  After written informed consent will be obtained, the women will randomly assigned into three groups: Auricular acupuncture, Sham acupuncutre or Control group. Acupuncture sessions will be performed twice weekly for 2 weeks (total four acupuncture sessions and a last session for the final assessments and needles take off). The control group made twice weekly visits to receive breastfeeding routine care. All groups will receive the same breastfeeding usual care. Auricular Acupuncture Group (true acupuncture group) The Auricular Acupuncture group will receive twice a week sessions of auricular acupuncture with a total of four sessions of intervention. The sessions will have an average duration of 10 minutes. Will be used a treatment protocol with the following auricular points: Shen Men; Sympathetic Autonomic; Kidney; Mammary gland, Endocrine, Prolactin, Tension point. The points were chosen based on scientific literature and clinical practice of the researchers. Ear will be cleaned with cotton and ethyl alcohol 70% and semi-permanent ear needles will be applied of 1mm deep and, fixed with hypoallergenic adhesive tape. The needles will stay in ear three-four days. Participants will be guided to call the researcher if occur excessive discomfort, itching and allergy signs. The needles will be taken by the researcher in the next session. Initially the needles were inserted at subject’s dominant side and, the treatment will be made unilaterally and each session ears will be alternated. Sample size: 40 Sham acupuncture - Placebo Group For this group we will use a simulation of the needles insertion by the touch of the tip of a toothpick, in the same points used for the real acupuncture group, followed by bonding with a hypoallergenic adhesive tape. Participants will be instructed to remain with the adhesive tapes for three-four days, being taken off by the researcher at the next session. The application will be made unilaterally and each session ears will be alternated. Sample size: 40 Usual care The control group will receive usual care, which consists of individualized care and guidance in breastfeeding. It will be held for the same period and frequency of the other interventions.Sample size:20. Likewise, all groups will receive the usual care provided by the Human Milk Bank. The materials needed for the research will be: semi-permanent ear needles of 1 mm, plate for adhesive tape, duct tape non-allergenic, probe, toothpick, scissors, tweezers, alcohol and 70%, cotton for antisepsis.

  O Grupo de intervenção Acupuntura Auricular receberá duas sessões semanais de acupuntura auricular por um período de três semanas. As sessões terão uma duração média de 10 minutos. Serão utilizados como protocolo de tratamento os seguintes pontos auriculares: Shenmen; Simpático; Rim; Glândulas mamárias, Endócrino, Prolactina, Ansiolítico. Os pontos foram escolhidos baseados na literatura científica, estudos prévios e experiência clínica dos pesquisadores. Para aplicação da acupuntura auricular será feita a higienização com algodão e álcool etílico 70% do pavilhão auricular e aplicação de agulhas auriculares semipermanentes de 1mm de profundidade, afixadas com fita adesiva hipoalergênica. O tempo de permanência das agulhas será de três ou quatro dias. As participantes serão orientadas a entrar em contato com a pesquisadora se houver desconforto excessivo, prurido e sinais de alergia. As agulhas serão retiradas pelo pesquisador na sessão seguinte. A aplicação será feita unilateralmente e a cada sessão serão revezadas as orelhas. Número de participantes: 40 Grupo Placebo – acupuntura sham Para o este grupo utilizaremos uma simulação da inserção das agulhas com o toque da ponta de um palito de dente, de forma suave, em pontos descritos para o grupo acima, seguido da colagem com uma fita adesiva hipoalergênica, contendo um ponto circular de coloração cinza escuro no centro, representando a agulha semipermanente. As participantes serão orientadas a permanecer com as fitas adesivas por três ou quatro dias, sendo retiradas pelo pesquisador na sessão seguinte. A aplicação será feita unilateralmente e a cada sessão serão revezadas as orelhas. Número de participantes: 40. Grupo Controle – Cuidado Usual O grupo controle receberá o cuidado usual, o qual consiste em atendimento individualizado para avaliação e orientação das mamadas. Será realizado por três semanas, semanalmente. Número de participantes: 20. Importante destacar que todos os grupos receberão o cuidado usual realizado pelo Banco de Leite Humano em retornos semanais. Entretanto o grupo intervenção e o grupo placebo retornaram outra vez na semana, apenas para a sessão de acupuntura. Os materiais necessários para a realização da pesquisa serão: agulhas auriculares semipermanentes de 1 mm, placa para fita adesiva, fita adesiva não alergênica, apalpador, palito de dente, tesoura, pinça, álcool 70% e algodão.

  Lactating mothers with low milk production will be enrolled in study, that is whose breastmilk volumes failed to meet their infants' requirements; 18 and older; between 5 and 30 days of postpartum.

  Participarão da pesquisa puérperas lactantes, que estejam amamentando, exclusivamente ou não, maiores de 18 anos, entre o quinto e 30º dia pós-parto e que apresentarem baixa produção láctea, ou seja, às quais o volume de leite produzido não é suficiente para atender as demandas da criança.

  To make use of galactogogues drugs during the research period; Being a smoker; history of breast surgery (breast reduction surgery or breast implant); never had made the auricular acupuncture before; pregnancy and delivery with any complications; baby who have an weight smaller than 2.500g and larger than 4.000g; twin pregnancy; preterm birth; be allergic to any materials used in auricular acupuncture; make use of other energy therapies (herbal treatments, Reiki, Flower therapy, homeopathy, etc); the patient has no time available to do the treatment.

  Presentation of expected outcome: The primary endpoint will be the milk production, which will be assessed through the milk volume and serum prolactin level. Presentation of the method used to verify the expected outcome:The volume of milk produced will be evaluated after 10 minutes of breast pumping, with an electric pump (Lactina Select, Medela) in the opposite breast offered in the last breastfeeding. Blood samples for serum prolactin level will be collected after thirty minutes of rest. Presentation of the parameters used to verify if the outcome actually occurred:There will be an increase in milk volume and serum prolactin levels.

  Apresentação do desfecho esperado: O desfecho primário será a produção láctea, a qual será avaliada através do volume de leite produzido e da dosagem de prolactina sérica. Apresentação do método usado para verificar o desfecho esperado: O volume de leite produzido será avaliado após 10 minutos de ordenha em bomba elétrica (Lactina Select, Medela) unilateralmente, uma hora após a última mamada, na mama oposta à oferecida na última mamada. A coleta de sangue para a prolactina sérica será realizada após a mulher permanecer trinta minutos de repouso em local silencioso e agradável. Apresentação dos parâmetros utilizados para verificar se o desfecho realmente ocorreu:Haverá aumento no volume de leite ordenhado e nos níveis séricos de prolactina.
