This is a randomized controlled trial with parallel arms of two arms with masking of the outcome assessors. Participants were randomly allocated to two groups through an electronically generated sequence with allocation concealment and the outcome assessors were the masked party as they had no knowledge of each participant group. A total of 100 participants with systemic arterial hypertension on optimized medication treatment were included with 50 in the intervention group and 50 in the control group. The intervention group maintained usual medication treatment and performed a supervised full body resistance training program divided into three sessions, named A, B and C, three times per week for 12 weeks with eight exercises per session two to three sets of eight to twelve repetitions sixty to ninety seconds rest between sets and at least twenty minutes per session at an intensity of six to seven on the Borg rating of perceived exertion scale with progressive load across three phases adaptation from the first to the fourth week development from the fifth to the eighth week and consolidation from the ninth to the twelfth week Session. A included bench press biceps curl squat hip abduction row triceps extension leg curl and lateral raise Session B included machine chest fly hammer curl leg extension hip adduction lat pulldown machine triceps good morning and front raise Session C included incline chest fly incline curl leg press lumbar extension bent over row lying triceps extension stiff leg deadlift and upright row. The control group maintained usual medical treatment and habitual physical activity routine without starting new exercise practices during the study period. Assessments were performed at two time points the initial assessment before the start of the intervention and the reassessment after 12 weeks of intervention

Trata-se de um ensaio clínico randomizado controlado de braços paralelos de dois braços com mascaramento dos avaliadores dos desfechos. Os participantes foram distribuídos aleatoriamente em dois grupos por meio de sequência gerada eletronicamente com ocultação de alocação sendo os avaliadores dos desfechos os responsáveis pelo mascaramento pois não tiveram conhecimento do grupo de cada participante. Foram incluídos 100 participantes com hipertensão arterial sistêmica em uso de tratamento medicamentoso otimizado sendo 50 no grupo intervenção e 50 no grupo controle. O grupo intervenção manteve o tratamento medicamentoso habitual e realizou um programa de treinamento resistido supervisionado de corpo inteiro dividido em três sessões denominadas A, B e C três vezes por semana durante 12 semanas com oito exercícios por sessão, duas a três séries, de oito a doze repetições, sessenta a noventa segundos de descanso entre séries e, no mínimo vinte minutos por sessão com intensidade de seis a sete na escala de percepção de esforço de Borg e carga progressiva em três fases sendo adaptação da primeira à quarta semana desenvolvimento da quinta à oitava semana e consolidação da nona à décima segunda semana. A sessão A incluiu supino rosca direta agachamento abdução de quadril remada tríceps francês flexão de joelhos e elevação lateral. A sessão B incluiu crucifixo na máquina rosca martelo cadeira extensora adução de quadril puxada alta tríceps na máquina good morning e elevação frontal. A sessão C incluiu crucifixo inclinado rosca inclinada leg press extensão lombar remada curvada tríceps testa stiff e remada alta. O grupo controle manteve o tratamento médico habitual e a rotina de atividade física habitual sem iniciar novas práticas de exercício durante o período do estudo. As avaliações foram realizadas em dois momentos a avaliação inicial antes do início da intervenção e a reavaliação após 12 semanas de intervenção.

Primary outcome blood pressure: the mean change in systolic blood pressure was minus 4.4 millimeters of mercury in the intervention group and minus 4.7 in the control group, with no significant difference between groups (p=0.91); diastolic blood pressure changed minus 2.3 in the intervention group and minus 5.7 in the control group. Among the secondary outcomes, in the intervention group between baseline and 12 weeks there were: a reduction in body fat percentage (41.6±9.3 to 39.1±9.2; p=0.009); an increase in muscle mass percentage (25.4±4.3 to 26.5±4.2; p=0.016); an improvement in flexibility (171.9±11.9 to 180.0±14.2; p<0.001); a reduction in central blood pressure (95.3±11.2 to 92.1±11.2 millimeters of mercury; p=0.047); a reduction in HDL cholesterol (50.9±13.0 to 45.2±10.8 mg/dL; p<0.001); an increase in creatinine (0.8±0.2 to 0.9±0.2 mg/dL; p<0.001); a reduction in estimated glomerular filtration rate (96.0±18.6 to 87.0±21.0 mL/min; p<0.001); and an increase in Timed Up and Go time (9.3±1.6 to 9.7±2.1 seconds; p=0.035). The remaining secondary outcomes (arterial stiffness by pulse wave velocity and augmentation index, handgrip strength, sit to stand test, sleep quality, pulmonary function, blood glucose, glycated hemoglobin, total cholesterol, triglycerides, physical activity, mental distress symptoms and food insecurity) showed no statistically significant change. Desfecho primário pressão arterial: a variação média da pressão arterial sistólica foi de menos 4,4 milímetros de mercúrio no grupo intervenção e menos 4,7 no grupo controle, sem diferença significativa entre os grupos (p=0,91); a pressão arterial diastólica variou menos 2,3 no grupo intervenção e menos 5,7 no grupo controle. Entre os desfechos secundários, no grupo intervenção entre o início e as 12 semanas houve: redução do percentual de gordura corporal (41,6±9,3 para 39,1±9,2; p=0,009); aumento do percentual de massa muscular (25,4±4,3 para 26,5±4,2; p=0,016); melhora da flexibilidade (171,9±11,9 para 180,0±14,2; p<0,001); redução da pressão arterial central (95,3±11,2 para 92,1±11,2 milímetros de mercúrio; p=0,047); redução do colesterol HDL (50,9±13,0 para 45,2±10,8 mg/dL; p<0,001); aumento da creatinina (0,8±0,2 para 0,9±0,2 mg/dL; p<0,001); redução da taxa de filtração glomerular estimada (96,0±18,6 para 87,0±21,0 mL/min; p<0,001); e aumento do tempo no teste Timed Up and Go (9,3±1,6 para 9,7±2,1 segundos; p=0,035). Os demais desfechos secundários (rigidez arterial por velocidade de onda de pulso e índice de aumentação, força de preensão manual, teste de sentar e levantar, qualidade do sono, função pulmonar, glicemia, hemoglobina glicada, colesterol total, triglicerídeos, atividade física, sintomas de sofrimento mental e insegurança alimentar) não apresentaram variação estatisticamente significativa. Systemic arterial hypertension is prevalent and physical exercise is investigated as a strategy complementary to medication treatment. This two arm randomized controlled trial assessed the effect of a resistance training program on blood pressure and the proteomic profile in adults with systemic arterial hypertension on medication treatment. Ninety five participants were included, 48 in the intervention group, which performed supervised resistance training three times per week for 12 weeks, and 47 in the control group, which maintained usual treatment. After 12 weeks, blood pressure decreased similarly in both groups, with no significant difference between them. In the intervention group there were improvements in body composition, flexibility and central blood pressure, while a reduction in HDL cholesterol and in estimated glomerular filtration rate and an increase in creatinine were observed. It is concluded that 12 weeks of resistance training was safe and was associated with benefits in body composition, flexibility and central blood pressure, without additional blood pressure reduction compared with the control group; the proteomic analysis will allow assessment of the associated mechanisms and will be incorporated later

A hipertensão arterial sistêmica é prevalente e o exercício físico é investigado como estratégia complementar ao tratamento medicamentoso. Este ensaio clínico randomizado controlado de dois braços avaliou o efeito de um programa de treinamento resistido sobre a pressão arterial e o perfil proteômico em adultos com hipertensão arterial sistêmica em tratamento medicamentoso. Foram incluídos 95 participantes, 48 no grupo intervenção, que realizou treinamento resistido supervisionado três vezes por semana durante 12 semanas, e 47 no grupo controle, que manteve o tratamento habitual. Após 12 semanas, a pressão arterial reduziu de forma semelhante nos dois grupos, sem diferença significativa entre eles. No grupo intervenção houve melhora da composição corporal, da flexibilidade e da pressão arterial central, enquanto foram observadas redução do colesterol HDL e da taxa de filtração glomerular estimada e aumento da creatinina. Conclui-se que o treinamento resistido de 12 semanas foi seguro e associou-se a benefícios na composição corporal, na flexibilidade e na pressão arterial central, sem redução adicional da pressão arterial em relação ao grupo controle; a análise proteômica permitirá avaliar os mecanismos associados e será incorporada posteriormente
