This is a single-group, open-label clinical trial. The intervention in the present study took place in 247 participants (before and after), through the use of therapeutic footwear with modification of the participant's footwear in a footwear with an intended therapeutic effect, aiming at relieving pressure, through the use of the personalized insole of according to the needs of each individual at risk of developing ulceration. Once the biomechanical alterations were detected due to the presence of calluses or deformities in the feet or amputation of fingers, alterations in the gait pattern and joint mobility of ankle and foot movement and areas of plantar hyperpressure, a mold of the foot and regions of hyperpressure was performed. through a plantigraph. Subsequently, a custom made insole made of flexible E.V.A plate was made with their respective pressure and gait aligning elements to be placed inside the footwear that the volunteer wore during the intervention period. This footwear must be made of soft material and without internal seams, height and width proportional to the size of the foot and its possible deformities, and a semi-flexible sole. At the time of the evaluation, the participant was asked to bring their shoes so that they could be adapted using the insole by the researcher according to the need observed by the Baropodometry exam and evaluation of the shoes during a period of 3 months. After this period, the patients were reassessed.

Trata-se de um estudo clínico de grupo único, aberto. A intervenção no presente estudo, se deu em 247 participantes (antes e depois), através do uso do calçado terapêutico com modificação do calçado do participante em um calçado com um efeito terapêutico pretendido, visando alívio de pressão, através do uso da palmilha personalizada de acordo com a necessidade de cada indivíduo com risco de desenvolver ulceração. Uma vez detectada as alterações biomecânicas pela presença de calosidades ou deformidades nos pés ou amputação de dedos, alterações no padrão da marcha e mobilidade articular de movimento de tornozelo e pé e áreas de hiperpressão plantar, foi realizado o molde do pé e das regiões de hiperpressão através de um plantígrafo. Posteriormente foi confeccionado uma palmilha sob medida feita em placa de E.V.A flexível com seus respectivos elementos alinhadores de pressão e marcha para ser colocada no interior do calçado que o voluntário usou durante o período da intervenção. Esse calçado deve ser de material macio e sem costuras internas, altura e largura proporcionais ao tamanho do pé e suas possíveis deformidades e solado semi flexível. No momento da avaliação, foi solicitado ao participante que trouxesse seu calçado para que fosse adaptado pelo uso da palmilha pelo pesquisador conforme necessidade observada pelo exame de Baropodometria e avaliação do calçado durante um período de 3 meses. Após esse período os pacientes foram reavaliados.

Among the individuals evaluated, most were female, with a mean age of 58.9 ± 14.5 years. About half of the participants had not completed elementary school (43.7%) and were retired (2.0%), with glycemic values above the references, glycated hemoglobin (HbA1c) with great variation (5.1-11.0%) and overweight. Regarding chronic complications, peripheral arterial disease was present in 49.5% of the individuals, gastrointestinal disorders in 32%, retinopathy in 17.5%, nephropathy in 2.5%, sexual dysfunction in 29% and amputation in 3.5% of the patients. patients. A statistically significant association was identified between changes in plantar pressure and absence of protective plantar sensitivity (p = 0.029), absent aquilus reflection (p = 0.016), reduced vibratory sensitivity (p = 0.048) and absence (p = 0.009), presence of calluses in the right foot (p = 0.006) and left foot (p = 0.001) and hallux valgus in the right foot (p = 0.002) and left foot (p = 0.002). There was a difference in the prevalence of alterations in exposed and unexposed limbs by the chi-square test at 5% of significance for: absent plantar protective sensitivity, absent aquilus reflex, reduced and absent vibratory sensitivity, callosity on the right and left foot and hallux valgus on the right foot and left foot.
Protective plantar sensitivity was 2.2 times more prevalent than having plantar alterations in both limbs of those with decreased plantar protective sensitivity. Para aqueles indivíduos que apresentaram sensibilidade vibratória diminuída, a prevalência é 61% menor de ter alteração plantar em ambos os membros em relação aos que apresentaram esse tipo de sensibilidade intacta, e para a sensibilidade vibratória ausente, é 2,5 vezes mais prevalente de ter alteração plantar alteração. de ambos os membros aquele que possui ou possui de forma diminuída.

Introduction: Diabetic peripheral neuropathy is a highly prevalent chronic condition that substantially affects people with Diabetes Mellitus. It is characterized by a diffuse, symmetrical, distal and progressive lesion of the sensory-motor and autonomic fibers, caused by chronic hyperglycemia, which causes a gradual loss of the protective sensation of the feet, increasing the risk of plantar changes, trauma, osteoarticular deformities, biomechanical and balance alterations. Appropriate interventions to prevent complications such as ulcers and amputations have been proposed, among them the use of personalized insole. Objectives: To identify dermatological, bone, sensory and biomechanical changes, and to evaluate the effect of personalized insole on plantar pressure, balance and foot health in Diabetic peripheral neuropathy people due to Type 2 Diabetes Mellitus. Method: Research carried out after ethical approval, cross-sectional and quasi-experimental in people diagnosed with type 2 Diabetes Mellitus and diabetic peripheral neuropathy enrolled in the public primary care network of a city in the east of São Paulo. For the cross-sectional study, signs of biomechanical, dermatological, sensory, bone and balance alterations were evaluated using the Berg Balance Scale instrument. Plantar pressure was identified by baropodometry with the BaroScan® platform. The quasi-experimental study, of the before and after type, was conducted with 45 people who presented changes in plantar pressure identified among the individuals treated at Basic Health Units. The intervention variable corresponded to the use of individualized and personalized insoles, made from the results obtained with baropodometry. The outcome variables were verified three months after the implementation of the intervention, namely: assessment of foot health, balance and plantar pressure. The data obtained were analyzed according to descriptive and inferential statistics, through measures of summary and central tendency, confidence interval, chi-square test, Student's t test, Fisher's exact test and Mann Whitney, adopting as a level of significance 5%. Results: In the cross-sectional study, 200 people were identified; changes in plantar pressure, absence of plantar protective sensation (p=0.029), of the Achilles reflex (p=0.016), decreased vibratory sensitivity (p=0.048) and absent (p=0.009), presence of calluses on the right foot ( p=0.006) and left (p=0.001), hallux valgus in the right foot (p=0.002) and left foot (p=0.002). In baropodometry, the dynamic assessment the mean on the right foot was 6.08 (±2) kgf/cm2 and 6.7 (±1.62) kgf/cm2 on the left foot, the center of static pressure on the right foot was lower ( 10.55±3.82) than in the left foot (11.97±3.90), indicating plantar hyperpressure. The risk of falling was high, ranging from 8 to 56 points, with a mean of 40.96(±10.77) on the Berg Balance Scale. In the quasi-experimental study, after using the customized insole for a period of three months, it was possible to observe a statistically significant reduction in dynamic plantar pressure in the left and right foot (p<0.001) and in static plantar pressure in the right foot (p=0.008 ) of the individuals investigated. There was a significant increase in the average scores of the Foot Status Health Questionnare, in all domains, indicating a significant improvement in the general health of the feet (p<0.001). Conclusion: Dermatological, bone and sensory changes in the feet are related to changes in plantar pressure that can lead to loss of balance and complications in people with diabetic peripheral neuropathy. The use of an insole made from the analysis of areas of plantar hyperpressure reduced plantar pressure and improved the general health of the feet of a group of people with diabetic peripheral neuropathy.

Introdução: A neuropatia periférica diabética é uma condição crônica altamente prevalente que afeta substancialmente as pessoas com Diabetes Mellitus. É caracterizada por lesão difusa, simétrica, distal e progressiva das fibras sensitivo-motoras e autonômicas, causadas pela hiperglicemia crônica, que ocasiona perda gradual da sensação protetora dos pés, aumentando o risco das alterações plantares traumas, deformidades osteoarticulares, alterações biomecânicas e equilíbrio. Intervenções adequadas para prevenção de complicações, como úlceras e amputações tem sido proposta, dentre elas, o uso de palmilha personalizada. Objetivos: Identificar alterações dermatológicas, ósseas, sensórias e biomecânicas, e avaliar o efeito de palmilha personalizada sobre a pressão plantar, equilíbrio e saúde dos pés de pessoas com neuropatia periférica diabética decorrente do Diabetes Mellitus Tipo 2. Método: Pesquisa realizada após aprovação ética, de base transversal e quase experimental em pessoas com diagnóstico de Diabetes Mellitus tipo 2 e neuropatia periférica diabética cadastradas na rede pública de atenção básica de um município do leste paulista. Para o estudo transversal, sinais de alterações biomecânicas, dermatológicas, sensórias, ósseas e de equilíbrio foram avaliados através da aplicação do instrumento Escala de Equilíbrio de Berg. A pressão plantar foi identificada pela baropodometria com a plataforma BaroScan®. O estudo quase experimental, do tipo antes e depois, foi conduzido com 45 pessoas que apresentaram alteração da pressão plantar identificadas dentre os indivíduos atendidos em Unidades Básicas de Saúde. A variável de intervenção correspondeu ao uso de palmilhas individualizadas e personalizadas, confeccionadas a partir dos resultados obtidos com a baropodometria. As variáveis de desfecho foram verificadas três meses após a implementação da intervenção, sendo elas: avaliação da saúde dos pés, equilíbrio e pressão plantar. Os dados obtidos foram analisados segundo estatística descritiva e inferencial, por meio de medidas de resumo e tendência central, intervalo de confiança, teste do Qui-quadrado, T de Student, Teste exato de Fisher e Mann Whitney, adotando-se como nível de significância 5%. Resultados: No estudo transversal, identificou-se 200 pessoas; as alterações na pressão plantar, ausência de sensação protetora plantar (p=0,029), do reflexo de Aquileu (p=0,016), sensibilidade vibratória diminuída (p=0,048) e ausente (p=0,009), presença de calosidades no pé direito (p=0,006) e esquerdo (p=0,001), hálux valgo no pé direito (p=0,002) e no pé esquerdo (p=0,002). Na baropodometria, a avaliação dinâmica a média no pé direito foi 6,08 (±2) kgf/cm2 e 6,7 (±1,62) kgf/cm2 no pé esquerdo, o centro de pressão estática no pé direito foi menor (10,55±3,82) que no pé esquerdo (11,97±3,90), indicando uma hiperpressão plantar. Houve aumento significante nos escores médios do Foot Status Health Questionnare, em todos os domínios, indicando melhora significativa na saúde geral dos pés (p<0,001). Conclusão: Alterações dermatológicas, ósseas e sensoriais nos pés estão relacionadas a alterações da pressão plantar que podem levar a perda do equilíbrio e complicações em pessoas com neuropatia periférica diabética. O uso de palmilha confeccionada a partir da análise de áreas de hiperpressão plantar reduziu a pressão plantar e melhorou a saúde geral dos pés de um grupo de pessoas com neuropatia periférica diabética.
