This is a clinical study, open-label, non-randomized, single-arm, with digital intervention as the mains objective. The intervention will be offered to all participants at inclusion with no control group. The intervention consists on weekly text messages via WhatsApp during 24 weeks. The messages will remind participants about the use of pre-exposure prophylaxis (PrEP), antiretroviral therapy, or other HIV prevention measures. We will include 400 participants in the sigle study arm. There will me no randmization or masking. Participants will return to the site for clinical visits at weeks 4, 12, 24, 36 and 48. In each visit, participants will be tested for HIV and other sexually transmitted infections (STIs) and complete sociobehavioral questionnaires. The primary outcome of the proposed pilot intervention is viral suppression (if HIV-positive) and initiation and adherence to PrEP (if HIV-negative), with measures taken throughout the continuum of HIV care or PrEP.

Este é um estudo clínico, aberto, não randomizado, de braço único, com intervenção digital como objetivo principal. A intervenção será oferecida a todos os participantes na inclusão, sem grupo controle. A intervenção consiste em mensagens de texto semanais via WhatsApp durante 24 semanas. As mensagens lembrarão os participantes sobre o uso de profilaxia pré-exposição (PrEP), terapia antirretroviral ou outras medidas de prevenção do HIV. Incluiremos 400 participantes no braço único do estudo. Não haverá randomização ou mascaramento. Os participantes retornarão ao local para consultas clínicas nas semanas 4, 12, 24, 36 e 48. Em cada consulta, os participantes serão testados para HIV e outras infecções sexualmente transmissíveis (ISTs) e preencherão questionários sociocomportamentais. O desfecho primário da intervenção proposta é a supressão viral (se HIV positivo) e o início e adesão à PrEP (se HIV negativo), com medidas tomadas ao longo do continuum de cuidados com HIV ou PrEP.

Expected outcome 1: The primary outcome of the proposed pilot intervention is viral suppression (if HIV-positive) and initiation and adherence to pre-exposure prophylaxys (PrEP; if HIV-negative), with measurements throughout the continuum of HIV care or PrEP. We will measure exposure to the intervention, including the number of exposures to WhatsApp messages during the 6-month intervention period. Intervention exposure and outcome data will be analyzed using multivariable logistic generalized estimating equations (GEE) with interchangeable correlation structures to account for repeated measures over time for each participant. The GEE will determine the association between measures of exposure to the intervention (e.g., receipt of intervention, number of intervention messages) and primary outcomes related to the continuum of HIV care and PrEP.

Introduction: Tailored interventions for young (18-24 years) men and non-binary individuals who have sex with men (YMSM/NB) are scarce, despite a disproportionately high HIV burden. Limited data exist on HIV prevention and care continuum among these populations. Objective: This study evaluated HIV care/prevention outcomes before and after a technology-based intervention among Brazilian YMSM/NB. Methods: Conectad@s, an intervention study conducted in Rio de Janeiro, Brazil (November/2021-September/2023), assessed HIV prevention and care outcomes among YMSM/NB over 48 weeks, recruited by respondent-driven sampling. Participants received quarterly visits and were offered weekly technology-based messages from Week 0. Sameday oral pre-/post-exposure prophylaxis (PrEP/PEP) and antiretroviral therapy (ART) were provided as applicable. Results: Among 409 participants (90.5% cis-MSM, 7.3% NB, 2.2% trans-MSM), 70.6% were Black/Pardo, median age 21 years (IQR:20-23), with 369 HIV-negative and 40 living with HIV (LWH), and 50.0% unaware of their HIV status; all accepted the intervention. Six (1.6%) seroconverted during the study. Study retention at W48 was 83.4% (303 HIV-negative, 38 LWH), with 16.6% (68) lost to follow-up. Comparing W0W48, there was an increase in linkage to HIV care (50.0% - 97.4%), ART use (50.0% - 97.4%), and viral suppression (40.0% - 81.6%). Among HIV-negative participants, 0.9% were on PrEP at W0, increasing to 68.7% at W48 (88.0% daily, 12.0% ondemand). Reasons for not using PrEP included difficulties in taking a daily pill (34.1%), not perceiving themselves at HIV risk (28.9%), and not willing to take a daily pill (27.0%). If available, injectable PrEP was the preferred option (35.5% at W0 and 66.1% at W48). Conclusion: The Conectad@s study successfully engaged and retained YMSM/NB over 48 weeks, showing substantial improvement in HIV outcomes, particularly in viral suppression and PrEP use. Simple interventions, adapted to the youth’s reality, combined with expanding PrEP options, may significantly contribute to enhancing HIV outcomes among YMSM/NB.

Objetivo: Este estudo avaliou os resultados dos cuidados/prevenção do HIV antes e depois de uma intervenção baseada em tecnologia entre YMSM/NB brasileiros. Métodos: Conectad@s, um estudo de intervenção realizado no Rio de Janeiro, Brasil (novembro/2021-setembro/2023), avaliou os resultados da prevenção e dos cuidados do HIV entre YMSM/NB ao longo de 48 semanas, recrutados por amostragem impulsionada por respondentes. Os participantes receberam visitas trimestrais e receberam mensagens semanais baseadas em tecnologia a partir da semana 0. Profilaxia pré/pós-exposição (PrEP/PEP) oral no mesmo dia e terapia antirretroviral (ART) foram fornecidas conforme aplicável. Resultados: Entre os 409 participantes (90,5% cis-MSM, 7,3% NB, 2,2% trans-MSM), 70,6% eram negros/pardos, com idade média de 21 anos (IQR: 20-23), sendo 369 HIV negativos e 40 vivendo com HIV (LWH), e 50,0% desconheciam seu status sorológico para o HIV; todos aceitaram a intervenção. Seis (1,6%) soroconverteram durante o estudo. A retenção do estudo na W48 foi de 83,4% (303 HIV-negativos, 38 LWH), com 16,6% (68) perdidos no acompanhamento. Comparando W0W48, houve um aumento na vinculação aos cuidados de HIV (50,0% - 97,4%), uso de ART (50,0% - 97,4%) e supressão viral (40,0% - 81,6%). Entre os participantes HIV negativos, 0,9% estavam em PrEP na W0, aumentando para 68,7% na W48 (88,0% diariamente, 12,0% sob demanda). As razões para não usar a PrEP incluíram dificuldades em tomar um comprimido diariamente (34,1%), não se perceberem em risco de contrair o HIV (28,9%) e não estarem dispostos a tomar um comprimido diariamente (27,0%). Se disponível, a PrEP injetável era a opção preferida (35,5% na semana 0 e 66,1% na semana 48). Conclusão: O estudo Conectad@s envolveu e reteve com sucesso os YMSM/NB durante 48 semanas, mostrando uma melhoria substancial nos resultados do HIV, particularmente na supressão viral e no uso da PrEP. Intervenções simples, adaptadas à realidade dos jovens, combinadas com a expansão das opções de PrEP, podem contribuir significativamente para melhorar os resultados do HIV entre os YMSM/NB.
