This is a randomised, controlled, three-arm parallel-group clinical trial in which participants will be blinded, whereas the operator will not be blinded. The sample will comprise 78 teeth, allocated to three groups, with 26 teeth per group. The tooth will be the unit of randomisation and sample size calculation. The total number of participants will depend on the number of eligible teeth contributed by each individual. Eligible teeth will be individually randomised to one of the three groups using the electronic True Random Number Service, available at www.random.org. If more than one tooth from the same participant is included, the potential correlation between observations will be accounted for in the statistical analysis. Group 1 — control: 26 teeth will receive a 5% sodium fluoride varnish. Following dental prophylaxis, relative isolation and gentle air-drying of the tooth surface for 5 seconds, the varnish will be applied using a flexible cotton applicator. Participants will be instructed to refrain from eating for 30 minutes after application. Four applications will be performed, one per week for four consecutive weeks. Group 2 — low-level laser therapy: 26 teeth will receive irradiation using a low-intensity infrared diode laser with a wavelength of 808 nanometres and an output power of 100 milliwatts. The laser will be applied in direct contact with the tooth surface, perpendicular to the treatment area, at three sites corresponding to the mesial, central and distal regions of the opacity. Each site will be irradiated for 10 seconds, delivering 1 joule of energy per site. Two applications will be performed, the first at baseline and the second after 48 hours. Group 3 — silver diamine fluoride: 26 teeth will receive a single application of 38% silver diamine fluoride associated with potassium iodide. Following prophylaxis with non-fluoridated prophylaxis paste, surface drying, protection of the soft tissues with a light-cured gingival barrier and petroleum jelly, and relative isolation with cotton rolls, one drop of the silver diamine fluoride solution will be applied using a microbrush, immediately followed by the application of two drops of the potassium iodide solution. The treated surface will then be dried with absorbent paper. The protocol will consist of a single application. To maintain participant blinding, the product containers will be masked, and the sound of low-level laser activation, recorded in advance, will be played using a mobile phone during procedures in all groups. In all groups, reassessments will be performed after 48 hours, weekly during the first month, and subsequently at 8 and 12 weeks

A reduction in the behavioural response to pain elicited by an evaporative stimulus is expected, as assessed using the Schiff Cold Air Sensitivity Scale, with scores ranging from 0 to 3, following the application of compressed air at 45 pounds per square inch for 1 second, from a distance of 1 centimetre from the tooth surface. Assessments will be conducted at baseline, 48 hours after treatment, weekly throughout the first month, and at 8 and 12 weeks of follow-up
