This is a two-arm, single-blind, randomized, controlled clinical trial. A total of 40 people met the eligibility criteria and were probabilistically allocated to the active group (tDCS) or sham group (sham stimulation) using a random number table in Excel. DC-STIMULATOR PLUS electrodes (NeuroConn GmbH) were positioned in the corresponding area BA 10 (anode) and right lateral BA 9, which is part of the dorsolateral prefrontal cortex (rDLPFC; cathode); the electrodes measured 20 cm² and 15cm², the sponges were moistened with saline solution, and the current intensity was 2mA. Active group: 20 people (10 women and 10 men) received 2mA stimulation for 20 minutes, with a 30-second up and down ramp. Sham group: 20 people (10 women and 10 men) received 2mA stimulation with a 30-second upslope, 1-minute stimulation, and a 30-second downslope. All participants attended the study for 5 days

Trata-se de um estudo clínico randomizado controlado de dois braços, mono-cego. Um total de 40 pessoas preencheram os critérios de elegibilidade e foram alocadas probabilisticamente no grupo ativo (tDCS) ou grupo sham (estimulação simulada) utilizando uma tabela de números aleatórios no Excel. Eletrodos DC-STIMULATOR PLUS (NeuroConn GmbH) foram posicionados na área correspondente BA 10 (ânodo) e lateral direito BA 9, que faz parte do córtex pré-frontal dorsolateral (rDLPFC; cátodo); os eletrodos mediam 20 cm² e 15cm², as esponjas foram umedecidas com soro fisiológico e a intensidade de corrente foi de 2mA. Expected outcome 1: We seek to verify that transcranial Direct Current Stimulation affects feelings of happiness and guilt in the context of costly altruistic decisions. Stimulation is expected to increase perceived happiness while decreasing perceived guilt after a non-government organization giving a game. It is expected that after tDCS stimulation the measurement of two metabolites, GABA and Glx, change in two areas of the brain: Ventromedial Prefrontal Cortex (vmPFC) and Dorsolateral Prefrontal Cortex (DLPFC). We expect an increase in GABA after 2mA transcranial Direct Current Stimulation on Ventromedial Prefrontal Cortex and a decrease in Glx on Dorsolateral Prefrontal Cortex.

Result found 1: We have found transcranial Direct Current Stimulation over Ventromedial Prefrontal Cortex, and Dorsolateral Prefrontal Cortex influenced self-reported happiness following a donation task. There was an increase in happiness in the active group, compared to the sham group, and a decrease in guilt in both groups across sessions. Also, we report increased levels of GABA in the Ventromedial Prefrontal Cortex and decreased levels of Glx in the Ventromedial Prefrontal Cortex of the active group.

Forty participants (20 women) living in Rio de Janeiro, Brazil (Mean age = 24.4 ± 10; range = 19 to 34 years old) were recruited. For reported Happiness, there was a significant main effect of Session (b=.178, SE=.064 t=2.773, p=.009), and a significant Session × Group interaction (b=.311, SE=.129, t=2.417, p=.021). Specifically, reported happiness increased significantly over time, however only in the active group (b=.325, 95%CI=[.146 .504]) and not in the sham group (b=.015, 95%CI=[-.173 .203]). The main effect of Group was not significant (p=.712), suggesting that happiness did not significantly differ between groups overall which may be explained by the lower starting happiness ratings in the active group. For reported Guilt, there was a main effect of Session (b=-.218, SE=.080, t=-.2.735, p=.009). The main effect of Group and the Session × Group interaction were not significant, ps>.287. These findings suggest that reported guilt decreased significantly over time, irrespective of stimulation. For GABA at vmPFC, there was a significant two-way interaction between Session and Group (b=.022, SE=.010, t=2.152, p=.038). GABA levels significantly increased post-intervention in the active group, ΔGABA=.017, 95%CI=[.002 .032], but did not significantly change in the sham group, ΔGABA=-.005, 95%CI=[-.021 .010]. No other effects were significant, ps>.271. For GABA at DLPFC, there was a non-significant main effect of Session (b=-.006, SE=.004, t=-1.704, p=.098), indicating a trend that GABA levels decreased post-intervention, irrespective of Group. No other effects were significant, ps>.541. For Glx at vmPFC, there was a marginal main effect of Group (b=-.007, SE=.004, t=-2.046, p=.056), indicating a trend that the overall Glx levels were lower in the active group than in the sham group. Understanding the neural, metabolic, and psychological mechanisms underlying human altruism and decision-making is a complex and important topic for science and society. Here, we investigated whether transcranial Direct Current Stimulation (tDCS) applied to two prefrontal cortex regions, the ventromedial prefrontal cortex (vmPFC, anode) and the right dorsolateral prefrontal cortex (DLPFC, cathode), can be used to induce changes in self-reported emotions and to modulate local metabolite concentrations. We employed in vivo quantitative MR Spectroscopy in healthy adult participants and quantified changes in GABA and Glx (glutamate + glutamine) before and after five sessions of tDCS delivered at 2mA for 20 minutes (active group) and 1 minute (sham group) while participants were engaged in a charitable donation task. In the active group, we observed increased levels of GABA in vmPFC. Glx levels decreased in both prefrontal regions, and self-reported happiness increased significantly over time in the active group. Self-reported guilt in both active and sham groups tended to decrease. The results indicate that self-reported happiness can be modulated, possibly due to changes in GABA and Glx concentrations following repeated stimulation. Therefore, local changes may induce remote changes in the reward network through interactions with other metabolites, previously thought unreachable with noninvasive stimulation techniques

Compreender os mecanismos neurais, metabólicos e psicológicos subjacentes ao altruísmo humano e à tomada de decisões é um tópico complexo e importante para a ciência e a sociedade. Aqui, investigamos se a Estimulação Transcraniana por Corrente Contínua (ETCC) aplicada a duas regiões do córtex pré-frontal, o córtex pré-frontal ventromedial (vmPFC, ânodo) e o córtex pré-frontal dorsolateral direito (DLPFC, cátodo), pode ser usada para induzir mudanças no auto-relato de emoções e modular as concentrações locais de metabólitos. Empregamos espectroscopia de RM quantitativa in vivo em participantes adultos saudáveis e quantificamos alterações em GABA e Glx (glutamato + glutamina) antes e depois de cinco sessões de ETCC a 2mA por 20 minutos (grupo ativo) e 1 minuto (grupo simulado) enquanto os participantes respondiam a uma tarefa de doação de caridade. No grupo ativo, observamos níveis aumentados de GABA no vmPFC. Os níveis de Glx diminuíram em ambas as regiões pré-frontais e a felicidade auto-relatada aumentou significativamente ao longo do tempo no grupo ativo. A culpa auto-relatada em ambos os grupos ativo e sham tendeu a diminuir. Os resultados indicam que a felicidade autorrelatada pode ser modulada, possivelmente devido a mudanças nas concentrações de GABA e Glx após estimulação repetida. Portanto, alterações locais podem induzir alterações remotas na rede de recompensa por meio de interações com outros metabólitos, anteriormente considerados inalcançáveis com técnicas de estimulação não-invasivas
