Patients diagnosed with severe peri-implantitis (≥50% bone loss) will be invited to participate in the study and, upon agreement, will be required to sign a written informed consent form. Implants will be randomly allocated using a simple randomization procedure by manual drawing. To ensure allocation concealment, the names of the respective groups will be printed on identical cards and placed in opaque, sealed envelopes. An independent researcher, not directly involved in the surgical procedures, will be responsible for shuffling and controlling the envelopes. The envelope corresponding to the implant under treatment will be selected by drawing and opened only at the time of the intervention.The clinical parameters to be assessed will include peri-implant recession, probing depth, bleeding on probing, suppuration, plaque index, gingival index, and keratinized mucosa width. All clinical measurements will be performed before surgery by a calibrated independent examiner using a millimeter periodontal probe. All surgical procedures will be performed by a periodontist following strict biosafety protocols. Under local anesthesia, intrasulcular and vertical incisions will be made, followed by removal of the inflammatory tissue. The extent of the bone defect will be confirmed using a periodontal probe. To standardize the spatial orientation for subsequent microscopic analyses, the buccal aspect of the implant platform will be marked with a round bur. At this stage, the corresponding randomization envelope will be opened to reveal the treatment allocation. In the control group, the implant will be immediately explanted using a reverse torque device without any prior mechanical instrumentation. In the test group, macroscopic mineralized deposits and biofilm will be removed using polyether-ether-ketone tips followed by conventional ultrasonic tips. Subsequently, the implant surface will undergo air polishing with glycine powder. A perioair-polishing handpiece will be used under 6-bar pressure with maximum water flow. The nozzle will be moved horizontally along the implant threads in a corono-apical direction while maintaining a working distance of approximately 2 millimeters. The angulation of the air-polishing jet may be adjusted according to the defect morphology and the area being treated. The total active instrumentation time will be strictly timed and recorded. The implant will then be explanted using the reverse torque device. Immediately after removal, the implants will be immersed in distilled water to eliminate residual blood. The surgical socket will be curetted and sutured. Patients will receive detailed postoperative instructions, individualized medication prescriptions, and suture removal will be performed after 14 days. Patients will be followed until complete healing has been achieved. Following explantation, each implant will be positioned under a stereomicroscope for the acquisition of high-resolution digital images of the buccal surface (previously identified during surgery). During image acquisition, a millimeter periodontal probe will be positioned adjacent to the implant in the same plane to serve as a calibration reference. The images will be coded, exported, and analyzed using ImageJ software by an examiner blinded to group allocation. The region of interest will be defined according to previous clinical and radiographic findings and will include the supramucosal area (corresponding to peri-implant recession), the submucosal area (corresponding to the peri-implant pocket and subdivided into three equal thirds), and the intraosseous area (the previously osseointegrated portion of the implant, which will not be included in the analysis). The examiner will perform planimetric delineation of the areas exhibiting visible residual biofilm or mineralized deposits. The primary outcome will be quantified and expressed as the percentage of decontaminated implant surface area. Subsequently, the remaining bacterial biofilm on the implant surface will be fixed in glutaraldehyde buffered with sodium cacodylate. After fixation, the specimens will undergo gradual dehydration in ascending ethanol solutions remaining for 10 minutes in each concentration. The final immersion in absolute ethanol will be repeated three times. Final specimen drying will be performed using the critical-point drying technique. Thereafter, the samples will undergo sputter coating to provide surface conductivity. This procedure will be performed at the Research Support Centers Complex of the State University of Maringá by an experienced technician. The morphology and surface topography of the implants will be evaluated using scanning electron microscopy, allowing high-resolution visualization of structures compatible with residual bacterial biofilm as well as detailed inspection of the titanium surface microstructure. Energy-dispersive x-ray spectroscopy will be used for the qualitative and semi-quantitative characterization of the elemental composition of the analyzed surfaces. Images will be acquired from the supramucosal and submucosal regions at both the thread valleys and thread crests under standardized magnifications at three predefined locations. The images will be exported to ImageJ software for bacterial counting by an examiner blinded to the treatment allocation. For this study, a minimum of seven implants per group (test and control) will be required, corresponding to a maximum of 10 patients

Pacientes com diagnóstico de peri-implante severa (50% de perda óssea) serão convidados a participar do estudo e, mediante anuência, deverão assinar o termo de consentimento livre e esclarecido. O implante será randomizado através de randomização simples por sorteio manual. Para assegurar a ocultação da alocação, os nomes dos respectivos grupos serão impressos em cartões idênticos e acondicionados em envelopes opacos e selados. Um pesquisador independente, não envolvido de forma direta nos procedimentos cirúrgicos, será responsável por embaralhar e controlar os envelopes. O envelope correspondente ao implante em tratamento será sorteado e aberto estritamente no momento da intervenção. Os parâmetros clínicos avaliados incluirão: recessão peri-implantar, profundidade de sondagem, sangramento à sondagem, supuração, índice de placa, índice gengival e largura da mucosa queratinizada. Todas as aferições clínicas serão realizadas previamente à cirurgia por um examinador independente e calibrado, utilizando uma sonda periodontal milimetrada. Todos os procedimentos cirúrgicos serão executados por um periodontista sob rigorosos protocolos de biossegurança. Sob anestesia local, incisões intrasulculares e verticais serão realizadas, seguido pela remoção do tecido inflamatório. A profundidade da perda óssea será confirmada com o uso da sonda periodontal. Para padronizar a orientação espacial nas análises microscópicas subsequentes, a face vestibular da plataforma do implante será demarcada utilizando uma broca esférica. Neste momento, o envelope de randomização correspondente será aberto para revelar a alocação do implante. No grupo controle o implante será imediatamente explantado utilizando um dispositivo de torque reverso, sem qualquer instrumentação mecânica prévia. No grupo teste, os depósitos minerais macroscópicos e o biofilme serão removidos utilizando pontas ultrassônicas revestidas por poliéter-éter-cetona e pontas ultrassônicas. Sequencialmente ao uso prévio das pontas ultrassônicas, a superfície do implante será submetida ao jateamento de ar com pó de glicina. Será utilizado a peça de mão airpolishing-perio, sob pressão de 6 bar e fluxo máximo de água. A ponta aplicadora será movimentada horizontalmente ao longo das espiras, em sentido corono-apical, mantendo uma distância de trabalho de aproximadamente 2 milímetros. A angulação do jato poderá ser adaptada em função da anatomia do defeito e da área tratada. O tempo total de instrumentação ativa será rigorosamente cronometrado e registrado. Em seguida, será realizado a explantação com o dispositivo de torque reverso. Imediatamente após a remoção, os implantes serão imersos em água destilada para a remoção do sangue residual, O alvéolo cirúrgico será curetado e suturado. Os pacientes receberão instruções pós-operatórias detalhadas, prescrição medicamentosa individualizada e a remoção das suturas ocorrerá após 14 dias e acompanhados até a completa cicatrização. Após a explantação, o implante será posicionado em um estereomicroscópio para a aquisição de imagens digitais de alta resolução da face vestibular (previamente demarcada no intraoperatório). Durante a aquisição fotográfica, uma sonda periodontal milimetrada será posicionada adjacente no mesmo plano para servir como referência de escala. As imagens obtidas serão codificadas, exportadas e analisadas no software ImageJ por um examinador cego quanto à alocação dos grupos. A região de interesse será delimitada com base nos dados clínicos e radiográficos prévios, seguindo os critérios: área supramucosa (correspondente à medida da recessão peri-implantar), área submucosa (correspondente à bolsa peri-implantar, subdividida em três terços) e área intraóssea (porção do implante previamente osseointegrada que não será contabilizada). O examinador delimitará planimetricamente as áreas exibindo presença visível de biofilme residual ou depósitos mineralizados. O desfecho será quantificado e expresso como a porcentagem de área de superfície descontaminada. Em seguida, o biofilme bacteriano remanescente na superfície dos implantes será fixado em solução de glutaraldeído a 2,5% tamponada com cacodilato de sódio. Após a fixação, as amostras serão submetidas a um processo de desidratação gradual em uma série crescente de soluções aquosas de etanol permanecendo por 10 minutos em cada concentração. A permanência em etanol absoluto será repetida três vezes. A secagem final das amostras será realizada pela técnica de ponto crítico. Ao término, as amostras serão submetidas ao processo de metalização para conferir condutividade à superfície, procedimento que será executado nas instalações do Complexo de Centrais de Apoio à Pesquisa da Universidade Estadual de Maringá por um profissional experiente. A avaliação da morfologia e da topografia superficial dos implantes será conduzida utilizando um microscópio eletrônico de varredura e permitirá a visualização em alta resolução de estruturas compatíveis com o biofilme bacteriano residual, bem como a inspeção detalhada das características microestruturais da superfície de titânio. A espectroscopia de raios-x por dispersão de energia será empregada para a identificação qualitativa e semiquantitativa da composição elementar da superfície analisada. Serão obtidas imagens para a região supramucosa e submucosa nos vales e topo das roscas do implante, sob magnificações padronizadas em 3 posições. As imagens serão exportadas para o software ImageJ e passarão por processos de contagem dr bactérias por um examinador cego. Para esse estudo, um mínimo de 7 implantes no grupo teste e no grupo controle serão necessários, contabilizando no máximo 10 pacientes
