This is a prospective, controlled, randomized study. Participants will be patients over 50 years of age with advanced shoulder osteoarthritis, refractory to conservative treatment, and with an indication for total shoulder arthroplasty (awaiting the procedure), or patients who are not clinically fit for the procedure (high surgical risk). Patients will be randomly allocated to the groups by drawing lots using the Research Randomizer software. Each patient will receive a sealed brown envelope containing the group to which they have been assigned. A researcher will be exclusively responsible for this process and will have no further contact with the research participant at any other time. The masking type will be open-label. Two groups will therefore be formed: 20 patients for the Experimental Group undergoing bone marrow injections, which will receive treatment through intra-articular injection with bone marrow aspirate, and 20 patients for the Control Group, treated with intra-articular corticosteroid injection (triamcinolone)

Trata-se de estudo prospectivo, controlado e randomizado. Participarão do estudo, pacientes acima de 50 anos de idade com osteoartrite avançada do ombro, refratárias ao tratamento conservador e com indicação para realização de artroplastia total do ombro (aguardando a realização da mesma), ou que não tenham condições clínicas para o procedimento (risco cirúrgico alto). Os pacientes serão alocados aleatoriamente nos grupos através de sorteio, por meio do Software Research Randomizer. Cada paciente receberá um envelope pardo lacrado com o grupo do qual fará parte. Haverá um pesquisador responsável exclusivamente por esse processo, não tendo mais contato com o participante de pesquisa em nenhum outro momento. O tipo de mascaramento será aberto. Serão formados então dois grupos: 20 pacientes do Grupo Experimental submetido às infiltrações com medula óssea, que será submetido ao tratamento através da infiltração intra-articular com aspirado de medula óssea, e 20 pacientes do Grupo Controle, submetido a tratamento com infiltração articular com corticóide (triancinolona)

Inclusion criteria:

Patients of both genders; aged over 50 years; with symptomatic glenohumeral osteoarthritis that has not improved with appropriate conventional conservative treatment and with an indication for shoulder arthroplasty; with glenohumeral osteoarthritis grades 1 to 5 according to the Hamada classification.

Exclusion criteria:

Failure to sign the Informed Consent Form; psychiatric and/or cognitive disorders that preclude assessment and follow-up; coagulopathies; malignant neoplasm within the last five years; orthobiologic procedures in the same joint within the past 6 months; stroke or acute myocardial infarction within the last 3 months; intolerance or allergy to local anesthetic.

We expect to observe an improvement in shoulder function after bone marrow aspirate injection, as measured by pain and functional scores (Visual Analog Scale, Short Form-12, Shoulder Pain and Disability Index, and the American Shoulder and Elbow Surgeons Shoulder Score)
