  Intervention Group (40 undergraduate nurses from the EPE / UNIFESP and 40 undergraduate nursing students from UFPR): use of the Self-Instructional Guide to Clinical Reasoning (GARC) called Instructions on How to Analyze a Case: Think Like a Nurse during the resolution of case studies writing. The GARC contains 11 sentences distributed in four sections that guide students to identify and analyze the relevant evidence in one case, process the evidence, and identify two or three possible nursing diagnoses / patient problems. Students are asked to identify more than one possible nursing diagnosis (SD) / patient problem to promote thought flexibility and facilitate the reasoning process by confirming and differential patient's ED / problems. Students are then guided to consider any possible safety imminent risks that the patient may experience and are asked to select a nursing diagnosis / priority patient problem for which it would be most important that care be given. Finally, students are encouraged to reflect on the evidence they have identified to determine the patients' SD / problems and to verify the accuracy of their reasoning. Control (35 undergraduate nurses from the EPE / UNIFESP and 30 undergraduate nursing students from UFPR): The control will consist of the resolution of written clinical cases without the help of the GARC. Data collection procedures Nursing students will be recruited to participate in the study during class time: - At the beginning of the semester following the end of the disciplines of Clinical and Surgical Nursing at the Federal University of São Paulo; - At the beginning of the semester following completion of the Nursing courses at the Federal University of Paraná. Pre-test - The principal investigator will arrive during the class period, explain to the students that the study was designed to evaluate the effectiveness of teaching strategies that will benefit students and will answer any questions; - The students will be given the terms of free and informed consent; - The terms of consent will be collected and stored in envelopes sealed by the researcher; - Students will be given the pre-test package, which consists of: Case study 1 with their respective resolution instructions and answer sheet, demographic data sheet, and an individual code generation form for identity protection. - The pre-test package will be explained to the students; - Students will complete the individual code generation forms and demographic data sheet and wait to turn the page to respond to the case study; - Students will have approximately 20 minutes to respond to the case study; The researcher will collect the packages and put them in a sealed envelope. "The researcher will thank the students and remind them that they will return in a week or two to apply a second case study to analyze. Post-test - The lead researcher will arrive during the class period, remind students briefly about the study, and thank them for participating in the second part of the study to test teaching strategies that will benefit nursing students. - The following post-test package will be delivered to randomized students to the Control Group: Case study 2 with their respective resolution instructions and answer sheet (Annex 3), individual code generation form for identity protection, and the questionnaire about research perceptions. For the students randomized to the Intervention Group, the entire post-test package plus the GARC will be delivered. - The post-test package will be explained to the students; - Students will fill out the individual code generation forms and wait to turn the page to answer the case study; - Students will have approximately 20 minutes to respond to the case study; - After 20 minutes, students will be asked to turn the page and complete the questionnaire; The researcher will collect the packages and put them in a sealed envelope. - The researcher will inform the students that the GARC was the strategy evaluated and that half of the room received it to enable the evaluation of the effect in the clinical trial.

  Grupo Intervenção (40 bacharelandos em enfermagem da EPE/UNIFESP e 40 bacharelandos em enfermagem da UFPR): uso do Guia Autoinstrucional para Raciocínio Clínico (GARC) denominado Instructions on How to Analyze a Case: Think Like a Nurse durante a resolução de estudos de caso clínicos por escrito. O GARC contém 11 sentenças distribuídas em quatro seções que norteiam os estudantes para identificar e analisar as evidências pertinentes em um caso, processar as evidências e identificar dois ou três possíveis diagnósticos de enfermagem/problemas dos pacientes. Solicita-se que os estudantes identifiquem mais de um possível diagnóstico de enfermagem (DE)/problema do paciente para promover flexibilidade de pensamento e facilitar o processo de raciocínio, confirmando e DEs /problemas dos pacientes diferenciais. Os estudantes são, então, norteados a considerar quaisquer possíveis riscos iminentes à segurança que o paciente possa experienciar e solicita-se que selecionem um diagnóstico de enfermagem/problema do paciente prioritário para o qual seria mais importante que o cuidado fosse voltado. Finalmente, os estudantes são norteados a refletir sobre as evidências que identificaram para determinar os DEs/problemas dos pacientes e verificar a acurácia do seu raciocínio. Controle (35 bacharelandos em enfermagem da EPE/UNIFESP e 30 bacharelandos em enfermagem da UFPR): O controle consistirá na resolução dos casos clínicos escritos sem auxílio dos GARC. Procedimentos de coleta de dados Os estudantes de enfermagem serão recrutados para participar do estudo durante o horário de aula: - No início do semestre subsequente ao término das disciplinas de Enfermagem Clínica e Cirúrgica na Universidade Federal de São Paulo; - No início do semestre subsequente ao término das disciplinas de Enfermagem na Universidade Federal do Paraná. Pré-teste - A pesquisadora principal chegará durante o período da aula, explicará aos estudantes que o estudo foi desenhado para avaliar a efetividade de estratégias de ensino que beneficiarão os estudantes e responderá quaisquer questões; - Aos estudantes, serão entregues os termos de consentimento livre e esclarecido; - Os termos de consentimento serão coletados e guardados em envelopes selados pelo pesquisador; - Será entregue aos estudantes o pacote pré-teste, que consiste em: Estudo de caso 1 com suas respectivas instruções de resolução e folha de respostas, ficha de dados demográficos, e um impresso de geração de código individual para proteção de identidade. - O pacote pré-teste será explicado aos estudantes; - Os estudantes preencherão os impressos de geração de código individual e ficha de dados demográficos e esperarão para virar a página para responder ao estudo de caso; - Os estudantes terão aproximadamente 20 minutos para responder ao estudo de caso; - A pesquisadora coletará os pacotes e os colocará em um envelope selado. - A pesquisadora agradecerá os estudantes e os lembrará de que retornará em uma ou duas semanas para aplicar um segundo estudo de caso para analisarem. Pós-teste - A pesquisadora principal chegará durante o período da aula, lembrará os estudantes brevemente sobre o estudo e os agradecerá pela participação na segunda parte do estudo para testar as estratégias de ensino que beneficiarão estudantes de enfermagem. - Será entregue aos estudantes randomizados para o Grupo Controle o seguinte pacote pós-teste: Estudo de caso 2 com suas respectivas instruções de resolução e folha de respostas (Anexo 3), impresso de geração de código individual para proteção de identidade, e o questionário sobre percepções quanto à pesquisa. Para os alunos randomizados para o Grupo Intervenção, será entregue todo o pacote pós-teste acrescido do GARC. - O pacote pós-teste será explicado aos estudantes; - Os estudantes preencherão os impressos de geração de código individual e esperarão para virar a página para responder ao estudo de caso; - Os estudantes terão aproximadamente 20 minutos para responder ao estudo de caso; - Após 20 minutos, será solicitado que os estudantes virem a página e preencham o questionário; - A pesquisadora coletará os pacotes e os colocará em um envelope selado. - A pesquisadora informará aos estudantes que o GARC era a estratégia avaliada e que metade da sala o recebeu para possibilitar a avaliação do efeito no julgamento clínico.
