This is a two-arm, randomized controlled clinical trial with a parallel interventional design, employing a quantitative approach, exploratory character, and open-label blinding. The sample will be collected by convenience, consisting of 100 volunteer patients aged 5 years or older undergoing conditioning for hematopoietic stem cell transplantation (HSCT) at the Hospital de Clínicas Complex of UFPR (CHC-UFPR), in Curitiba/PR. During the study period, patients will be recruited and randomized into two groups (intraoral photobiomodulation therapy - IOPBT and extraoral photobiomodulation therapy - EXOPBT). Randomization will be simple, by electronic lottery, through the website random.org, where selection is done randomly by lottery. In both groups, IOPBT will be performed daily from D-X (before the start of the conditioning regimen) until D+8 or until bone marrow healing. Patients will be evaluated at 5 experimental time points: D-X, D+8, D+12, D+15, and D+30. During these days, the condition of oral health, the severity of oral mucosa (WHO scale) and pain (Visual Analogue Scale) will be evaluated, saliva will be collected for assessment of constituents and inflammatory mediators, and thermographic images will be obtained.

Trata-se de um estudo clínico controlado randomizado de dois braços e desenho de intervenção paralelo com abordagem quantitativa, caráter exploratório e mascaramento do tipo aberto. A amostra será coletada por conveniência, constituída por 100 pacientes voluntários com idade igual ou superior a 5 anos em condicionamento para transplante de células-tronco hematopoiéticas (TCTH) no Complexo do Hospital de Clínicas da UFPR (CHC-UFPR), em Curitiba/PR. Durante o período do estudo, os pacientes serão recrutados e randomizados em dois grupos (terapia de fotobiomodulação intraoral - TFBM IO e terapia de fotobiomodulação extraoral - TFBM EO). A randomização utilizada será simples, por sorteio eletrônico, através do site random.org, em que a seleção é feita de forma aleatória por sorteio. Em ambos a TFBM será realizada diariamente desde o D-X (antes do início do regime de condicionamento) até D+8 ou até a cicatrização da MO. Os pacientes serão avaliados em 5 tempos experimentais: D-X, D+8, D+12, D+15 e D+30. Nestes dias será avaliada a condição da saúde bucal, a severidade da MO (escala OMS) e dor (Escala Visual Analógica), coleta de saliva para avaliação de constituintes e mediadores inflamatórios e a obtenção de imagens termográficas.

Inclusion criteria:

Volunteer patients aged 5 years or older; of both sexes; regardless of weight, height, race, or social condition; who will begin the Hematopoietic Stem Cell Transplantation (HSCT) protocol due to onco-hematological or hematological diseases.

Exclusion criteria:

Patients undergoing autologous transplantation who are under 5 years of age; who use photosensitive medications (tetracycline, sulfonamide, retinoic and glycolic acid); as well as patients who present with elevated core temperature (fever); allergies or any alteration in the skin of the face during treatment.

Pacientes que realizarão transplante autólogo com idade inferior a 5 anos; que fazem uso de medicamentos fotossensíveis (tetraciclina, sulfamida, ácido retinóico e glicólico); assim como pacientes que apresentarem temperatura central elevada (febre); alergias ou qualquer alteração na pele da face durante o tratamento.
