This is a randomized, controlled, two-arm, unblinded clinical study. The control group consists of nine hemodialysis patients who will undergo an intradialytic physiotherapy program based on multifunctional exercises for eight weeks, twice a week during hemodialysis, totaling sixteen sessions. These exercises consist of resisted knee flexion and extension exercises, hip flexion with ankle weights, and hip abduction with elastic bands for the lower limbs and, in the absence of a fistula, shoulder flexion and elbow flexion with dumbbells for the upper limbs. If the patient has a fistula, the exercise should only be performed on the free limb. Passive stretching will be performed at the end. The load will be increased by increasing the number of repetitions, with two sets of fifteen repetitions being performed on each limb in the first four weeks, and three sets of fifteen repetitions on each limb in the last four weeks, within the patient's capabilities. Experimental group: nine patients on hemodialysis who will undergo an intradialytic physiotherapy program associated with inspiratory muscle training. This group will undergo the same intradialytic physiotherapy protocol based on multifunctional exercises that was performed by the control group, but they will also undergo inspiratory muscle training during hemodialysis. The intradialytic physiotherapy protocol includes resisted exercises of knee flexion and extension and hip flexion with ankle weights and hip abduction with elastic bands for the lower limbs and, in the absence of a fistula, shoulder flexion and elbow flexion with dumbbells for the upper limbs. If the patient has a fistula, the exercise should only be performed on the free limb. At the end, passive stretching will be performed. The increase in load will occur based on the increase in the number of repetitions, with two sets of fifteen repetitions being performed with each limb in the first four weeks and three sets of fifteen repetitions with each limb in the last four weeks, within the patient's capabilities. Inspiratory muscle training will be performed with linear pressure resistance equipment and, like the multifunctional exercise program, will be performed for eight weeks, twice a week during hemodialysis, totaling sixteen sessions. The training load will be forty percent of the maximum inspiratory pressure and the progression of the load will occur based on the increase in the number of repetitions within the possibilities, with three sets of twenty repetitions being performed in the first four weeks and four sets of twenty repetitions being performed in the last four weeks

O grupo controle: nove pacientes em hemodiálise que realizarão um programa de fisioterapia intradialítica baseadas em exercícios multifuncionais durante oito semanas, duas vezes por semana durante a hemodiálise, totalizando dezesseis sessões. Grupo experimental: nove pacientes em hemodiálise que realizará o programa de fisioterapia intradialítica associado ao treinamento muscular inspiratório. Esse grupo realizará o mesmo protocolo de fisioterapia intradialítica baseadas em exercícios multifuncionais que foi realizado pelo grupo controle, porém o mesmo também irá realizar o treinamento muscular inspiratório durante a hemodiálise. O protocolo de fisioterapia intradialítica inclui exercícios resistidos de flexo extensão de joelho e flexão de quadril com caneleira e abdução de quadril com faixa elásticas para membros inferiores e, na ausência de fístula, flexão de ombro e flexão de cotovelo com halteres para membros superiores. Se o paciente possuir fístula deverá realizar o exercício somente no membro livre. Ao final, serão realizados alongamentos passivos. O incremento de carga se dará a partir do aumento do número de repetições, sendo que nas primeiras quatro semanas serão realizadas duas séries de quinze repetições com cada membro e nas últimas quatro semanas três séries de quinze repetições com cada membro, dentro das possibilidades do paciente. O treinamento muscular inspiratório será realizado com o equipamento de resistência linear pressórica e, assim como o programa de exercícios multifuncionais, será realizado durante oito semanas, duas vezes por semana durante a hemodiálise, totalizando dezesseis sessões. A carga de treinamento será de quarenta porcento da pressão inspiratória máxima e a progressão de carga se dará a partir do incremento no número de repetições dentro das possibilidades, sendo que nas primeiras quatro semanas serão realizadas três séries de vinte repetições e nas últimas quatro semanas serão realizadas quatro séries de vinte repetições

In the control group, maximum inspiratory pressure increased from 69.5 (95% confidence interval: 41–98) to 81 (61–99), with p=0.09. In the intervention group, it increased from 70 (24–115) to 78.7 (34–120), with p=0.06. For maximum expiratory pressure, the control group presented values of 99.5 (57–152) pre-intervention and 104.5 (55–158) post-intervention, with p=0.67. The intervention group presented values of 94.4 (51–138) in the pre-intervention and 104.4 (73–139) in the post-intervention, with p=0.24. Secondary outcomes included mobility and functional balance, assessed by the timed up and go test, and functional capacity, assessed by the distance covered in the six-minute walk test. Introduction: Although inspiratory muscle training is widely accepted as an effective approach for various pathologies, there is little evidence on its effects when used in patients with chronic kidney disease undergoing hemodialysis, especially in conjunction with physiotherapy. Objective: To evaluate the effects of inspiratory muscle training associated with a physiotherapy protocol in patients undergoing hemodialysis. Methods: This is a randomized, two-arm, unblinded study conducted in a general hospital in the Northwest of the State of Rio Grande do Sul. Patients were divided into: a control group, which followed an intradialytic physiotherapy protocol with functional exercises, and an inspiratory muscle training group, which followed the same protocol but with the addition of inspiratory muscle training. The program lasted 8 weeks, with an initial load of 40% of PImáx, progressively adjusted according to each participant's capacity. Results: The sample included 13 patients with a mean age of 52.1 ± 12.4 years. The inspiratory muscle training group showed a reduction in distance in the 6-minute walk test (345.3-310.8 m/p=0.61), while the control group increased it (360-381.9 m/p=0.17). In the Timed Up and Go test, both groups reduced the time, with statistical significance in the control group (8.5-6.86 s/p≤0.4). In terms of maximum inspiratory pressure and maximum expiratory pressure, both groups showed an increase, with greater evidence in the inspiratory muscle training group. Conclusion: Inspiratory muscle training did not provide additional benefits to functional capacity, balance, and mobility compared to training alone, but it did show positive effects on respiratory muscle strength

Introdução: Ainda que o treinamento muscular inspiratório seja amplamente aceito como uma abordagem eficaz para diversas patologias, existem poucas evidências sobre seus efeitos quando utilizado em pacientes com doença renal crônica submetidos à hemodiálise, especialmente em conjunto com a fisioterapia. Objetivo: Avaliar os efeitos do treinamento muscular inspiratório associado a um protocolo de fisioterapia em pacientes submetidos à hemodiálise. Métodos: Trata-se de um estudo randomizado de dois braços, sem cegamento, conduzido em um hospital geral no Noroeste do Estado do Rio Grande do Sul. Os pacientes foram divididos em: grupo Controle, que seguiu um protocolo de fisioterapia intradialítica com exercícios funcionais, e grupo treinamento muscular inspiratório, que seguiu o mesmo protocolo, mas com o acréscimo do treinamento muscular inspiratório. O programa durou 8 semanas, com uma carga inicial de 40% da PImáx, ajustada progressivamente conforme a capacidade de cada participante. Resultados: A amostra incluiu 13 pacientes com média de 52,1±12,4 anos. O grupo treinamento muscular inspiratório apresentou uma redução na distância no teste de caminhada de 6 minutos (345,3-310,8m/p=0,61), enquanto o grupo controle aumentou (360-381,9m/p=0,17). No Timed Up and Go, ambos reduziram o tempo, com significância estatística no grupo controle (8,5-6,86s/p≤0,4). Já na pressão inspiratória máxima e a pressão expiratória máxima, ambos os grupos tiveram aumento, com maior evidência no grupo treinamento muscular inspiratório. Conclusão: O treinamento muscular inspiratório não trouxe benefícios adicionais à capacidade funcional, equilíbrio e mobilidade em relação ao treinamento isolado, mas mostrou efeitos positivos na força muscular respiratória
