The patients were divided into test and control groups. The 33 patients in the test group were treated with Dynamic Hip Screw by minimally invasive approach of the hip, an access performed on the side of the hip, approximately 3 centimeters long. The remaining 33 patients in the control group were submitted to treatment with the same implant, due to the traditional Hardinge approach. A surgical approach of approximately 15 centimeters. In both groups, the degree of postoperative pain, the hematimetric loss, the elevations of inflammatory tests and the degree of immediate postoperative active mobility of the hip were compared.

Os pacientes foram divididos em grupos teste e controle. Os 33 pacientes do grupo teste foram tratados como sistema Dynamic Hip Screw por acesso minimamente invasivo do quadril, um acesso realizado na face lateral deste, com aproximadamente 3 centímetros de comprimento. Os demais 33 pacientes presentes no grupo controle foram submetidos ao tratamento com o mesmo implante, pelo acesso tradicional de Hardinge. Uma via ampla de aproximadamente 15 centímetros. Em ambos grupos foram comparados: o grau de dor pos-operatória, a perda hematimétrica, a elevações de provas inflamatórias e grau de mobilidade ativa pós-operatória imediata do quadril.

Been brazilian 60 years old or older Any gender Patients with intertrochanteric fracture in codition approprated to the proposal surgical procedure Patients that authorize their participation through the signing of the consent form

Exclusion criteria:

Patients who declared themselves indian; Patients with intertrochanteric fractures with obliquity reverse fracture, whose treatment with DHS is not recommended; Patients with pathological intertrochanteric fractures due to malignant or benign tumor disease; Cases in which the anesthesia team refused to perform the intraoperative analgesia protocol chosen for the study; Patients who needed blood transfusion in the immediate trans or postoperative period; Patients with low level of discernment, to answer the pre and postoperative questions, of the study.

Pacientes que se autodeclararam indígenas; Pacientes com fraturas intertrocantéricas com obliqüidade do traço reversa, cujo tratamento com DHS não é recomendado; Pacientes com fraturas intertrocantéricas patológicas por doença tumoral maligna ou benigna; Casos em que a equipe de anestesia se recusou a realizar o protocolo de analgesia intraoperatória escolhido para o estudo; Pacientes que necessitaram de hemotransfusão no trans ou pós-operatório imediato; Pacientes com nível de discernimento baixo, para responder às questões pré e pós-operatórias, do estudo.

The study purpose was to compare two different surgical approaches for intertrochanteric fractures treatment with DHS system. The minimally invasive hip and Hardinge lateral approach. The evaluation of pain on the first postoperative day showed that the lower aggressiveness of the minimally invasive approach generates a lower pain. This was also demonstrated into active hip mobility tests on the first postoperative day (the patient was instructed to perform the flexion of the operated hip and after that, try to sit on the bed and hold this position for a few seconds). Patients submitted to minimally invasive fixation presented better performance in these tests. Postoperative pain assessment was the primary endpoint of the study

A proposta do estudo foi comparar duas vias de acesso diferentes para a realização da fixação de fraturas intertrocantéricas com o sistema DHS. A via minimamente invasiva do quadril e o acesso lateral de Hardinge. A avaliação da dor no primeiro dia pós-operatório demonstrou que a menor agressividade do mini acesso gera menor quadro álgico. Isso se traduz também nos testes de mobilidade ativa do quadril no primeiro dia pós-operatório (o paciente era orientado a realizar a flexão do quadril operado e após isso, tentar sentar-se ao leito e manter-se nesta posição por alguns segundos). Os pacientes submetidos à fixação pela via minimamente invasiva apresentaram melhor performance nesses testes. A avaliação da dor pós-operatória foi o desfecho primário principal do estudo

The degree of hematimetric loss is essential to evaluate the aggressiveness of a surgical technique. The comparison between the Hardinge and minimally invasive approaches demonstrated that the second one cause reduced blood cell depletion, translated by a lower hemoglobin and hematocrit loss.
