Blinded randomized controlled trial (analytical, longitudinal and prospective). Initially, a total of 56 participants participated in the study, comprising 16 men and 40 women, randomly distributed between the control groups (CG, n=28) and intervention group (IG, n=28). After the first few weeks, there were 11 renunciation, 4 elderly men and 7 elderly women abandoned the research, an age range between 74 and 98 years old. The reasons for it were due to lack of interest, aversion to physiotherapy or difficulty adapting to a new, more hectic routine, despite respecting individual rest, eating and medication schedules. At the end of the twelve weeks of physiotherapeutic intervention, the control group had n=24 (18 women and 6 men) and n=21 remained in the intervention group (15 women and 6 men), in an age range between 70 and 100 years. Data analysis was carried out using the final sample number (n=45) for all variables presented. The control group, through a randomized process via an online platform (https://www.randomizer.org/), had 24 participants . This group first performed a warm-up corresponding to a 6-minute walk in a place without obstacles, irregularities in the floor or inappropriate use of footwear (option for closed shoes that the participant usually uses). After this, the protocol was focused on muscle strengthening in abductors, adductors, hamstrings and quadriceps. All exercises mentioned were performed in 3 sets with 10 repetitions in each series and the rest time between sets was 2 minutes . After performing the exercises, there was a period of muscle relaxation with stretching of the muscles of both lower limbs: adductor, abductor, flexor and knee extensor. The estimated total time for carrying out the muscle strengthening protocol was 60 minutes. The estimated initial load, in the elderly participant, was 40%RM with gradual evolution until the final load estimated at 80%RM using shin guards with multiple values of 0.5 kg for this graduation during the 12-week intervention period. To obtain and modulate the %RM, the Holten Diagram was used, in which, through mathematical calculations, the desired values in %RM were discovered (40%RM, for example), by observing the relationship between the number of repetitions performed by the participant and the percentage that this load represented. The intervention group, made up of 21 participants, performed the same muscle strengthening protocol (warm-up and specific exercises that target muscle strength) plus sensorimotor exercises: adhesive star, colored path, rubber step and obstacles on the path.

Participaram do estudo, inicialmente, um total de 56 participantes compreendidos entre 16 homens e 40 mulheres, distribuídos aleatoriamente entre os grupos controle (GC, n=28) e grupo intervenção (GI, n=28).Ao passar das primeiras semanas, houve 11 desistências, 4 idosos e 7 idosas abandonaram a pesquisa, faixa etária compreendida entre 74 e 98 anos. Os motivos das desistências foram por falta de interesse, aversão à fisioterapia ou dificuldade de adaptação a uma nova rotina mais agitada, apesar do respeito aos horários individuais de descanso, alimentação e medicação. Ao término das doze semanas de intervenção fisioterapêutica, o grupo controle apresentou n=24 (18 mulheres e 6 homens) e n=21 permaneceram no grupo intervenção (15 mulheres e 6 homens), numa faixa etária compreendida entre 70 e 100 anos. A análise de dados foi realizada através do número da amostragem final (n=45) para todas as variáveis apresentadas.O grupo controle, através de processo randomizado via plataforma online (https://www.randomizer.org/), teve 24 participantes. Este grupo realizou, primeiramente, aquecimento correspondente a uma caminhada de 6 minutos em local sem obstáculos, irregularidades no piso ou uso não adequado de calçados (opção por calçado fechado de uso habitual do participante). Após isto, o protocolo foi focado no fortalecimento muscular em abdutores, adutores, isquitibiais e quadriceps.Todos os exercícios citados foram executados em 3 sets com 10 repetições em cada série e o tempo de descanso entre as séries foram de 2 minutos. Após a execução dos exercícios, houve um período de relaxamento muscular com alongamento da musculatura de ambos os membros inferiores: adutora, abdutora, flexora e extensora do joelho. O tempo total estimado para a execução do protocolo de fortalecimento muscular foi 60 minutos. A carga inicial estimada, no participante idoso, foi 40%RM com evolução gradativa até a carga final estimada em 80%RM sendo utilizado as caneleiras com valores múltiplos de 0,5 kg para esta graduação durante o período de 12 semanas na intervenção. Para a obtenção e modulação do %RM foi utilizado o Holten Diagram em que através de cálculos matemáticos foram descobertos os valores desejados em %RM (40%RM, por exemplo), ao observar a relação existente entre o número de repetições realizadas pelo participante e o percentual que esta carga representava. O grupo intervenção, composto por 21 participantes, executou o mesmo protocolo de fortalecimento muscular (aquecimento e exercícios específicos atuantes na força muscular) acrescido dos exercícios sensoriomotores: estrela adesiva, caminho colorido, step de borracha e obstaculos no caminho.

Outcome found 1: Regarding functionality, our data directed to Timed Up and Go, show that for both control and intervention groups, the applicability of muscle strengthening training alone or associated with sensorimotor exercises proved to be effective for the elderly population monitored during the period 12 weeks. In the TUG variable, it is shown that there was no significant difference between the CG and IG when analyzing the moments before and after the 12 weeks of intervention. On the other hand, when analyzing the before and after for each group separately, both showed a significant difference (CG p=0.002; IG p<0.001). Outcome found 2: Due to the choice of exercise protocol aimed at muscle strengthening in both the control and intervention groups, it was noticed that there was a significant gain in strength in both. Outcome found 3: When raising the hypothesis that sensorimotor exercises affect balance with greater significance, it was found that the sensorimotor exercises covered in the intervention group induced significant improvements in all aspects related to balance through the SPPB test battery. The SPPB variable was analyzed in the following aspects: balance, gait speed, gait speed time (s) and sit-stand. Initially, a total of 56 participants participated in the study, comprising 16 men and 40 women, randomly distributed between the control groups (CG, n=28) and intervention group (IG, n=28). Four elderly men and seven elderly women withdrew from the study, aged between 74 and 98 years. At the end of the twelve weeks of physiotherapeutic intervention, the control group had n=24 (18 women and 6 men) and n=21 remained in the intervention group (15 women and 6 men), in an age range between 70 and 100 years. It is observed that the majority of participants for both CG and IG are female (75% and 71.42%, respectively) and presented the dominant side as the right [21 participants (87.5%) and 17 participants (80.95%), respectively]. All were considered sedentary, as there was no physical activity in the Institutions and as they were all inmates there was no connection with professional activity outside the homes. They were also not undergoing physiotherapeutic intervention for any associated condition since the beginning of data collection.

When comparing the CG and IG with each other for the moment before and after the 12 weeks of intervention, it is observed that in the evaluated muscle groups (Quadriceps, Abductors, Adductors and Hamstrings) on the dominant and non-dominant side, only the muscle Dominant quadriceps showed a significant difference after 12 weeks of intervention between the two groups (p=0.05). In the TUG variable, there was no significant difference between the CG and IG when analyzing the moments before and after the 12 weeks of intervention. On the other hand, when analyzing the before and after for each group separately, both showed a significant difference (CG p=0.002; IG p<0.001).The variable that symbolizes the difference in data between after and before TUG did not show a significant difference between the two groups. Regarding the risk of falling ascertained using the FES and ABC scales, the CG values 58.3% for both FES-I ≥ 31 and ABC < 67%. The IG presented 57.1% for FES-I ≥ 31 and 61.9% for ABC < 67%.

Introduction: Sensory systems provide the information necessary for a motor response to be provided. Often, muscle weakness and sarcopenia are the most predominant factors for the increase in the rates of comorbidities associated with falls, many elderly people receive only the suggestion of muscle strengthening as professional guidance. In this sense, the objective of this study is to evaluate the effectiveness of a sensorimotor exercise program on proprioceptive acuity, balance, muscle strength, functional mobility and risk of falls in institutionalized elderly. Methodology: 56 participants (84.6±8.4 years) were randomly assigned to the control group (CG, n=28) and intervention group (IG, n=28). The CG performed a protocol based on warm-up, muscle strengthening and calm down and the IG performed the same intervention, with the addition of sensorimotor exercises. Joint Position Sense (SPA) in both limbs at 20º and 45º angles, balance, functional mobility, fear of falling in the elderly and muscle strength of the quadriceps, hamstrings, adductors and abductors in both limbs were evaluated before and after the 12 weeks of intervention. Results: Both groups showed gains in muscle strength. When analyzing functionality through TUG, before and after for each group in isolation, both showed a significant difference (CG p=0.002; IG p<0.001). For the SPPB variable, there were significant differences in the IG regarding balance (p<0.001), gait speed time (s) (p=0.004) and sit-stand (p=0.002). In the SPA, significant differences were recorded for the Absolute Error 45º No Dom (p=0.045) and Relative Error 45º No Dom (p=0.045) in the CG and Relative Error 45º No Dom for IG (p=0.018). In the variable FES-I, there were significant improvements in the CG (p=0.006) and in the IG (p=0.002). However, only in the IG there were significant improvements (p = 0.013) for ABC in a comparison between before and after the 12-week period of research. Conclusions: The effectiveness of sensorimotor exercises provides the elderly with a gain in balance and positively impacts their confidence when performing their functions. It is concluded that the protocol presented in its different levels of difficulty is effective and important for the quality of life of the institutionalized sedentary elderly.

Introdução: Os sistemas sensoriais fornecem as informações necessárias para que resposta motora seja fornecida. Frequentemente, são a fraqueza muscular e a sarcopenia os fatores mais preponderantes para aumento dos índices de comorbidades associadas às quedas, muitos idosos recebem como orientação profissional, apenas, sugestão de fortalecimento muscular. Nesse sentido, o objetivo deste trabalho é avaliar a efetividade de um programa de exercícios sensoriomotores na acuidade proprioceptiva, equilíbrio, força muscular, mobilidade funcional e risco de quedas em idosos institucionalizados. Metodologia: 56 participantes (84.6±8.4 anos) foram distribuídos aleatoriamente entre os grupos controle (GC, n=28) e grupo intervenção (GI, n=28). O GC realizou protocolo baseado em aquecimento, fortalecimento muscular e relaxamento e o GI realizou a mesma intervenção, com acréscimo dos exercícios sensoriomotores. Foram avaliados o Senso de Posição Articular (SPA) em ambos os membros nas angulações de 20º e 45º, o equilíbrio, a mobilidade funcional, o medo de cair dos idosos e a força muscular dos quadricípites, isquiotibiais, adutores e abdutores em ambos os membros, antes e após as 12 semanas de intervenção. Resultados: Ambos os grupos apresentaram ganho de força muscular. Ao analisar a funcionalidade através do TUG, antes e depois para cada grupo de maneira isolada, ambos apresentaram diferença significativa (GC p=0,002; GI p<0,001). Para a variável SPPB, verificaram-se diferenças significativas no GI no equilíbrio (p<0,001), tempo de velocidade da marcha (s) (p=0,004) e sentar-levantar (p=0,002). No SPA, foram registradas diferenças significativas para o Erro Absoluto 45º Não Dom (p=0,045) e Erro Relativo 45º Não Dom (p=0,045) no GC e Erro Relativo 45º Não Dom para GI (p=0,018). Na variável FES-I verificaram-se melhorias significativas no GC (p=0,006) e no GI (p=0,002). Todavia, apenas no GI se verificaram melhorias significativas (p = 0,013) para ABC numa comparação entre antes e após o período de 12 semanas de pesquisa. Conclusões: A efectividade dos exercícios sensoriomotores proporciona nos idosos ganho de equilíbrio e impacta positivamente na confiança deles ao realizarem suas funções. Conclui-se que o protocolo apresentado em seus diferentes níveis de dificuldade seja eficaz e importante para a qualidade de vida do idoso sedentário institucionalizado.
