The study will be a randomized, prospective, parallel, double-blind clinical trial (patient and evaluator), previously submitted to the Research Ethics Committee of the School of Dentistry of São Paulo State University “Júlio de Mesquita Filho” (FOA-UNESP), with its initiation planned after approval. The parents or legal guardians of the patients will receive the research information letter, the statement of awareness and consent to participate in the research, and the informed consent form. The patients included in this study will be recruited from the Center for Dental Care for People with Disabilities (CAOE), an Auxiliary Unit of Complex Structure of the School of Dentistry, Araçatuba Campus (FOA), São Paulo State University “Júlio de Mesquita Filho” (UNESP).Patients with intellectual disabilities, aged 6 to 20 years, who present at least one active carious lesion, Class I or Class II (ICDAS – International Caries Detection and Assessment System codes 3 to 6), in permanent teeth, with good periodontal health and no clinical history of allergy to dental products, will be eligible for participation. The exclusion criteria will be patients diagnosed with pulp exposure, fistula, pain, undergoing orthodontic treatment, or with endodontically treated teeth. Two minimally invasive techniques will be used by two calibrated operators. Thirty posterior teeth will be allocated to the control group and treated using the atraumatic restorative treatment (ART) technique with a hybrid glass ionomer cement (EQUIA Forte Fil, GC, Tokyo, Japan), followed by the application of a nanoparticulate surface coating (EQUIA Forte Coat, GC, Tokyo, Japan). Another 30 posterior teeth will be allocated to the test group and restored using an adhesive system (FL Bond II, Shofu, Kyoto, Japan) and a bulk-fill composite resin (Beautiful Bulk Restorative, Shofu, Kyoto, Japan), resulting in a total of 60 restorations. Two calibrated operators with clinical experience, trained by faculty members specialized in restorative dentistry and dental care for patients with special needs, will perform the restorative procedures. Prior to the clinical procedures, they will undergo laboratory training consisting of theoretical and practical instruction on carious tissue removal techniques using the necessary instruments, as well as training in the restorative techniques according to the manufacturers' recommendations for the materials used, in order to standardize the operative procedures. Thirty typodont teeth will be used and proportionally divided between both protocols. The two minimally invasive techniques will be randomly assigned to each tooth using stratified randomization. Tooth type (premolar and molar) and cavity classification (Class I and Class II) will be used as stratification variables during the randomization process to minimize selection bias. The teeth will be considered the experimental units, and those eligible for inclusion will be recorded in an Excel® spreadsheet (Microsoft Office 2020) according to tooth type and cavity classification. To ensure a homogeneous distribution of these two factors among the study groups, the teeth will be ordered as follows: (a) premolar/Class I, (b) premolar/Class II, (c) molar/Class I, and (d) molar/Class II. This stratified randomization method was based on a parallel clinical trial by Menezes-Silva et al. (2021). For the ART technique, the cavity may be open if access has already been achieved by the carious lesion. If access is not available, an Opener instrument (Duflex, Brazil), hatchet, or chisel will be used. The lesion will be enlarged when necessary using a small excavator. Affected dentin will be removed by eliminating only the completely softened, necrotic tissue, particularly beneath the cusps and along the enamel-dentin junction. If resistance is encountered or the patient reports pain, excavation will be discontinued. For the selective caries removal technique, the carious lesion will be completely removed from the cavity walls using a dentin excavator or a low-speed rotary instrument. However, on the pulpal floor, only infected dentin that is not capable of remineralization will be carefully removed using hand instruments. Relative isolation will be performed using sterilized cylindrical cotton rolls (APOLO, Belo Horizonte, MG, Brazil), strategically positioned and maintained: (a) in the maxillary teeth, in the vestibular sulcus region; and (b) in the mandibular teeth, in the floor of the mouth, with the aid of a bilateral plastic retainer (JON, São Paulo, SP, Brazil) to secure the cotton rolls, together with intermittent suction. No prior local or topical anesthesia will be required. The cavity will be conditioned for 10 seconds with 10% polyacrylic acid (Riva Conditioner – SDI) using a cotton pellet. Subsequently, the cavity will be rinsed with a cotton pellet moistened with water, followed by a dry cotton pellet. The capsule seal will then be broken by applying digital pressure to the plunger, and the capsule will be inserted into the amalgamator for 10 seconds. It will then be placed in the applicator gun, and the hybrid glass ionomer cement will be inserted into the cavity. Digital pressure will be applied using a gloved finger previously coated with petroleum jelly for 4 minutes during the setting phase, allowing excess material to flow out and improving material adaptation, while also preventing saliva contamination. After occlusal adjustment and removal of proximal excesses using dental floss and finishing and polishing strips, the nanoparticulate surface coating will be applied and light-cured for 20 seconds (Radii-Cal, SDI, Victoria, Australia). For Class II cavities, a pre-contoured matrix system (TDV, Pomerode, SC, Brazil), adjusted to the height of the marginal ridge, will be used for restoration of the proximal contact, with the aid of anatomical wooden wedges (TDV, Pomerode, SC, Brazil). The patient's caregiver will be instructed that the patient should not chew for 1 hour. For posterior teeth to be restored with the adhesive system and composite resin, the rubber dam will be marked according to the number of teeth to be isolated and the treatment area. The appropriate clamp will be selected for each case. The rubber dam will be placed over the tooth or teeth to be isolated. It will cover the entire tooth and surrounding gingiva to prevent saliva or oral fluids from entering the operative field. The self-etch adhesive system primer (Step 1) will be applied to the dentin and enamel surfaces for 10 seconds, followed by air drying for 10 seconds. Subsequently, a homogeneous layer of adhesive (Step 2) will be applied and light-cured for 5 seconds using the same light-curing unit described above. The bulk-fill composite resin will be inserted in a single increment of up to 4 mm and adapted using a sculpting instrument, with excess material removed. The restoration will then be light-cured for 20 seconds using the same equipment. For Class II cavities, a pre-contoured matrix system (TDV, Pomerode, Brazil), adjusted to the height of the marginal ridge and adapted with the aid of a wooden wedge (TDV, Pomerode, SC, Brazil), will be used to restore the proximal contact.

O estudo será um ensaio clínico randomizado, prospectivo, paralelo e duplo cego (paciente e avaliador), previamente submetido ao Comitê de Ética da Faculdade de Odontologia da Universidade Estadual Paulista “Júlio de Mesquita Filho” (FOA-UNESP), com início previsto após a aprovação. Aos pais ou responsáveis pelos pacientes serão encaminhados a carta de esclarecimento da pesquisa, a ciência e consentimento de participação na pesquisa e o termo de consentimento livre e esclarecido. Os pacientes incluídos neste estudo pertencerão ao Centro de Assistência Odontológica à Pessoa com Deficiência (CAOE), unidade Auxiliar de Estrutura Complexa da Faculdade de Odontologia do Campus de Araçatuba (FOA) da Universidade Estadual Paulista “Júlio de Mesquita Filho” (UNESP). Participarão os pacientes com deficiência intelectual, na faixa etária de 6 a 20 anos, que ao exame clínico apresentarem pelo menos, uma lesão de cárie ativa, classe I ou classe II (ICDAS- International Caries Detection and Assessment System 3 a 6) em dentes permanentes, com boa saúde periodontal e sem história clínica de alergia aos produtos odontológicos. Os critérios para exclusão serão pacientes com diagnóstico de exposição pulpar, fístula, dor, tratamento ortodôntico e dentes tratados endodonticamente. Neste estudo duas técnicas minimamente invasivas serão utilizadas por dois operadores calibrados, sendo 30 dentes posteriores do grupo controle, sendo utilizada a técnica ART e restaurados com cimento híbrido de vidro (EQUIA Forte Fil, GC, Tóquio, Japão), seguido do selante de superfície nanoparticulado (EQUIA Forte Coat, GC, Tóquio, Japão). Outros 30 dentes posteriores, do grupo teste, serão restaurados aplicando-se sistema adesivo (FL Bond II, Shofu, Kyoto, Japan) e resina composta bulk-fill (Beautiful Bulk Restorative, Shofu, Kyoto, Japan), totalizando 60 restaurações. Dois operadores calibrados, com experiência clínica e treinados por um corpo docente especializado em odontologia restauradora e atendimento de pacientes especiais, realizarão os procedimentos restauradores. Para isso, participarão previamente de treinamento laboratorial com a descrição teórica e prática das técnicas de remoção de tecido cariado utilizando-se os instrumentais necessários, assim como, o estudo da técnica restauradora, seguindo as recomendações dos fabricantes dos materiais utilizados, a fim de padronizar os procedimentos operatórios. Serão utilizados 30 dentes artificiais troquelizados, divididos proporcionalmente entre ambos os protocolos. As duas técnicas de mínima intervenção serão atribuídas aleatoriamente a cada um dos dentes de maneira estratificada. O tipo de dente (pré-molar e molar) e de classe (Classe I e II) serão variáveis estratificadas no processo de randomização, minimizando o viés de seleção. Os dentes serão considerados como unidade amostral e, aqueles elegíveis, serão registrados em planilha Excel® (Microsoft Office 2020) de acordo com o tipo e classe. Para haver uma distribuição homogênea desses dois fatores nos grupos de estudo, os dentes serão ordenados da seguinte maneira: a) Pré-molar/Classe I, b) Pré-molar/Classe II, c) Molar/Classe I e d) Molar/Classe II. Este método estratificado de randomização foi baseado em um estudo clínico paralelo (Menezes-Silva et al., 2021). Para técnica do ART, a cavidade pode estar aberta e o acesso já ter ocorrido pela cárie. Caso não haja acesso, será utilizado o instrumento manual Opener (Duflex, Brasil) ou um machado ou cinzel. O alargamento da lesão será realizado caso necessário com uma colher de dentina pequena. A remoção da dentina afetada será realizada removendo-se apenas o tecido totalmente amolecido, necrosado, principalmente sob as cúspides e na linha amelo-dentinária. Havendo resistência do tecido ou acuso de dor pelo paciente, a escavação será interrompida. Na técnica de remoção seletiva da cárie dentária, será realizada a remoção completa da lesão cariosa nas paredes circundantes das cavidades com colher de dentina ou instrumento rotatório em baixa rotação, porém, nas paredes de fundo, somente a dentina infectada, não passível de remineralização, será cuidadosamente removida com o auxílio de instrumentos manuais.Para a realização do isolamento relativo será utilizado roletes confeccionados com fibras de algodão no formato cilíndrico esterilizados (APOLO, Belo Horizonte, MG, Brasil) e estrategicamente acomodados e mantidos: a) nos dentes superiores na região de fundo de vestíbulo e, b) nos dentes inferiores, na região de assoalho bucal com o auxílio de retentor plástico bilateral (JON, São Paulo, SP, Brasil) para fixação dos roletes de algodão, acompanhado de sucção intermitente. Não haverá necessidade prévia de anestesia local ou tópica.Será realizado condicionamento cavitário por 10 segundos com o ácido poliacrílico a 10% (condicionador ácido poliacrílico, Riva – SDI) com bolinha de algodão. Posteriormente, a cavidade será lavada com bolinha de algodão embebida em água e por seguinte uma bolinha de algodão seca. Será então realizado o rompimento do lacre da cápsula com pressão digital no êmbolo, a inserção da cápsula no amalgamador, por 10 segundos, com a adaptação na pistola aplicadora e a inserção do cimento híbrido de vidro na cavidade. A pressão digital, com o dedo enluvado e previamente vaselinado será realizada durante 4 minutos durante sua geleificação, com a finalidade de permitir escoamento de excessos e adaptação do material, além de evitar a contaminação do material com a saliva. Após ajustes oclusais e remoção dos excessos proximais com fio dental e trias de acabamento e polimento, será aplicado o selante de superfície nanoparticulado e com a fotoativado por 20 segundos (Radii-Cal, SDI, Victoria, Australia). Ressalta-se que em cavidades classe II, para a restauração do contato proximal será utilizado um sistema de matriz pré-conformada (TDV, Pomerode, SC, Brasil) ajustada na altura da crista marginal e adaptada com o auxílio de cunhas de madeira anatômicas (TDV, Pomerode, SC, Brasil). O responsável será orientado para que o paciente não mastigue por 1 hora. Para dentes posteriores que serão restaurados com sistema adesivo e resina composta, o dique de borracha será demarcado com base na quantidade de dentes a serem isolados e na área a ser tratada. Será selecionado o grampo ideal para cada caso. O dique de borracha se colocado sobre o dente ou dentes a serem isolados. É importante que o dique cubra toda a área do dente e a gengiva ao redor, para impedir a entrada de saliva ou fluídos bucais na área de trabalho.Será aplicado o primer do sistema adesivo auto-condicionante (passo 1) nas superfícies de dentina e esmalte por 10 segundos, seguido por jato de ar por 10 segundos. Em seguida, será aplicada uma camada homogênea do adesivo (passo 2), fotopolimerizado por 5 segundos com o mesmo aparelho fotopolimerizador citado acima. A inserção da resina composta bulk-fill será realizada em incremento único de até 4 mm, com posterior adaptação do material com o esculpidor, removendo os excessos. Em seguida, será realizada a fotopolimerização com o mesmo equipamento por 20 segundos. Em cavidades classe II, para a restauração do contato proximal será utilizado um sistema de matriz pré-conformada (TDV, Pomerode, Brasil) ajustada na altura da crista marginal e adaptada com o auxílio de cunha de madeira (TDV, Pomerode, SC, Brasil).

We will evaluate the survival of restorations performed with different restorative materials in patients with intellectual disabilities using the Kaplan–Meier survival curve. All tests will be performed at a significance level of 5% (α = 0.05).
