  The 90 patients included in this research will be clinically evaluated for the diagnosis of oral mucositis (Pinheiro et al., 2019) and, during the clinical examination, the medical history will be recorded. Patients will receive oral hygiene instructions and will be divided evenly randomized (www.random.org.br) in 3 groups and 2 subgroups: Subgroup Without ILIB (control): In this treatment group, patients will be evaluated initially according to the degree of mucositis present in the oral cavity, so that, later, treatment can be started. As for the therapy protocols, in the prevention and treatment groups, photodynamic therapy (PDT) using the photosensitizer curcumin (1.5 g / L concentration) will be used diluted in 980 mL of distilled water (adapted from Leite et al., 2014; Andrade et al., 2013) (Formula & Action, São Paulo, SP, Brazil). The patient will be instructed to rinse 20 mL of the solution for 5 min, discarding the contents soon after. Then, the oral cavity will be irradiated with blue LED (UltraLumen SL, Sander do Brasil Ltda., Minas Gerais, MG, Brazil) with 1200 mW of power and a wavelength of 468 nm, for 5 min (Pinheiro et al., 2019). After PDT, photobiomodulation therapy (PBM-T) will be performed, using the low intensity laser (DMC, São Carlos, São Paulo, Brazil) with 100 mW of power, 660 nm of wavelength, in continuous mode, spot of 0.028 cm², 1 J of energy and flow of 35.7 J / cm². The energy used of 1 J was based on the study of Antunes et al. (2017) and Guedes et al. (2018), in addition to the information contained in the studies by Fekrazad and Chiniforush (2014) and Zecha et al. (2016a). It will be irradiated 26 points for 10 s at each point: one point on each lip commissure (2 points), three points on the mucosa of upper lip, three points on the mucosa of lower lip, three points on each side of the mucosa cheek (6 points), three points on each side of the tongue (6 points), two points on the tongue floor, two points on the tongue belly and two spots on the soft palate (Brandão et al., 2018; Guedes et al., 2018). This protocol will be carried out weekly (Simões et al., 2009; Pinheiro et al., 2019) and will consist of 5 sessions (Mobadder et al., 2018). In the prevention group, PBM-T + PDT will start in the first treatment session with chemotherapy and / or radiotherapy (Zecha et al., 2016b; Martins et al., 2019), before the sessions (Brandão et al., 2018; Martins et al., 2019); Subgroup with ILIB: In addition to the protocols previously established with the realization of PBM-T + PDT (subgroup without ILIB), the technique intravascular blood irradiation with laser (ILIB) no invasive will be performed with a bracelet on the radial artery, which is located anatomically medial to the radius bone, close to the wrist. Its path at this point is superficial and parallel to the radio, so it will be easily palpable between the styloid process and the tendon of the radial flexor carpi muscle (Pacheco and Schapochnik, 2018). The ILIB will be applied using the laser of low intensity Therapy EC (DMC, São Carlos, São Paulo, Brazil) with power of 100 mW, wavelength of 660 nm (Pacheco and Schapochnik, 2018), spot of 0.098 cm² and 180 J of total irradiation energy (Wirz-Ridolfi, 2013), during 30 min of treatment (Wirz-Ridolfi, 2013; Pacheco and Schapochnik, 2018). Therapy will be performed weekly and the protocol will consist of 5 sessions (Pacheco and Bezinelli, 2018); Group ILIB: The ILIB technique will be performed solely and exclusively, using the same parameters previously established (subgroup with ILIB).

  Os 90 pacientes incluídos na pesquisa serão avaliados clinicamente para o diagnóstico da mucosite oral (Pinheiro et al., 2019) e, durante o exame clínico, o histórico médico será registrado. Os pacientes receberão instruções de higiene oral e serão divididos de forma randomizada (www.random.org.br) em 3 grupos e 2 subgrupos: Subgrupo S/ ILIB (controle): No grupo tratamento, os pacientes serão avaliados inicialmente de acordo com o grau de mucosite presente na cavidade oral, para que, posteriormente, seja iniciado o tratamento. Quanto aos protocolos das terapias, nos grupos prevenção e tratamento será utilizada a terapia fotodinâmica (PDT) utilizando o fotossensibilizante curcumina (concentração de 1,5 g/L diluída em 980 mL de água destilada) (adaptado de Leite et al., 2014; Andrade et al., 2013) (Fórmula & Ação, São Paulo, SP, Brasil). O paciente será orientado a bochechar 20 mL da solução por 5 min, desprezando o conteúdo logo após. Em seguida, a cavidade oral será irradiada com LED azul (UltraLumen SL, Sander do Brasil Ltda., Minas Gerais, MG, Brasil) com 1200 mW de potência e comprimento de onda de 468 nm, por 5 min (Pinheiro et al., 2019). Após a PDT, será realizada a terapia de fotobiomodulação (PBM-T), utilizando o laser de baixa intensidade (DMC, São Carlos, São Paulo, Brasil) com potência de 100 mW, comprimento de onda de 660 nm, em modo contínuo, spot de 0,028 cm², 1 J de energia e fluência de 35,7 J/cm². A energia utilizada de 1 J foi baseada nos trabalhos de Antunes et al. (2017) e Guedes et al. (2018), além das informações contidas nos estudos de Fekrazad e Chiniforush (2014) e Zecha et al. (2016a). Serão irradiados 26 pontos por 10 s em cada ponto: um ponto em cada comissura labial (2 pontos), três pontos na mucosa labial superior, três pontos em mucosa labial inferior, três pontos em cada lado da mucosa jugal (6 pontos), três pontos em cada bordo lateral da língua (6 pontos), dois pontos em assoalho de língua, dois pontos no ventre da língua e dois pontos em palato mole (Brandão et al., 2018; Guedes et al., 2018). Este protocolo será realizado semanalmente (Simões et al., 2009; Pinheiro et al., 2019) e consistirá de 5 sessões (Mobadder et al., 2018). No grupo da prevenção, a PBM-T + PDT terão início na primeira sessão dos tratamentos com quimioterapia e/ou radioterapia (Zecha et al., 2016b; Martins et al., 2019), antes das sessões (Brandão et al., 2018; Martins et al., 2019); Subgrupo C/ ILIB: Em adição aos protocolos estabelecidos anteriormente com a realização de PBM-T + PDT (subgrupo S/ ILIB), a técnica não invasiva da irradiação intravascular do sangue com laser (ILIB) será executada com uma pulseira na artéria radial, a qual se localiza anatomicamente medial ao osso rádio, próximo ao punho. Seu trajeto neste ponto é superficial e paralelo ao rádio, por isso será facilmente palpável entre o processo estiloide e o tendão do músculo flexor radial do carpo (Pacheco e Schapochnik, 2018). A ILIB será aplicada utilizando o laser de baixa intensidade Therapy EC (DMC, São Carlos, São Paulo, Brasil) com potência de 100 mW, comprimento de onda de 660 nm (Pacheco e Schapochnik, 2018), spot de 0,098 cm² e 180 J de energia total de irradiação (Wirz-Ridolfi, 2013), durante 30 min de tratamento (Wirz-Ridolfi, 2013; Pacheco e Schapochnik, 2018). A terapia será realizada semanalmente e o protocolo consistirá de 5 sessões (Pacheco e Bezinelli, 2018); Grupo ILIB: Será realizada, única e exclusivamente, a técnica da ILIB, executada utilizando os mesmos parâmetros estabelecidos anteriormente (subgrupo C/ ILIB).

  Group without oral mucositis the protocol for the prevention of oral mucositis will be used: patients who do not have lesions of oral mucositis; patients diagnosed with head and/or neck cancer who are going to start chemotherapy and/or radiation therapy in the oncology at the Hospital of the Pontifical Catholic University of Campinas; patients or guardians who agree to participate in the research by signing the Informed Consent Form. Group with oral mucositis, lesions already present and treatments will be carried out: patients who present oral mucositis with scores ranging from 1 to 4 according to the WHO classification (Oton-Leite et al., 2015; Mobadder et al., 2018); patients diagnosed with head and/or neck cancer undergoing chemotherapy and/or radiation therapy at the Hospital's oncology sector the Pontifical Catholic University of Campinas; patients using other prophylactic measures for mucositis, such as oral glutamine, steroids, immunoglobulins, vitamins and chlorhexidine, among others; patients or guardians who agree to participate in the research by signing the Informed Consent Form

  Grupo sem mucosite oral será utilizado o protocolo para a prevenção da mucosite oral: pacientes que não apresentarem lesões de mucosite oral; pacientes diagnosticados com câncer de cabeça e/ou pescoço que vão iniciar o tratamento de quimioterapia e/ou radioterapia no setor de oncologia do Hospital da Pontifícia Universidade Católica de Campinas; pacientes ou responsáveis que concordarem em participar da pesquisa assinando o Termo de Consentimento Livre e Esclarecido. Grupo com mucosite oral, as lesões já estarão presentes e serão feitos os tratamentos: pacientes que apresentarem mucosite oral com os escores variando entre 1 a 4 segundo a classificação da OMS (Oton-Leite et al., 2015; Mobadder et al., 2018); pacientes diagnosticados com câncer de cabeça e/ou pescoço em tratamento de quimioterapia e/ou radioterapia no setor de oncologia do Hospital da Pontifícia Universidade Católica de Campinas; pacientes que façam uso de outras medidas profiláticas para mucosite, como glutamina por via oral, esteroides, imunoglobulinas, vitaminas e clorexidina, entre outros; pacientes ou responsáveis que concordarem em participar da pesquisa assinando o Termo de Consentimento Livre e Esclarecido

  It is expected that ILIB associated or not with photobiomodulation therapy and photodynamic therapy will be an alternative in the prevention and treatment of oral mucositis in cancer patients. In prevention, the percentage and time of onset of oral mucositis lesions after therapies (weekly for 5 weeks) will be measured. In the treatment, before and after its performance, the degree of oral mucositis of the patients will be evaluated according to the WHO.

  No desfecho primário, espera-se que a ILIB associada ou não à terapia de fotobiomodulação e terapia fotodinâmica seja uma alternativa na prevenção e no tratamento da mucosite oral em pacientes oncológicos. Na prevenção, a porcentagem e o tempo de aparecimento das lesões de mucosite oral após as terapias (semanais por 5 semanas) serão mensurados. No tratamento, antes e após a sua realização, o grau de mucosite oral dos pacientes será avaliado de acordo com a OMS.

  It is expected to reduce the pro-inflammatory cytokine IL-6 and the count of viable bacteria present in saliva of the patients included in the research. In the first and last session of therapies, the measurement of the pro-inflammatory cytokine IL-6 will be performed using the quantitative immunoenzymatic assay kit (ELISA) and the bacterial count, in turn, will be subjected to microbiological evaluation. The follow-up of patients will be carried out for a period of six months, during which they will be submitted to clinical reevaluation to verify the appearance / reappearance of oral mucositis and / or any adverse effects of the therapies applied in this study. During the follow-up period, information about the patients survival and loss of patients or loss of continuity will also be noted (PINHEIRO et al., 2019). If the lesion appears / reappears, the therapy protocols will be applied again and, therefore, patients will receive additional sessions (MARTINS et al., 2019).

  Espera-se, inclusive, a redução da citocina pró-inflamatória IL-6 e da contagem de bactérias viáveis presentes na saliva dos pacientes incluídos na pesquisa. Na primeira e última sessão das terapias, a mensuração da citocina pró-inflamatória IL-6 será realizada utilizando o kit quantitativo de ensaio imunoenzimático (ELISA) e a contagem de bactérias, por sua vez, será submetida à avaliação microbiológica. O acompanhamento dos pacientes será realizado por um período de seis meses, no qual serão submetidos à reavaliação clínica para verificar o aparecimento/reaparecimento da mucosite oral e/ou quaisquer efeitos adversos das terapias aplicadas neste estudo. No período de acompanhamento, também serão anotadas informações acerca da sobrevida e perda dos pacientes ou perda da sua continuidade (PINHEIRO et al., 2019). Caso haja o aparecimento/reaparecimento da lesão, os protocolos das terapias serão novamente aplicados e, portanto, os pacientes receberão sessões adicionais (MARTINS et al., 2019).
