This study is a two-arm cluster-randomized controlled trial. A total of 15 Primary Care Health settings in São Paulo City, Brazil, were randomized. Surveys were conducted before the intervention and again at 90, 180, and 360 days after baseline. The intervention group (n=7 primary care health units) received a brief intervention using the FAES model (Feedback, Advice, Empathy, and Self-efficacy). The control intervention (n=8 primary care health units) received a brief intervention based on the FRAMES model (Feedback, Responsibility, Advice, Menu of options, Empathy, and Self-efficacy). Counseling time in both situations did not exceed 8 min. We obtained baseline data from 202 participants who consulted a Primary Health Care Center for any reason.

Trata-se de um ensaio clínico randomizado por conglomerados com dois braços. Um total de 15 unidades de Atenção Primária à Saúde na cidade de São Paulo, Brasil, foi randomizado. Foram realizadas avaliações antes da intervenção e, novamente, aos 90, 180 e 360 dias após a linha de base. O grupo de intervenção (n=7 unidades de atenção primária à saúde) recebeu a intervenção breve modelo FAES (Feedback, aconselhamento, empatia e autoeficácia). O grupo controle (n=8 unidades de atenção primária à saúde) recebeu a intervenção breve baseada no modelo FRAMES (Feedback, responsabilização, aconselhamento, menu de opções, empatia e autoeficácia). Em ambas as situações, o tempo de aconselhamento não ultrapassou 8 minutos. Obtivemos dados de linha de base de 202 participantes que buscaram atendimento em uma unidade de Atenção Primária à Saúde por qualquer motivo.

Expected outcome 1: A reduction of 2-points in the Alcohol Use Disorder Identification Test - Concise (AUDIT-C) in those individuals who received the brief FAES model intervention (Feedback, Counseling, Empathy, and Self-Efficacy) was expected during post-test assessments. Differences were assessed using a linear mixed-effects model for repeated measures.

The participants in this study had an average age of 39.65 years (±14.22). 53.23% (n=107) were women, 59.30% (n=118) self-declared as black/brown, 40.59 % (n=82) single, and 36.8 % (n=3) with a family income of less than US$607.20. Regarding the patterns of alcohol use, the AUDIT mean was 11.3 (±3.1) for both groups. Risky use was the most prevalent (CG=84%; IG= 88%)

A total of 4597 participants were screened; of these, 3060 were excluded for not meeting the inclusion criteria. Of the 706 potentially eligible participants, another 504 were excluded, resulting in a sample of 202 participants who were randomized into two groups. 77 participants were assigned to the experimental group; 64 completed the intervention. A control group of 125 participants was allocated; thus, 2 participants did not receive the intervention, and 80 completed the study

In the Mixed Model analysis, there was a significant result on the main effect of time, it suggests that there was a change in the AUDIT-C variable across different time points between both groups (P<0.001). Aims: We aimed to assess the effectiveness of a simplified Brief Intervention, configured by Feedback, Advice, Empathy, Self-efficacy (FAES), delivered by telephone to reduce alcohol consumption. Design: We conducted a two-arm cluster-randomized controlled trial. Primary Care Health settings were randomized. Surveys were conducted before the intervention and again at 90, 180, and 360 days after baseline. The intervention group received the FAES model. The control group received usual care based on the FRAMES model. Setting: A total of 15 Primary Care Health settings in São Paulo City, Brazil. Participants: We obtained baseline data from 202 randomizable participants who consulted a Primary Health Care Center for any reason. All provided written informed consent and completed valid interviews. The retention rate was 71% over 360 days. Intervention and comparator: The experimental group's brief intervention had elements of FAES: feedback, advice, empathy, and self-efficacy. Counseling time did not exceed 8 min. The control intervention used the FRAMES model, adding responsibility and a menu of options. Findings: At follow-up, when analyzed on an intention-to-treat basis, treatment outcomes showed similar effectiveness between the FAES and FRAMES models, indicating that the new proposal reduces alcohol consumption in the population served by Primary Health Care facilities in Brazil. Conclusions: The results showed a positive effect on behavior change related to alcohol use. Our study begins to recognize the potential of new technology in health care and uncovers the many possibilities of using these tools as strategies to prevent drinking problems or complications related to alcohol use in Primary Health Care.

Delineamento: Realizamos um ensaio clínico controlado, randomizado por conglomerados, com dois braços. Unidades de Atenção Primária à Saúde foram randomizadas. Foram realizadas avaliações antes da intervenção e novamente aos 90, 180 e 360 dias após a linha de base. O grupo de intervenção recebeu o modelo FAES. O grupo controle recebeu o cuidado habitual baseado no modelo FRAMES. Local: Um total de 15 unidades de Atenção Primária à Saúde na cidade de São Paulo, Brasil. Participantes: Obtivemos dados de linha de base de 202 participantes elegíveis para randomização que consultaram uma unidade de Atenção Primária à Saúde por qualquer motivo. Todos forneceram consentimento livre e esclarecido por escrito e completaram entrevistas válidas. A taxa de retenção foi de 71% ao longo de 360 dias. Intervenção e comparador: A intervenção breve do grupo experimental continha elementos do FAES: feedback, aconselhamento, empatia e autoeficácia. O tempo de aconselhamento não excedeu 8 minutos. A intervenção controle utilizou o modelo FRAMES, acrescentando os elementos responsabilidade e um menu de opções. Resultados: No acompanhamento, ao analisar com base na intenção de tratar, os desfechos do tratamento demonstraram eficácia semelhante entre os modelos FAES e FRAMES, indicando que a nova proposta reduz o consumo de álcool na população atendida pelas unidades de Atenção Primária à Saúde no Brasil. Conclusões: Os resultados mostraram um efeito positivo na mudança de comportamento relacionada ao uso de álcool. Nosso estudo começa a reconhecer o potencial de novas tecnologias na assistência à saúde e revela as diversas possibilidades de utilização dessas ferramentas como estratégias para prevenir problemas com a bebida ou complicações relacionadas ao uso de álcool na Atenção Primária à Saúde.
