This is a randomized controlled clinical study with two groups, double-blind, with 22 newborns in total (11 in each group) with indication for enteral feeding via transpyloric tube. At the time of tube insertion, newborns will be randomly divided, using a computer-generated random sequence, between 2 techniques for transpyloric tube insertion; using or not gastric insufflation.

Trata-se de um estudo clínico randomizado controlado com dois grupos, duplo-cego, com 22 recém-nascidos no total (11 em cada grupo) com indicação de alimentação por via enteral em sonda transpilórica. No momento da passagem da sonda, os recém-nascidos serão aleatoriamente divididos, utilizando uma sequência aleatória gerado por computador, entre 2 técnicas para inserção da sonda transpilórica; utilizando ou não insuflação gástrica.

All eligible participants in the study were hospitalized newborns with an indication for a transpyloric feeding tube. The gestational age at birth of participants in both groups ranged from 26 to 37 weeks. Birth weight ranged from 660 to 3292 grams. 4 participants were male and 18 were female. Approximately 50% of each group obtained correct probe position on confirmatory x-ray.

22 newborns participated in the study, 11 with each group (with or without air insufflation in the gastric portion during insertion of the transpyloric tube). Every newborn hospitalized with an indication for transpyloric tube feeding was analyzed in order to fulfill inclusion criteria to enter the study, consecutively. A total of 38 newborns were indicated for a transpyloric tube, 22 of which met criteria for inclusion randomly into two groups.

Introduction: Feeding catheters are commonly used in hospitalized newborns to meet their nutritional needs, which is a vital component of treatment. The transpyloric catheter for enteral nutrition is an essential resource, especially for newborns with suspected or confirmed cases of gastroesophageal reflux and bronchoaspiration. The literature is still scarce about an insertion method that is effective not only in measuring the distance between the oral or nasal cavity and the third/fourth portion of the duodenum, but also in auxiliary methods for its implantation, such as air injection or the use of prokinetic drugs. Proper insertion is essential to reduce the risks related to misplaced catheters in this clientele. Objective: to compare the proportion of catheters correctly located in the transpyloric portion of the digestive tract of newborns, visualized in radiographic examination, considering two insertion methods, with or without air insufflation. Methods: Blind randomized clinical trial, to be carried out in a neonatal hospitalization unit of a tertiary public teaching hospital. The sample size will be 22 subjects, 11 in each group: subjects will be randomized between these groups. In one group, air insufflation will occur when the catheter is in the gastric position, before finalizing its introduction up to the third or fourth duodenal portion. In the other group, no such insufflation will occur. The technique for gastric measurement will be the formula based on weight, while the measurement to reach the duodenal portion will be the glabella-calcaneal measurement. The nurse collaborating in the study will pass the catheter and its final position will be confirmed by a simple X-ray of the abdomen by the attending physician. To compare the proportion between the correct catheter placement groups, the chi-square or Fisher's exact test will be applied. To analyze the relationships between the method used and the characteristics of newborns, Student's t test or Mann-Whitney test will be applied, depending on the distribution of variables. The study will follow the recommendations of Resolution 466/2012 of the National Health Council.

Introdução: Cateteres para alimentação são comumente usados em recém-nascidos hospitalizados para atender às suas necessidades nutricionais, o que é um componente vital do tratamento. O cateter transpilórico para nutrição enteral é um recurso essencial, sobretudo para recém-nascidos com suspeita de ou caso confirmado de refluxo gastroesofágico e broncoaspiração. A literatura ainda é escassa sobre um método de inserção que seja efetivo não apenas em mensurar a distância entre a cavidade oral ou nasal até a terceira/quarta porção do duodeno, como também nos métodos auxiliares para sua implantação como a injeção de ar ou uso de medicamentos pró-cinéticos. A inserção adequada é imprescindível para diminuir os riscos relacionados ao mal posicionamento de cateteres nessa clientela. Objetivo: comparar a proporção de cateteres localizados corretamente na porção transpilórica do tubo digestivo de recém-nascidos, visualizados em exame radiográfico, considerando dois métodos de inserção, com ou sem insuflação de ar. Métodos: Ensaio clínico randomizado cego, a ser realizado em uma unidade de internação neonatal de um hospital universitário público, de nível terciário. O tamanho amostral será de 22 sujeitos, sendo 11 em cada grupo: os sujeitos serão randomizados entre esses grupos. Em um grupo ocorrerá a insuflação de ar quando o cateter estiver na posição gástrica, antes de finalizar sua introdução até a terceira ou quarta porção duodenal. No outro grupo não ocorrerá tal insuflação. A técnica para a medida gástrica será a da fórmula baseada no peso, enquanto que a medida para se atingir a porção duodenal será a medida glabela-calcâneo. O enfermeiro colaborador do estudo realizará a passagem do cateter e seu posicionamento final será confirmado por radiografia simples de abdômen pelo médico assistente. Para comparar a proporção entre os grupos de posicionamento correto do cateter, será aplicado o teste de qui-quadrado ou exato de Fisher. Para analisar as relações existentes entre o método utilizado e as características dos recém-nascidos, será aplicado o teste t de Student ou o teste de Mann-Whitney, conforme a distribuição das variáveis. O estudo seguirá as recomendações da Resolução 466/2012 do Conselho Nacional de Saúde.

The individual data of the unidentified participants, such as: method of passage of the transpyloric tube, x-ray to confirm its positioning and anthropometric data, will be shared, in the form of an excel program database, with the statistician responsible for the statistical analysis of the data. These data will be made available to the statistician by the researcher in charge, through an electronic file, only for the time necessary for the statistical analysis procedures.
