15 patients will be evaluated in a single arm, without masking or randomization. Cerebral oximetry will be recorded during acute normovolemic hemodilution, in a descriptive manner only.

The target population was patients undergoing general anesthesia for major non-cardiac surgeries; age were between 18 and 65 years old; classified as class I or II by the American Society of Anesthesiology; both male and female

Exclusion criteria:

Patients who refused or withdrew from participation were excluded. Patients with arrhythmias; heart failure; previous heart attacks; restrictive or obstructive lung diseases; significant renal dysfunction (creatinine clearance < 60 mg/L/min) or who had an oliguric or anuric presentation were also excluded; Patients with a preoperative hematocrit lower than 30% were also excluded, except in contexts in which the patient refused to receive blood transfusions, making this limitation less restrictive given the benefit in these cases; Patients undergoing laparoscopic surgeries; neurosurgeries; Surgeries in which neuraxial anesthesia was associated intraoperatively were also excluded

Results summary:

Blood component transfusion is associated with negative outcomes in various contexts. However, its necessity is also well established in others. Regardless of the choice to transfuse or not, the search for effective, safe, and reproducible strategies in the perioperative blood management setting has gained relevance within this controversy. Among patient blood management strategies, acute normovolemic hemodilution (ANH) stands out as the only method involving autologous whole blood that is not previously stored, preserving red blood cells, platelets, coagulation factors, and 2,3-diphosphoglycerate (2,3-DPG). However, by inducing profound anemia, ANH can reduce oxygen delivery to tissues, raising concerns particularly about cerebral oxygenation.
This single-center prospective study aimed to investigate the impact of acute normovolemic hemodilution (ANH) on cerebral oxygenation in 15 patients undergoing standardized general anesthesia, using cerebral oximetry (rSO₂) measured by near-infrared spectroscopy (NIRS). Our study found that rSO₂ significantly changed only when hematocrit (HTC) levels dropped below 28.7% of baseline values. We also identified a linear correlation with an R² index of 0.27 and p<0.0001, establishing a relationship between the decrease in rSO₂ values and hematocrit levels. Lactate levels did not show significant alterations, and central venous oxygen saturation (ScvO₂) only decreased significantly when Hct dropped below 30.7% of baseline values, though still within clinically acceptable levels. Thus, it was demonstrated that ANH leads to a reduction in rSO₂; however, when performed with hct values as low as 25% there were no signs of systemic oxygenation imbalancept-br
A transfusão de hemocomponentes é associada a desfechos negativos em diversos contextos. Entretanto, também está estabelecida a sua necessidade em outros. Independentemente da opção pela transfusão ou não, a busca por estratégias eficazes, seguras e reprodutíveis no manejo de hemocomponentes no perioperatório ganhou relevância dentro dessa controvérsia. Dentre as estratégias de gerenciamento de sangue do paciente, a hemodiluição normovolêmica aguda (HNA) tem destaque, sendo o único método com sangue total autólogo não armazenado previamente, conferindo a essa técnica a conservação de hemácias, plaquetas, fatores de coagulação e 2,3-difosfoglicerato (2,3-DPG). Contudo, a HNA ao induzir estados de anemia profundos pode gerar diminuição da oferta de oxigênio aos tecidos, trazendo questionamentos principalmente sobre a oxigenação cerebral.
Este estudo unicêntrico prospectivo se propôs a investigar o impacto da HNA na oxigenação cerebral de 15 pacientes submetidos a anestesia geral padronizada através da oximetria cerebral (rSO2) aferida pela espectroscopia próxima ao vermelho (near infrared spectroscopy - NIRS). Nosso estudo encontrou que a rSO2 se alterou significativamente somente com quedas do hematócrito (HTC) a partir de 28,7% dos valores basais. Encontramos ainda uma correlação linear com índice R2 de 0,27 e p<0,0001 estabelecendo uma relação entre a queda dos valores de rSO2 e de hematócrito. A lactatemia não teve alteração significativa, já a queda da saturação venosa central só ocorreu de maneira significativa com quedas a partir de 30,7% do valor basal de HTC, ainda assim em níveis clinicamente aceitáveis. Com isso, ficou demonstrado que a HNA promove diminuição da rSO2, mas que quando performada em valores até de 25% de Htc não houve sinais de desbalanço de oxigenação no corpo
