  This is a three-arms, double-blind, randomized controlled clinical trial. Before restorative procedures, where necessary, teeth will be anesthetized with a 3% mepivacaine solution (Mepisv, Nova DFL, Rio de Janeiro, RJ, Brazil) and cleaned with 3M Clinpro Glycine Prophy Powder, followed by rinsing and drying. Then, shade selection will be made using a shade guide. After rubber dam is placed, the restoration procedures will be conducted according to the following groups: Control group (conventional technique) / n = 120 restorations - Conventional technique using Mylar matrix strips + Scothbond Universal Plus (ER)* + Filtek Universal (Capsules); Experimental group 1 (BT + warmed composite) / n = 120 restorations - Alternative technique using BT Bioclear Matrix System + Scothbond Universal Plus (ER)* + Filtek Supreme flowable (Warmed) + Filtek Universal (Capsules / Warmed); and Experimental group 2 (BT + non-warmed composite) / n = 120 restorations - Alternative technique using BT Bioclear Matrix System + Scothbond Universal Plus (ER)* + Filtek Supreme flowable + Filtek Universal (Capsules / Non-warmed). In the control group, after the mylar matrix strips and wedges are correctly inserted, a 37% phosphoric acid will be applied for 15 seconds, then rinsed and air-dried. One coat of the Universal adhesive system Scothbond Universal Plus (3M Oral Care) will be gently scrubbed on the entire enamel and dentin surface for approximately 20 seconds, according to the manufacturer’s recommendations. Then, the adhesive will be evaporated by gentle air stream for 5 s and light cured for 10 s at 1200 mW/cm2 (Elipar, 3M Oral Care). After adhesive application, the resin composite Filtek Universal (3M Oral care) in capsules will be used, usually with one or two increments. The resin composite will be inserted directly in the cavity with an application device, each increment being light cured for 20 s (each face) at 1200 mW/cm2. The restorations will be finished immediately with fine and extra-fine #2200 diamond burs (KG Sorensen), 3M Sof-Lex Extra-Thin Contouring and Polishing Discs and polished with 3M Sof-Lex Spiral Finishing and Polishing Wheels. In the experimental group 1, after the BT Bioclear Matrix size are correctly selected with the system’s probe, and are correctly inserted, a 37% phosphoric acid will be applied for 15 seconds, then rinsed and air-dried. One coat of the Universal adhesive system Scothbond Universal Plus (3M Oral Care) will be gently scrubbed on the entire enamel and dentin surface for approximately 20 seconds, according to the manufacturer’s recommendations. Then, the adhesive will be evaporated by gentle air stream for 5 s and will not be light cured. After adhesive application, the resin composite Filtek Supreme flowable (3M Oral care) pre-warmed will be used, with one increment, and then, the resin composite Filtek Universal in capsules, pre-warmed according to the manufacturer’s instructions, will be inserted also in only one increment, using the application device. After that, the whole “restoration block” will be light cured for 20 s (each face) at 1200 mW/cm2. The restorations will be finished immediately with fine and extra-fine #2200 diamond burs (KG Sorensen), 3M Sof-Lex Extra-Thin Contouring and Polishing Discs and polished with 3M Sof-Lex Spiral Finishing and Polishing Wheels. The restorations will be finished immediately with fine and extra-fine #2200 diamond burs (KG Sorensen), 3M Sof-Lex Extra-Thin Contouring and Polishing Discs and polished with 3M Sof-Lex Spiral Finishing and Polishing Wheels.

  Trata-se de um estudo clínico randomizado controlado de três braços, duplo-cego. Antes dos procedimentos restauradores, quando necessário, os dentes serão anestesiados com solução de mepivacaína a 3% (Mepisv, Nova DFL, Rio de Janeiro, RJ, Brasil) e limpos com Clinpro Prophy Powder, seguido de enxágue e secagem. Em seguida, a seleção de cores será feita usando um guia de cores. Após a colocação do dique de borracha, os procedimentos de restauração serão conduzidos de acordo com os seguintes grupos: Grupo controle (técnica convencional) / n = 120 restaurações - Técnica convencional usando tiras de matriz de poliéster + Scothbond Universal Plus + Filtek Universal (Cápsulas); Grupo experimental 1 (BT + resina composta aquecida) / n = 120 restaurações - Técnica alternativa utilizando BT Bioclear Matrix System + Scothbond Universal Plus + Filtek Supreme flowable (Aquecida) + Filtek Universal (Cápsulas / Aquecida); e Grupo experimental 2 (BT + resina composta não aquecida) / n = 120 restaurações - Técnica alternativa utilizando BT Bioclear Matrix System + Scothbond Universal Plus + Filtek Supreme flowable + Filtek Universal (Cápsulas / Não aquecida). No grupo controle, após a inserção correta das tiras e cunhas da matriz de poliéster, será aplicado ácido fosfórico a 37% por 30 segundos, depois enxaguado e seco ao ar. Uma aplicação do sistema adesivo Universal Scothbond Universal Plus (3M) será esfregada suavemente em toda a superfície de esmalte e dentina por aproximadamente 20 segundos, de acordo com as recomendações do fabricante. Em seguida, o adesivo será evaporado por jato de ar suave por 5 s e fotopolimerizado por 10 s a 1200 mW/cm2 (Elipar, 3M). Após a aplicação do adesivo, será utilizada a resina composta Filtek Universal (3M) em cápsulas, geralmente com um ou dois incrementos. A resina composta será inserida diretamente na cavidade com um dispositivo de aplicação, cada incremento sendo fotopolimerizado por 20 s (cada face) a 1200 mW/cm2. As restaurações serão finalizadas imediatamente com pontas diamantadas #2200 finas e extrafinas (KG Sorensen), Discos de acabamento e Polimento Extrafinos 3M Sof-Lex e polidas com Rodas de Polimento e Acabamento Espiral 3M Sof-Lex. No grupo experimental 1, após o tamanho do BT Bioclear Matrix ser selecionado corretamente com a sonda do sistema e inserido corretamente, o esmalte será condicionado com ácido fosfórico a 37% por 15 segundos, depois enxaguado e seco ao ar. Uma camada do sistema adesivo Universal Scothbond Universal Plus (3M) será esfregada suavemente em toda a superfície de esmalte e dentina por aproximadamente 20 segundos, de acordo com as recomendações do fabricante. Em seguida, o adesivo será evaporado por jato de ar suave por 5 s e não será fotopolimerizado. Após a aplicação do adesivo, será utilizada a resina composta Filtek Supreme flowable (ESPE 3M), pré-aquecida, com um incremento, e em seguida, a resina composta Filtek Universal em cápsulas, pré-aquecida conforme instruções do fabricante, será inserida também em apenas um incremento, usando o dispositivo de aplicação. Depois disso, todo o “bloco de restauração” será fotopolimerizado por 20 s (cada face) a 1200 mW/cm2. As restaurações serão finalizadas imediatamente com pontas diamantadas #2200 finas e extrafinas (KG Sorensen), Discos de acabamento e Polimento Extrafinos 3M Sof-Lex e polidas com Rodas de Polimento e Acabamento Espiral 3M Sof-Lex. E no grupo experimental 2, após o tamanho do BT Bioclear Matrix ser selecionado corretamente com a sonda do sistema e inserido corretamente, o esmalte será condicionado com ácido fosfórico a 37% por 15 segundos, depois enxaguado e seco ao ar.   Participants from both gender; must be in good general health; be at least 18 years old; have an acceptable oral hygiene level; present at least 20 teeth under occlusion; have at least three black spaces to be restored in anterior teeth (these black spaces have to be non-retentive, with an average area higher than 1 mm2, and can involve only the enamel or both enamel and dentin, in vital teeth without mobility)

  The restorations will be evaluated by Modified World Federation criteria (FDI) at baseline (one week) and after 6,12, 24 and 36 months of clinical service. Only the clinically relevant measures for evaluation of adhesive and composite performance will be used and scored. The primary clinical endpoint will be restoration retention/fracture

  As restaurações serão avaliadas pelos critérios da Federação Mundial de Odontologia (FDI) e USPHS na linha de base (uma semana) e após 6,12, 24 e 36 meses de serviço clínico. Somente as medidas clinicamente relevantes para avaliação do desempenho do adesivo e do compósito serão usadas e pontuadas. O desfecho clínico primário será a retenção/fratura da restauração

• Secondary outcomes:

  The following secondary endpoints will also be evaluated: Post-operative sensitivity, marginal staining, marginal adaptation, gingival inflammation, fractures, surface texture, recurrence of caries and periodontal health. The evaluation of the spontaneous postoperative sensitivity will be performed one week after the restorative procedure by asking the patient if he experienced any pain during the period. During the restoration procedures, clinical time necessary to perform the restorations will also be evaluated. And finally, the patient view will also be considered in all the evaluation periods. These variables will be ranked according to modified FDI criteria into clinically very good, clinically good, clinically sufficient/ satisfactory, clinically unsatisfactory but repairable, and clinically poor (replacement required). In the case of marginal staining and marginal adaptation, the semiquantitative criteria (SQUACE) will be used

  Os seguintes desfechos secundários também serão avaliados: sensibilidade pós-operatória, coloração marginal, adaptação marginal, inflamação gengival, fraturas, textura da superfície, recorrência de cárie e saúde periodontal. A avaliação da sensibilidade pós-operatória espontânea será realizada uma semana após o procedimento restaurador, perguntando ao paciente se sentiu alguma dor durante o período. Durante os procedimentos de restauração, também será avaliado o tempo clínico necessário para realizar as restaurações. E por fim, a visão (opinião) do paciente sobre as restaurações também será considerada em todos os períodos de avaliação. As variáveis serão classificadas de acordo com os critérios da FDI modificados em clinicamente muito bom, clinicamente bom, clinicamente satisfatório, clinicamente insatisfatória mas reparável, e clinicamente pobre (troca necessária)
