During the pre-surgical consultation, data regarding sex, age, general health status, complete blood count (CBC), platelet count, International Normalized Ratio (INR), and glycated hemoglobin will be collected. All patients will undergo a standardized surgical technique supported by scientific evidence, performed in an outpatient setting under local anesthesia and strict biosafety protocols. Eligible study participants will be randomly assigned—using a list of random numbers generated in Microsoft Excel via the `RANDBETWEEN` function—to one of three groups based on the hemostatic agent used: lyophilized gelatin hemostatic sponge (Hemospon®) immediately after tooth extraction (G1); oxidized regenerated cellulose (Equitamp®) immediately after tooth extraction (G2); or tranexamic acid (Transamin®) immediately after tooth extraction (G3). In the postoperative period, all volunteers across all groups will receive supportive medication. Postoperative instructions will include local cooling and the use of an analgesic (paracetamol 500 mg every 8 hours for 24 hours), as this is recommended as rescue medication in methodologies similar to this one (Kyyak et al., 2023). Patients will be instructed to take the medication upon experiencing pain or local discomfort. Postoperative instructions will also be carefully explained to each patient, including maintaining a cold liquid diet for the first 24 hours, practicing rigorous oral hygiene, and avoiding vigorous mouth rinsing to prevent postoperative hemorrhage. Patients will be informed that they should contact the surgeon by telephone in the event of persistent bleeding or if they deem it necessary. In addition, patients will be asked to report any physical symptoms experienced during the study's postoperative periods, such as nausea, vomiting, dizziness, headache, insomnia, and signs of infection. In cases where an infectious process is identified, amoxicillin (500 mg) will be prescribed every 8 hours for 7 days, or an alternative pharmacological regimen more appropriate for the clinical presentation will be used if a confirmed allergy to this drug exists. In the presence of intraoral or extraoral abscesses, a surgical drainage procedure will be performed in conjunction with the aforementioned medication. All patients will undergo a standardized surgical technique supported by scientific evidence, performed in an outpatient setting under local anesthesia and following strict biosafety protocols. A single oral and maxillofacial surgeon will perform all surgical procedures. Removal of the mandibular molars will be performed under local anesthesia using 3% prilocaine with 0.03 IU felypressin (Prilonest; DFL, Brazil), utilizing a total of two or three cartridges, each containing 1.8 mL of the anesthetic solution. A full thickness triangular flap will be raised, followed by peripheral ostectomy using a high-speed bur cooled with 0.9% saline solution. Finally, the surgical wound will be securely closed using 4-0 silk sutures

Em consulta prévia ao procedimento cirúrgico, serão coletados dados referentes a sexo, idade, estado de saúde geral, hemograma, contagem de plaquetas, razão normalizada internacional (INR), hemoglobina glicada. Todos os pacientes serão submetidos a uma técnica cirúrgica padronizada e respaldada por evidência científica, realizada em ambiente ambulatorial, sob anestesia local, seguindo um rigoroso controle de biossegurança. Inicialmente, os indivíduos considerados elegíveis para participar do estudo serão randomicamente alocados através de uma lista de números aleatórios gerados no Microsoft Excel por meio da função randbetween em um de três grupos a depender do uso de agentes hemostáticos: uso de esponja hemostática de gelatina liofilizada (hemospon) imediato após a exodontia (G1), uso de celulose oxidada e regenerada (Equitamp) imediato após a exodontia (G2); uso de ácido tranexâmico (Transamin) imediato após a exodontia (G3). No período pós-operatório, todos os voluntários de ambos os grupos serão assistidos com medicação de suporte. As instruções pós-operatórias incluirão resfriamento local e analgésico (paracetamol 500 mg em intervalos de 8 horas por 24 h) em virtude de ser recomendada como medicação de resgate em metodologias similares a esta, segundo KYYAK et al., 2023. Todos os pacientes serão instruídos a ingeri-la quando perceberem dor ou desconforto local. Instruções pós-operatórias também serão lidas e explicadas com a devida atenção para cada paciente, incluindo seguir dieta líquida e fria durante as primeiras 24 horas, realizar rigorosa higiene oral, e evitar bochechos vigorosos para prevenir a ocorrência de hemorragia pós cirúrgica. Os pacientes serão informados de que deveriam entrar em contato com o cirurgião inicialmente por telefone no caso de sangramento persistente ou se julgarem necessário. Além disso, os pacientes também serão solicitados a relatar quaisquer sintomas físicos experimentados durante os períodos pós-operatórios do estudo, tais como náusea, vômito, tontura, cefaleia, insônia e sinais de infecção. Nos casos em que se constate a presença de processo infeccioso, será prescrita amoxicilina 500mg em intervalos de 8 horas, por 7 dias, ou outro esquema farmacológico mais apropriado ao quadro apresentado caso seja constatada comprovada alergia a esse fármaco. Quando da presença de abscesso intraorais ou extraorais, será realizado procedimento cirúrgico de drenagem em associação à prescrição medicamentosa anteriormente citada. Todos os pacientes serão submetidos a uma técnica cirúrgica padronizada e respaldada por evidência científica, realizada em ambiente ambulatorial, sob anestesia local, seguindo um rigoroso controle de biossegurança. Um único cirurgião bucomaxilofacial realizará todos os procedimentos cirúrgicos. A remoção dos molares inferiores foi realizada sob anestesia local com prilocaina 3% e felipressina 0,03 UI (Prilonest; DFL, Brasil), utilizando um total de dois ou três cartuchos contendo 1,8 ml da solução anestésica em cada um destes. Um retalho triangular de espessura total será realizado, seguido por ostectomia periférica usando broca em alta rotação arrefecida com soro fisiológico 0,9%. Ao final, a ferida cirúrgica será rigorosamente fechada através de sutura com fio de seda 4-0
