  Participants in this research will be children and parents and / or guardians, who have low income, and attending public daycare centers in Teresina. They have access to fluoridated water from the public system. The epidemiological clinical examination will be carried out in the day care center itself, to select the sample. Two kindergartens from the same neighborhood will be selected by low-income children. Children will be randomly selected to participate in one of two groups. Each participant will be inserted into a list based on numerical sequence. Through Excel will be held the lottery to allocate each participant to his group. Parents / guardians will be asked to perform the dental brushing of the child in the day care facilities. At that moment the researcher will evaluate the procedure and provide guidance on how this can be improved, clarifying all doubts and seeking to assist caregivers in the execution of oral hygiene. Then the children will be divided into two groups (114 per group). In group 1, composed of 114 children will receive application of fluoride varnish (Duraphat, Colgate®). The teeth will be isolated with cotton and dry with gauze, and the varnish applied with the help of microbrush (KG Sorensen. The application will be carried out on a quarterly basis during 3 years of follow up. In group 2, composed of 114 children, fluoride neutral gel (2% sodium fluoride - DFL, Rio de Janeiro) will be applied with toothbrush for one minute. The excesses will be removed with gauze. The application will be performed on a quarterly basis during 3 years of follow-up. Responsible oral hygiene guidelines will occur each year and applications of fluorides (fluoridated neutral gel or lacquer) with fluoride dentifrice will be given quarterly, with toothbrush donation, for three years. After each year of intervention, the questionnaire will be reapplied to parents / guardians to evaluate changes in dietary habits and oral hygiene. The children will be reexamined in the day care center by another previously trained and calibrated examiner (kappa index considered satisfactory of approximately 0.80), who will not know to which group the child belongs.

  Participarão desta pesquisa crianças e pais e/ou responsáveis, que possuam baixa renda, e frequentadoras de creches públicas de Teresina. Elas têm acesso à água fluoretada a partir do sistema público. Será realizado o exame clínico epidemiológico na própria creche, para seleção da amostra. Serão selecionadas duas creches do mesmo bairro frequentadas por crianças de baixa renda. As crianças serão selecionadas aleatoriamente para participar de um dos dois grupos. Cada participante será inserido em uma lista baseada em sequência numérica. Através do Excel será realizado o sorteio para alocar cada participante ao seu grupo. Será solicitado aos pais /responsáveis que realize a escovação dentária da criança, nas dependências da creche. Nesse momento a pesquisadora avaliará o procedimento e fornecerá orientações de como esse pode ser melhorado, esclarecendo todas as dúvidas e procurando auxiliar aos cuidadores para a execução da higiene bucal. Em seguida, as crianças serão dividas em dois grupos (114 por grupo). No grupo 1, composto por 114 crianças receberão aplicação de verniz fluoretado (Duraphat, Colgate®). Os dentes serão isolados com algodão e secos com gaze, e o verniz aplicado com auxilio de microbrush (KG Sorensen. A aplicação será realizada de forma quadrimestral durante 3 anos de acompanhamento. No grupo 2, composto por 114 crianças será aplicado gel fluoretado neutro (Fluoreto de sódio 2% - DFL, Rio de Janeiro) com auxílio de escova de dente durante um minuto. Os excessos serão removidos com gaze. A aplicação será realizada de forma quadrimestral durante 3 anos de acompanhamento. As orientações de higiene bucal aos responsáveis ocorrerão a cada ano e as aplicações de fluoretos (gel fluoretado neutro ou verniz) com doação de dentifrício fluoretado serão quadrimestrais, com doação de escova dental, durante três anos. Após cada ano de intervenção, será reaplicado o questionário aos pais / responsáveis para avaliar alterações nos hábitos de dieta e higiene bucal. As crianças serão reexaminadas na própria creche por outra examinadora previamente treinada e calibrada (índice kappa considerado satisfatório de aproximadamente 0,80), que não saberá a qual grupo pertence a criança.

  Children who received topical application of professional fluoride in the six months prior to the survey will not be included, present more than 10 surfaces with caries lesion in dentin, have teeth with pulp involvement or systemic disease that may be aggravated by dental problem.
