This was a single-centre, single-blinded, parallel-group, randomized, controlled trial. The selection of participants will take place on a non-probabilistic basis, on a voluntary basis. The recruitment of participants will be carried out in medical screening in a specific outpatient clinic for the care of patients already infected with COVID-19 at the University Hospital of the Federal University of Santa Catarina. Those who are interested will be referred to baseline assessments and will follow the flow of participation in the randomized clinical trial (allocation in one of the groups, follow-up until the final assessment). Patients considered eligible for the sample will be randomly allocated to one of the two study groups. One group will undergo a clinical and functional rehabilitation program (30) and the other group will be considered a control group (30). Those patients allocated to the control group will receive general information on the importance of physical activity and should be included in a late rehabilitation process, which is characterized by the same protocol proposed to the intervention group, which will be paid between 1.5 and 2 years after the beginning of the treatment. study. The intervention will last 24 weeks and will consist of two weekly sessions in the first 11 weeks, reassessments and regenerative activities in the 12th week. From the 13th to the 24th week, training will be performed in three weekly sessions. The proposed training will have two major stages, the first consisting of two mesocycles, with progression primarily in volume and the second of three mesocycles, with progression primarily in intensity.

Trata-se de um estudo clínico randomizado de dois braços, unicentrico, cegado. A seleção dos participantes ocorrerá de forma não-probabilística, por voluntariedade. O recrutamento dos participantes será realizado em triagem médica em ambulatório específico para atendimento de pacientes já infectados por COVID-19 do Hospital Universitário da Universidade Federal de Santa Catarina. Aqueles que se interessarem, serão encaminhados às avaliações da linha de base e seguirão o fluxo de participação no ensaio clínico randomizado (alocação em um dos grupos, seguimento até a avaliação final). Os pacientes considerados elegíveis para amostra serão alocados randomicamente em um dos dois grupos do estudo. Um grupo será submetido a um programa de reabilitação clínica e funcional (30) e o outro grupo será considerado grupo controle (30). Aqueles pacientes alocados no grupo controle receberão informações gerais da importância da atividade física e devem ser inseridos em um processo reabilitativo tardio, que se caracteriza pelo mesmo protocolo proposto ao grupo intervenção, o qual será pago entre 1,5 a 2 anos após o início do estudo. A intervenção terá duração de 24 semanas e será composta por duas sessões semanais nas primeiras 11 semanas, reavaliações e atividades regenerativas na 12ª semana. Da 13ª a 24ª semana o treinamento passará a ser executado em três sessões semanais. O treinamento proposto terá duas grandes etapas, sendo a primeira composta de dois mesociclos, com progressão primariamente em volume e a segunda de três mesociclos, com progressão primariamente em intensidade.

Groups were similar in baseline characteristics. The majority of participants were middle-aged, overweight men with hypertension. Furthermore, the majority of participants were admitted to the Intensive Care Unit (60%). The most prevalent symptoms in the acute phase of the disease were dyspnea, cough, fever and fatigue, while the most prevalent symptoms in the post-acute phase were dyspnea, fatigue, memory loss and myalgia. Participants in the intervention group presented the following characteristics: mean age of 47.61 ± 12.13 years; 11 men and 10 women; post-COVID hospitalization time of 384.71±171.07 days; mean body mass index of 30.46 ±7.37 kg/m²; 61.9% were admitted to the ICU (mean of 12.23±8.29 days); 19% were admitted to the nursery; 19% did not undergo hospitalization and the average number of days spent in hospital for patients in the intervention group was 20.06±15.14 days. Participants in the control group presented the following characteristics: average age of 54.05 ± 11.43 years; 10 men and 8 women; post-COVID hospitalization time of 353.61±149.79 days; mean body mass index of 31.73 ± 8.72 kg/m²; 66.7% were admitted to the ICU (average of 18.23±17.76 days); 5.5% were admitted to the nursery; 27.8% did not undergo hospital admission and the average number of days spent in hospital for patients in the control group was 24.07±20.33 days.

89 patients were screened, of which 17 were not clinically discharged. Of the 72 who were clinically discharged, 14 were unable to participate for the following reasons: living far away (n=6) and lack of time (n=8). Thus, 58 subjects agreed to participate and carried out the initial evaluations. Throughout this process, 18 people dropped out, for reasons such as impossibility of scheduling (3), impossibility of moving to the location (n=13) and/or physical training co-intervention (n=2). Therefore, 40 individuals were allocated to the groups, with a success rate of 45% (40 of 89). Of these 40 individuals, 21 were allocated to the intervention group and 19 to the control group. Regarding the follow-up of the study, in the intervention group, three patients dropped out over the 24 weeks of the protocol, while in the control group, five patients were excluded from the study because they had not participated in the reevaluations. In this way, of the 40 randomized participants, 32 participants completed the study (18 in the intervention group and 14 in the control group).

Introduction: People infected with the COVID-19 virus experience a series of signs and symptoms after the acute phase of the disease, which can result in losses in a series of outcomes, including functional capacity, quality of life, autonomy and social interaction. Rehabilitation programs via physical exercise are necessary to allow adults and elderly people who were infected by Sars-COV-2 to resume their daily activities. Objectives: To analyze the effects of multicomponent training on functional, clinical, morphological, behavioral and psychosocial outcomes of adults and elderly people post-COVID-19 infection. Methods: This is a randomized and controlled clinical trial, with the experimental group undergoing an intervention involving a multicomponent physical rehabilitation program, carried out at the Sports Center in partnership with the University Hospital of the Federal University of Santa Catarina, in Florianópolis, Brazil. The participants were adults and elderly people, of both sexes, in a post-COVID-19 infection state, who presented moderate to severe clinical conditions during the infection. The intervention lasted 24 weeks and included a multicomponent physical training program, with gradual progression in frequency, duration and intensity over time. Regarding outcomes, before, on the 12th and after 24 weeks of intervention, functional (primary outcome = functional index of aerobic capacity), clinical, morphological, behavioral and psychosocial results were evaluated. Results: The outcomes of functional capacity and functionality symptoms showed significant improvements, especially in the GI, and even when in both groups, the improvements were more pronounced and with greater magnitude in the GI, highlighting aerobic capacity, fatigue level and fragility. The outcomes quality of life, quality of sleep, percentage of dilation (vascular parameters), isometric muscle strength of the lower limbs showed significant improvements only in the GI. The outcomes systolic blood pressure, diastolic blood pressure, mean arterial pressure, depressive symptoms, relative volume and carbon monoxide diffusion (respiratory parameter) showed a time effect, improving in both groups, intervention and control. In body composition, an improvement was identified in body mass index, body fat, fat mass, visceral adipose tissue and phase angle in the GI. No changes in physical activity levels and sedentary behavior were identified after 24 weeks of training. Conclusion: It is concluded that a multicomponent training protocol such as post-COVID rehabilitation lasting 24 weeks showed significant improvements in certain parameters that make up functional, cardiovascular, neuromuscular, respiratory and psychosocial outcomes only in the intervention group and in certain parameters significant improvements were observed in both groups.

Introdução: As pessoas infectadas pelo vírus da COVID-19 experimentam uma série de sinais e sintomas pós fase aguda da doença, os quais podem implicar em prejuízos em uma série de desfechos, a citar capacidade funcional, qualidade de vida, autonomia e interação social. São necessários programas de reabilitação via exercício físico para proporcionar a retomada das atividades da vida diária dos adultos e idosos que foram infectados pelo Sars-COV-2. Objetivos: Analisar os efeitos do treinamento multicomponente em desfechos funcionais, clínicos, morfológicos, comportamentais e psicossociais de adultos e idosos pós-infecção por COVID-19. Métodos: Trata-se de um ensaio clínico randomizado e controlado, sendo o grupo experimental submetido a uma intervenção envolvendo um programa de reabilitação física multicomponente, realizado no Centro de Desportos em parceria com o Hospital Universitário da Universidade Federal de Santa Catarina, em Florianópolis, Brasil. Os participantes foram adultos e idosos, de ambos os sexos, em estado pós-infecção por COVID-19, que apresentaram quadros clínicos moderados a severos durante a infecção. A intervenção teve uma duração total de 24 semanas e incluiu um programa de treinamento físico multicomponente, com progressão gradual na frequência, duração e intensidade ao longo do tempo. Em relação aos desfechos, antes, na 12ª e após 24 semanas de intervenção, foram avaliados resultados funcionais (resultado primário = índice funcional de capacidade aeróbica), clínicos, morfológicos, comportamentais e psicossociais. Resultados: Os desfechos de capacidade funcional e sintomas de funcionalidade, apresentaram melhoras significativas especialmente no GI, e mesmo quando em ambos os grupos, as melhoras foram mais acentuadas e com maior magnitude no GI, destacando-se a capacidade aeróbica, nível de fadiga e fragilidade. Os desfechos qualidade de vida, qualidade do sono, percentual de dilatação (parâmetros vasculares), força muscular isométrica de membros inferiores apresentaram melhoras significativas somente no GI. Já os desfechos pressão arterial sistólica, pressão arterial diastólica, pressão arterial média, sintomas depressivos, volume relativo e difusão de monóxido de carbono (parâmetros respiratórios) apresentaram efeito tempo, melhorando em ambos os grupos, intervenção e controle. Na composição corporal, foi identificada melhora no índice de massa corporal, gordura corporal, massa gorda, no tecido adiposo visceral e ângulo de fase no GI. Não foram identificadas mudanças nos níveis de atividade física e comportamento sedentário após as 24 semanas de treinamento. Conclusão: Conclui-se que um protocolo de treinamento multicomponente como reabilitação pós-COVID com duração de 24 semanas apresentou melhoras significativas de determinados parâmetros que compôe os desfechos funcionais, cardiovasculares, neuromusculares, respiratórios e psicossociais somente no grupo intervenção e, em determinados parâmetros, melhoras significativas foram observadas em ambos os grupos.
