This is a randomized, controlled, parallel-group, double-blind clinical study. All participants will initially undergo a standardized preoperative phase, including oral hygiene instruction, supragingival scaling, and professional prophylaxis. Participants will be randomly assigned to one of two groups: control (n=25) (treated using the conventional technique) or test (n=25) (using the Blades Contour instrument for the primary gingivectomy incision). The surgical sequence will be standardized for both groups. Terminal infiltration anesthesia using 2% lidocaine with 1:100,000 epinephrine will be administered. Subsequently, the depth of the gingival sulcus or periodontal pocket will be marked using a calibrated probe or Crane-Kaplan forceps, transferring the measurement to the external gingival surface via bleeding points. The amount of tissue to be removed will be determined based on these markings. In the control group, the primary incision will be made using the conventional technique with Kirkland or Orban periodontal knives, depending on the region's natomy. The incision will be positioned approximately 0.5 to 1.0 mm apical to the bleeding points, with an angle designed to preserve remaining tissue and establish a proper gingival contour. Next, the gingival collar will be removed, and gingivoplasty will be performed using curved scissors or other periodontal instruments. In the test group, the same surgical principles will be applied. The difference will be the use of the Blades Contour knife for the primary incision and the excision of the gingival collar. At the end of the surgery, the area will be irrigated with saline solution. No surgical dressing will be used. Paracetamol (500 mg) will be prescribed every 6 hours for 2 days, and 0.12% chlorhexidine digluconate will be prescribed twice daily for 7 days. Randomization will be performed using a computerized method, with allocation concealment via sealed, opaque envelopes that will be opened only at the time of the intervention. Trata-se de um estudo clínico randomizado controlado de dois braços, paralelo, duplo cego. Todos os participantes serão inicialmente submetidos a uma fase pré-operatória padronizada, incluindo orientação de higiene bucal, raspagem supragengival e profilaxia profissional. Os participantes serão randomizado previamente em dois grupos: controle (n=25) (tratado pela técnica convencional) ou teste (n=25) (utilizado o Blades Contour para a incisão principal da gengivectomia). A sequência operatória será padronizada para ambos os grupos. Será realizada anestesia infiltrativa terminal utilizando lidocaína 2% com adrenalina 1:100.000. Depois, será feita a marcação da profundidade do sulco gengival ou da bolsa periodontal com sonda milimetrada ou pinça de Crane-Kaplan, transferindo-se a medida para a face externa da gengiva por pontos sangrantes. A partir dessa marcação será definida a quantidade de tecido a ser removida. No grupo controle, a incisão primária será feita pela técnica convencional, com gengivótomos periodontais de Kirkland ou Orban, conforme a anatomia da região. A incisão será posicionada cerca de 0,5 a 1,0mm apicalmente aos pontos sangrantes, com angulação adequada à preservação do tecido remanescente e à formação de contorno gengival. Na sequência, ocorrerá a remoção do colarinho gengival e a gengivoplastia com tesouras curvas ou outros instrumentais periodontais. No grupo teste, os mesmos princípios cirúrgicos serão mantidos. A diferença será a utilização do gengivótomo Blades Contour na incisão principal e na excisão do colarinho gengival. Ao final da cirurgia, será realizada irrigação com soro fisiológico. Não será utilizado cimento cirúrgico. Será prescrito paracetamol 500mg, 6/6 horas por 2 dias; e digluconato de clorexidina 0,12%, 2x/dia por 7 dias. A randomização será feita por método computadorizado, com ocultação da alocação em envelopes opacos e lacrados, que serão abertos apenas no momento da intervenção. O operador não será cegado, mas paciente e avaliador serão cegados

Age over 18 years; good general and oral health; aesthetic complaint regarding a gummy smile in the second sextant; indication for external-bevel gingivectomy/gingivoplasty in the maxillary anterior region; 3 mm band of keratinized gingiva; consent to participate in the study
