This is a two-arm, single-blind, randomized controlled clinical trial. Intervention group: The 75 participants in this group will receive foot care instructions according to the International Working Group on the Diabetic Foot (IWGDF) guidelines and will undergo photobiomodulation (PBM) using the following protocol: 808 nm, 100 mW power, 5 J/cm², continuous wave, invisible beam, three times a week for a total of 12 weeks. The treatment area includes the entire plantar surface of the foot and the extension of the tibial, superficial fibular, deep fibular, common fibular, sciatic, and saphenous nerves of the leg. Control group: The 75 participants in this group will receive foot care instructions according to the International Working Group on the Diabetic Foot (IWGDF) guidelines. If the intervention proves effective in the intervention group, photobiomodulation will be offered to the participants in the control group, according to the protocol applied in the intervention group.

Trata-se de um estudo clínico randomizado controlado de dois braços, simples-cego. Grupo intervenção: Os indivíduos alocados neste grupo, 75 participantes, receberão as orientações para cuidado com os pés de acordo com o guideline do International Working Group on the Diabetic Foot (IWGDF) e serão submetidos a FBM ao seguinte protocolo: 808 nm, potência de 100mW, 5J/cm2, onda contínua, feixe invisível, três vezes por semana, totalizando 12 semanas. A área de tratamento inclui toda a superfície plantar do pé e a extensão dos nervos tibial, fibular superficial, fibular profundo, fibular comum, ciático e safeno da perna. Grupo controle: Os indivíduos alocados neste grupo, 75 participantes, receberão as orientações para os cuidados com os pés de acordo com o guideline do International Working Group on the Diabetic Foot (IWGDF). Se a intervenção demonstrar eficácia no grupo de intervenção, será oferecida a aplicação da fotobiomodulação aos participantes do grupo controle, conforme o protocolo aplicado no grupo de intervenção.

Age between 18-65 years; diagnosis of diabetic peripheral neuropathy; ability to walk independently without equipment or devices.

It is expected that pain and symptoms of diabetic peripheral neuropathy will improve through treatment with photobiomodulation, based on a decrease in the painDETEC scale score, which assesses pain.

It is expected that the perception of stress related to diabetes mellitus will improve through photobiomodulation treatment, based on a decrease in the score on the Diabetes Distress Scale, which assesses the stress caused by the disease.
