This is a clinical trial with a convenience sample of children aged 6 to 8 years with speech sound substitutions. Group randomization was performed by the researchers and protected by the "randombetween" function in Google Sheets, through randomization with minimization by the degree of speech severity, to ensure better homogeneity in the sample. The control group, composed of four participants, underwent phonological-based therapy with the Cycles Model, while the experimental group, composed of 5 participants, underwent phonological-based therapy with the addition of executive function activities, as proposed by the CAFE Model (Cycles Associated with Executive Functions). The pre-intervention assessment period consisted of three meetings, of up to 60 minutes each, in which assessments of language, phonology, cognitive development screening, orofacial motor skills, and hearing were applied. Therefore, after a series of pre-intervention assessments, the children who met the inclusion criteria for the research began an intervention phase. The speech-language therapy clinical intervention lasted 15 sessions, held weekly and lasting 40 minutes each. The therapies were planned, organized, and scripted by the researchers and applied by volunteer speech-language pathology students, who received prior training. All research participants were blinded to the intervention models of their respective groups, and their guardians did not receive any guidance on conducting speech-language therapy activities outside the therapeutic environment, in order to avoid possible contamination

Trata-se de um ensaio clínico, com amostro por conveniência e população de crianças de 6 a 8 anos de idade com trocas de sons na fala. A aleatoriedade dos grupos foi realizada pelos pesquisadores e garantida pela função “aleatórioentre” na ferramenta Planilhas Google, por meio de uma randomização com minimização pelo grau de severidade de fala; para garantir melhor homogeneidade na amostra. O grupo controle, composto por quatro participantes, foi submetido à terapia de base fonológica com Modelo de Ciclos, enquanto o grupo experimental, composto por 5 participantes, foi submetido à terapia de base fonológica com acréscimo de atividades de funções executivas, proposta do Modelo CAFE (Ciclos Associado às Funções Executivas). O período de avaliações pré-intervenção ocorreu em três encontros, de até 60 minutos cada, nos quais foram aplicadas avaliações de linguagem, fonologia, rastreio do desenvolvimento cognitivo, motricidade orofacial e audição. Sendo assim, após a bateria de avaliações pré-intervenção, as crianças que se enquadravam nos critérios de inclusão da pesquisa iniciavam a etapa de intervenção. A intervenção clínica fonoaudiológica teve duração de 15 sessões com frequência semanal e duração de 40 minutos cada. As terapias foram planejadas, organizadas e roteirizadas pelos pesquisadores e aplicadas por acadêmicos de fonoaudiologia voluntários, os quais receberam treinamento prévio. Todos os participantes da pesquisa foram cegados em relação aos modelos de intervenção dos respectivos grupos e os responsáveis não receberam qualquer orientação para a realização de atividades fonoaudiológicas fora do ambiente terapêutico, a fim de evitar possível contaminação

Of the 72 individuals who showed interest in participating in the research, 51 were excluded before the start of pre-intervention assessments. In the evaluations, 10 children were excluded because they met one of the research exclusion criteria. Thus, 12 children were able to start the interventions, therefore, they were randomized to the control (traditional cycle model) and experimental (CAFE model) groups. At this stage, three children were excluded, one who had completed a session and dropped out because the family had moved to another city and was unable to travel to the research site; and two other children, one who had attended three sessions and the other 11, both were removed from the research due to loss of connection. Thus, nine children made up the sample for the present study. The research was terminated early because it was part of a student's Final Course Project and was therefore tied to the institutional schedule. As a result, participant recruitment and the application of procedures were interrupted due to the established deadline for the completion and presentation of the work.

Dos 72 indivíduos que demonstraram interesse em participar da pesquisa, 51 foram excluídos antes do início das avaliações pré-intervenção. Nas avaliações, 10 crianças foram excluídas por apresentarem algum dos critérios de exclusão da pesquisa. Dessa forma, 12 crianças estavam aptas para dar início às intervenções, portanto, foram randomizadas para os grupos controle (modelo de ciclos tradicional) e experimental (modelo de CAFE). Nessa etapa, três crianças foram excluídas, uma que havia realizado uma sessão e por desistiu porque a família havia mudado para outra cidade e não tinha condições de deslocamento até o local da pesquisa; e outras duas crianças, uma que havia realizado três atendimentos e a outra 11, ambas foram desligadas da pesquisa por perda de vínculo. Assim, nove crianças compuseram a amostra do presente estudo. A pesquisa foi encerrada de forma antecipada por integrar o Trabalho de Conclusão de Curso de uma discente, estando, portanto, vinculada ao cronograma institucional. Dessa forma, o recrutamento de participantes e a aplicação dos procedimentos foram interrompidos em virtude da data limite estabelecida para a conclusão e apresentação do trabalho.

Staying in the sealed acoustic booth may cause mild and momentary discomfort. The discomfort and risk expected during therapeutic procedures are minimal and limited to the child's stay in the University laboratory room during the session period due to possible fatigue.

Student's t-test for paired data and McNemmar's test were used to compare pre- and post-intervention differences in each group, and Student's t-test, Fisher's exact test and chi-square test were used to compare variables with one assessment and post-intervention between groups. Results with p-value <0.05 were considered significant. To calculate the effect size of the intervention within and between groups, Cohen's d measure was used, which can be considered as small (d>0.2), medium (d>0.5) and large (d>0.8) effect sizes. Cohen's d measure of effect size demonstrates that the results obtained with the intervention applied in the experimental group showed greater pre- and post-intervention effects when compared to the control group. In the comparison between groups, the percentage of correct consonants and the syllabic positions of initial complex onset, medial complex onset and final coda were variables with large effect size. When performing the statistical analysis of the data, it was not possible to observe a significant difference between the groups after the intervention in relation to the phonological processes and the phonological system and PCC. Furthermore, the degree of speech severity did not present statistically significant changes before and after the intervention in the groups and between the groups after the intervention. For the experimental group, it was not possible to apply the statistical test in final coda since the participants maintained the same amount of phonemes before and after the intervention. The same occurred with the control group in medial coda. There was no statistically significant change between the groups in these syllabic positions. It was also not possible to compare the pre and post-intervention data in initial complex onset and medial complex onset between the groups, since the control group maintained zero productions in both syllabic positions before and after the intervention.

Foram utilizados os testes t de Student para dados pareados e McNemmar para comparar as diferenças pré e pós em cada grupo e os testes t de Student, Exato de Fisher e Qui-Quadrado para a comparação das variáveis com uma avaliação e da avaliação pós entre os grupos. Foram considerados significativos os resultados com p-valor<0.05. Para calcular o tamanho do efeito da intervenção nos grupos e entre eles foi utilizada a medida “d” de Cohen, na qual podem ser considerados como tamanhos de efeitos pequenos (d>0,2), médios (d>0,5) e grandes (d>0,8). A medida “d” de Cohen do tamanho de efeito demonstra que os resultados obtidos com a intervenção aplicada no grupo experimental evidenciaram maiores efeitos pré e pós-intervenção quando comparado ao grupo controle. Na comparação entre os grupos, o percentual de consoantes corretas e as posições silábicas de onset complexo inicial, onset complexo medial e coda final foram variáveis com grande tamanho de efeito. Ao se fazer a análise estatística dos dados, não foi possível observar diferença significativa pós intervenção entre os grupos em relação aos processos fonológicos e ao sistema fonológico e PCC. Além disso, o grau de severidade de fala não apresentou mudanças estatisticamente significativas pré e pós intervenção nos grupos e entre os grupos no pós-intervenção. Para o grupo experimental, não foi possível aplicar o teste estatístico em coda final uma vez que os participantes mantiveram a mesma quantidade de fonemas pré e pós-intervenção. O mesmo ocorreu com o grupo controle em coda medial. Não houve alteração estatisticamente significativa entre os grupos nessas posições silábicas. Também não foi possível comparar os dados pré e pós-intervenção em onset complexo inicial e onset complexo medial entre os grupos, já que o grupo controle manteve zero produções em ambas as posições silábicas pré e pós-intervenção.

Purpose: To compare the effectiveness of two therapeutic intervention models: one using the conventional Cycle Model and the other the CAFE Model, which is structurally similar to the Cycle Model but includes executive function activities. Methods: Nine children aged between six to eight years old diagnosed with Phonological Disorder participated in the study, divided into two groups: a control group, consisting of four children who received phonological-based therapy, and an experimental group, consisting of five children, received phonological-based therapy with additional executive function activities. The intervention lasted for 15 weeks, with weekly sessions. The results were statistically analyzed. Results: All children in the study showed improvement in their percentage of correct consonants, with greater improvement noted in the experimental group. Four participants in the experimental group and two in the control group achieved the selected targets. Participants in the experimental group showed a greater reduction in the production of phonological processes compared to the control group. All participants in the experimental group reduced their speech severity, while only half of the control group did so. All children in the experimental group showed positive changes in their phonological system. In contrast, one child in the control group maintained the same number of phonemes before and after intervention, while showed a decrease in the number of final coda phonemes post-intervention. Conclusion: The findings from the experimental group suggest that combining traditional phonological therapy with executive function activities can benefit and enhance the treatment of children with Phonological Disorder.

Objetivo: Comparar a efetividade de dois modelos de intervenção terapêutica, um utilizando o Modelo de Ciclos convencional e o outro Modelo CAFE, cuja estrutura é semelhante ao Modelo de Ciclos, com acréscimo de atividades de funções executivas. Método: Participaram da pesquisa nove crianças de seis a oito anos diagnosticadas com Transtorno Fonológico, divididas em dois grupos: grupo controle, composto por quatro crianças, submetido à terapia de base fonológica; e experimental, composto por cinco crianças, submetido à terapia de base fonológica com acréscimo de atividades de funções executivas. A intervenção ocorreu por 15 semanas, com frequência semanal. Os resultados foram analisados estatisticamente. Resultados: Todas as crianças da pesquisa apresentaram melhora em seu percentual de consoantes corretas, com destaque maior para o grupo experimental. Quatro participantes do grupo experimental e dois do grupo controle adquiriram os alvos selecionados. Os participantes do grupo experimental apresentaram maior diminuição na produção de processos fonológicos, quando comparados ao grupo controle. Todos os participantes do grupo experimental diminuíram seu grau de severidade de fala, enquanto apenas metade do grupo controle o fez. Todas as crianças do grupo experimental apresentaram mudanças positivas em seu sistema fonológico. Já no grupo controle, uma criança manteve a mesma quantidade de fonemas pré e pós-intervenção e outra teve diminuição na quantidade de fonemas que possuía em coda final no pós-intervenção. Conclusão: Os achados do grupo experimental indicam que associar o modelo de terapia fonológica tradicional com atividades de funções executivas pode beneficiar e maximizar o tratamento de crianças com Transtorno Fonológico.
