  Experimental group: 45 individuals with obesity BMI 40 kg/m² (regardless of the presence of comorbidities) or 35 kg/m² with comorbidities associated with obesity (Diabetes Mellitus, Systolic Arterial Hypertension, Dyslipidemia, Coronary Artery Disease, Cerebral Pseudotumor, Obstructive Sleep Apnea, Asthma, Venous Stasis, Severe Urinary Incontinence, Debilitating Arthritis, Gastroesophageal Reflux Disease, and Non-Alcoholic Liver Steatosis) with indication for Bariatric and Metabolic surgery (preferably with the gastric bypass technique). Control group: 30 eutrophic individuals (BMI between 18.5 and 24.9 kg/m²) with indication for Cholecystectomy, without inflammatory or metabolic conditions. Both groups will have about 1 g of visceral adipose tissue collected intraoperatively from the region surrounding the gastrointestinal tract that will undergo surgical manipulation for histopathological tissue analysis, this sample will be stored in formalin, shaped in a paraffin block and the slides will have cuts transversals that will be stained with hematoxylin and eosin, to observe the presence and/or absence of inflammatory cells, tissue degeneration, volume, and amount of cellularity. Blood collection and analysis will be performed before surgery and 3, 6, and 12 months after surgery for the intervention group and will be collected before surgery for the control group. 18 ml of blood will be collected from patients fasting for 8 hours, by peripheral puncture of the forearm vein, to measure adiponectin, leptin, total cholesterol and triglyceride fractions, insulin, and blood glucose. In the intervention group, the assessment of food intake will be collected through a 3-day non-consecutive food record, including a weekend day and two weekdays, in the periods before the surgical procedure and 3, 6, and 12 months after surgery; in the control group, the food record will be collected before surgery. Both groups will undergo body composition to assess body mass, body fat, fat-free mass, and body volume, which will be performed by air displacement plethysmography; muscle mass, visceral fat, and degree of obesity will be evaluated by multifrequency bioimpedance; height will be assessed by a stadiometer with a 0.1cm precision scale; waist and hip circumference will be assessed by an inelastic measuring tape and Metabolic Rest Rate (RMR) will be assessed by indirect calorimetry using a portable metabolic analyzer.

  Grupo experimental: 45 indivíduos com obesidade IMC 40 kg/m² (independente da presença de comorbidades) ou 35 kg/m² com comorbidades associadas à obesidade (Diabetes Mellitus, Hipertensão Arterial Sistólica, Dislipidemia, Doença Arterial Coronariana, Pseudotumor Cerebral, Apneia Obstrutiva do Sono, Asma, Estase venosa, Incontinência Urinária Severa, Artrite Debilitante, Doença do Refluxo Gastroesofágico e Esteatose Hepática Não Alcoólica) com indicação para realização de cirurgia Bariátrica e Metabólica (de preferência com a técnica do bypass gástrico). Grupo controle: 30 indivíduos eutróficos (IMC entre 18,5 e 24,9 kg/m²) com indicação de Colecistectomia, sem quadros inflamatórios ou metabólicos. Ambos os grupos terão cerca de 1 g de tecido adiposo visceral coletado no momento intraoperatório da região circundante do trato gastrointestinal que ocorrerá a manipulação cirúrgica para análise histopatológica do tecido, essa amostra será armazenada em formalina, enformado em bloco de parafina e as lâminas terão cortes transversais que serão coradas com hematoxilina e eosina, para observação de presença e/ou ausência de células inflamatórias, degeneração tecidual, volume e quantidade de celularidade. A coleta e análise sanguínea serão realizadas antes da cirurgia e depois de 3, 6 e 12 meses da cirurgia para o grupo intervenção e serão coletadas antes da cirurgia para o grupo controle. Serão coletados 18 ml de sangue com os pacientes em jejum de 8 horas, por punção periférica da veia do antebraço, para realizar a dosagem de adiponectina, leptina, colesterol total e frações triglicérides, insulina e glicemia. No grupo intervenção a avaliação da ingestão alimentar será coletado por registro alimentar de 3 dias, não consecutivos, incluindo um dia de fim de semana e dois dias de semana, nos períodos antes do procedimento cirúrgico e 3, 6 e 12 meses após a cirurgia; no grupo controle, o registro alimentar será coletado antes da cirurgia. Ambos os grupos passarão pela composição corporal para avaliação da massa corporal, gordura corporal, massa livre de gordura e volume corporal que serão realizadas por pletismografia por deslocamento de ar; massa muscular, gordura visceral e grau de obesidade serão avaliadas por bioimpedância de multifrequência; a estatura será avaliada por um estadiômetro com escala de precisão de 0,1cm; circunferência da cintura e quadril será avaliada por uma fita métrica inelástica e Taxa Metabólica de Repouso (TMR) será avaliada por calorimetria indireta utilizando um analisador metabólico portátil.
