A randomized clinical trial will be carried out. The included 34 children will be randomly randomized by the Research Randomizer Quick Tutorial software into two groups: Respiron Group (GR) and Water Group (GA). The Respiron Group will perform expiratory muscle training with Respiron Kids and the Água Group will perform expiratory muscle training with Positive Expiratory Pressure (PEP) in Selo de Água. Both groups will carry out training for three months until they complete a total of 10 specific training sessions.

Será realizado um ensaio clínico randomizado. As 34 crianças incluídas serão randomizadas aleatoriamente pelo software Research Randomizer Quick Tutorial em dois grupos: Grupo Respiron (GR) e Grupo Água (GA). O Grupo Respiron realizará um treino muscular expiratório com Respiron Kids e o Grupo Água realizará um treino muscular expiratório com Pressão Positiva Expiratória (PEP) em Selo de Água. Ambos os grupos realizarão os treinamentos durante três meses até que realizem um total de 10 sessões de treinamento específico.

Inclusion criteria:

Healthy subjects, aged between 3 and 12 years old, who do not have comorbidities, will be included in the study.

The training protocol was carried out over 6 weeks, from three sets of ten repetitions per week, in which one group used the Respiron® apparatus, inverted, at level I resistance and the other group used the PEP in Selo of Water at 10cmH2O. For the Respiron Group, the subject was asked to perform three sets of ten repetitions of maximum exhalation using the device's mouthpiece, which was positioned inverted and at level I of resistance, with the aim of moving the three spheres of the device, especially the colored one. red. If the red sphere did not rise, the repetition was cancelled. For the Water Group, the subject was asked to perform three sets of ten repetitions of maximum exhalation through a straw positioned at 10cmH2O inside a 500ml plastic bottle. If the straw went to another water pressure level, the repetition was canceled. One week after completing the protocol, the post-protocol evaluation of the study variables was carried out. In addition, the variables were evaluated again in a three-month follow-up.

O protocolo de treinamento foi realizado em 6 semanas, a partir de três séries de dez repetições por semana, em que um grupo utilizou o aparato Respiron®, de forma invertida, em nível I de resistência e, o outro grupo utilizou o PEP em Selo de Água em 10cmH2O. Para o Grupo Respiron foi solicitado que o sujeito realizasse três séries de dez repetições de expiração máxima no bocal do aparato, sendo esse posicionado de forma invertida e em nível I de resistência, com objetivo de movimentar as três esferas do aparato, principalmente a de cor vermelha. Caso a esfera vermelha não erguesse, a repetição era anulada. Para o Grupo Água foi solicitado que o sujeito realizasse três séries de dez repetições de expiração máxima em um canudo posicionado a 10cmH2O dentro de uma garrafa plástica de 500ml. Caso o canudo fosse para outro nível de pressão de água, a repetição era anulada. Uma semana após a finalização do protocolo, foi realizado a avaliação pós-protocolo das variáveis de estudo. Além disso, foi realizado novamente a avaliação das variáveis em um follow-up de três meses.

Some of the participants in this study, especially in the water group, observed the characteristics of drooling during sleep and keeping their mouth open during activities of daily living – ADLs, such as when watching television, which may be signs of mouth breathing. Oral breathing is considered an adaptive characteristic, the persistence of which can lead to various changes such as dental, craniofacial and postural changes.

Foram observadas entre alguns dos participantes deste estudo, principalmente do grupo água, as características de babar durante o sono e permanecer com a boca aberta em atividades de vida diárias – AVDs, como ao assistir televisão, podendo essas ser sinais da respiração oral. Introduction: There is currently a high prevalence of respiratory pathologies in preschool age. Respiratory muscle weakness is one of the main functional repercussions of the deterioration of lung function in children with respiratory disorders, as it can lead to intolerance to physical effort, worsening the subject's quality of life. An effective strategy to reverse this situation is through respiratory muscle training. Objective: The objective was to investigate the effect of expiratory muscle training on the respiratory capacity of healthy children. Methodology: A longitudinal and prospective randomized clinical trial was carried out at a State School in the interior of southern Brazil. Healthy subjects of both sexes, with an average age of six years, who did not have any declared respiratory comorbidity, were included. Subjects with any type of comorbidity were excluded, including a clinical history of asthma, bronchitis, pervasive developmental disorder, and neurogenic conditions. The subjects were evaluated pre- and post-intervention, in addition to being re-evaluated at a 3-month follow-up. Training followed, in both groups, the performance of 3 sets of 10 repetitions per week, for six weeks. The Water Group used PEP in Water Seal, while the Respiron Group used Respiron®. Results: 34 children were included and the total sample showed significant results in relation to respiratory capacity and respiratory muscle strength, both in the short term, medium and long term, in which all subjects showed improvement with statistical differences in these variables ( p= <0.001). This result shows that both devices are effective, but the statistical results showed no difference between them, making it impossible to say which device is more effective. Conclusion: The proposed physiotherapeutic protocol promoted improvements in children's respiratory strength and capacity, both in the short and long term.

Introdução: Atualmente há uma elevada prevalência de patologias respiratórias em idade pré-escolar. A fraqueza muscular respiratória é uma das principais repercussões funcionais da deterioração da função pulmonar em crianças com distúrbios respiratórios, pois podem levar a uma intolerância ao esforço físico, piorando a qualidade de vida deste sujeito. Uma estratégia eficaz para reverter esse quadro é através do treinamento muscular respiratório. Objetivo: Teve-se como objetivo investigar o efeito do treinamento muscular expiratório na capacidade respiratória de crianças hígidas. Metodologia: Realizou-se um ensaio clinico randomizado de período longitudinal e prospectivo, realizado em uma Escola Estadual do interior do Sul do Brasil. Foram incluídos sujeitos hígidos, de ambos os sexos, com idade média de seis anos, que não possuam qualquer comorbidade respiratória declarada. Foram excluídos sujeitos com qualquer tipo de comorbidade, incluindo histórico clínico de asma, bronquite, transtorno do desenvolvimento global, e afecções neurogênicas. Os sujeitos foram avaliados pré e pós intervenção, além de reavaliados em um follow-up de 3 meses. O treinamento seguiu, em ambos os grupos, a realização de 3 séries de 10 repetições por semana, durante seis semanas. O Grupo Água utilizou o PEP em Selo de Água, enquanto o Grupo Respiron utilizou Respiron®. Resultados: Foram incluídas 34 crianças em que a amostra total evidencia resultados expressivos em relação à capacidade respiratória e à força muscular respiratória, tanto em curto prazo, quanto a médio e longo prazo, em que todos os sujeitos apresentaram melhora com diferença estatística nessas variáveis (p= <0,001). Esse resultado mostra que ambos dispositivos são eficazes, porém os resultados estatísticos não mostraram diferença entre eles, não sendo possível afirmar qual dos dispositivos é mais eficaz. Conclusão: O protocolo fisioterapêutico proposto promoveu melhora na força e capacidade respiratórias junto as crianças, tanto a curto quanto a longo prazo.
