudo, ao final do estudo, 76 pacientes completaram o protocolo, com 12 perdas por inviabilidade na análise das amostras de sangue (devido à hemólise) A randomização foi estratificada via planilha Excel®, utilizando a função =RAND() para alocação. Intervenção e grupos: Os pacientes foram alocados em 3 grupos, conforme a taxa de filtração glomerular (TFG), calculada pelas equações CKD-EPI 2021 (creatinina) e 2012 (cistatina C): Grupo Controle: TFG >60 ml/min/1,73m² Grupo 1: TFG <30 ml/min/1,73m² Grupo 2: TFG entre 30–59 ml/min/1,73m² Cada grupo foi dividido em subgrupos (intervenção e não intervenção). Ambos receberam hidratação venosa com SF 0,9% a 1 ml/kg/h, 12h antes e 12h após o procedimento O subgrupo intervenção recebeu adicionalmente n-acetilcisteína 600 mg VO 12/12h por 48h (EMS®) e atorvastatina 80 mg antes e 40 mg após o procedimento (Eurofarma®) Desfechos primários: Níveis de creatinina sérica medidos 24h antes, e 24h e 72h após o procedimento A definição de CI-AKI seguiu critérios de aumento absoluto ≥0,5 mg/dL ou relativo ≥25% Níveis de cistatina C sérica (método turbidimétrico) medidos 24h antes e 24h após O aumento ≥10% foi considerado indicativo de lesão renal Análise estatística: Os dados foram tabulados em Excel® e analisados no SPSS v.26.0 (IBM®) Realizou-se análise descritiva, testes de significância com p<0,05 e intervalo de confiança de 95% Fluxo de participantes: 88 pacientes foram inicialmente incluídos Doze foram excluídos devido à inviabilidade de análise laboratorial por hemólise Assim, 76 pacientes completaram o estudo.
