Randomized clinical trial using WhatsApp messages for appropriate dietary recommendations in pregnant women with gestational diabetes compared to standard nutritional guidance in an outpatient setting. Randomization was done using numbered envelopes described as control or intervention, in a sealed box with the number of participants calculated by the sample size, thus ensuring the groups were identical and homogeneous throughout the study. The intervention will be the sending of WhatsApp messages with daily nutritional information. Both the control and intervention groups will receive similar nutritional guidance during their in-person prenatal appointments. The study will include 176 pregnant women. The frequency of messages will be daily (7 times a week, until the end of the patient's pregnancy), with messages ceasing when the patient notifies of her delivery. Each patient will complete one 24-hour dietary recall per week until the end of the pregnancy (which will be, on average, 6 more weeks). In total, approximately 240 24-hour dietary recalls will be needed for each group

Ensaio clinico randomizado com a utilização de mensagens de Whatsapp para recomendações dietéticas adequadas em gestantes com diabetes gestacional comparado a conduta ambulatorial de orientação nutricional padrão. A aleatorização foi realizada por intermedio de envelopes numerados e descritos como controle ou intervenção, em uma caixa fechada com o numero de participantes calculado pelo tamanho amostral, dessa forma os grupos ao longo do estudo foram idênticos e homogêneos. A intervenção será o envio de mensagens de Whatsapp com informações nutricionais diárias, o grupo controle e o grupo intervenção receberão de forma similar as orientacões nutricionais durante as suas consultas pré-natais presenciais. O número de participantes do estudo será de 176 gestantes. A frequência de mensagens será diária (7 vezes por semana, ate o final da gestação da paciente), as mensagens serão suspensas no momento que a paciente notificar o seu parto. Cada paciente realizará 1 recordatório alimentar de 24h por semana ate o final da gestação (que será em media mais 6 semanas). Ao total serão necessários cerca 240 recordatórios de 24h para cada grupo

Expected outcome 1: We expect to find a difference in glycemic control outcomes when using social media as an intervention in pregnant women with diabetes, compared to pregnant women with the same condition but without the use of social media messaging. By employing the method of sending daily nutritional recommendation messages via WhatsApp to pregnant women with diabetes, randomized against a control group that received guidance only during in-person consultations, we will observe through capillary blood glucose measurements whether the intervention is responsible for differences in glycemic indices

The patients' ages ranged from 18 to 46 years, with a mean of 34.3 years (standard deviation 5.6). The majority identified themselves as mixed race (39.5%) and were married (55.6%). The number of pregnancies ranged from 1 to 8, with an average of 3.1 pregnancies (standard deviation 1.5). Patients in the intervention group were slightly younger (33.4 versus 35.5) and thinner (body mass index 28.8 versus 31.1) than those in the control group, but there were no statistically significant differences in sociodemographic variables between the two groups.

Eighty-six women agreed to participate in the study, but 5 were excluded for not meeting the minimum follow-up criteria. All exclusions were done before randomization. Thirty-four were randomized to the intervention group and 47 to the control group. A total of 188 24-hour dietary recalls (R24) were conducted in the control group and 290 in the intervention group. All the pregnant women were followed until the last consultation before delivery.

Patients may have felt uncomfortable describing the dietary recall. Patients in the intervention group may have been uncomfortable with the messages. Patients in the control group may have received less information than those in the intervention group. There was no impact on the gestational outcome, as participation in the study did not influence medical decision-making.

The intervention group showed better glycemic control in the third (p=0.027), fourth (p<0.001), fifth (p=0.04), sixth (p=0.011) and seventh (p=0.009) weeks of follow-up. In the first (p=0.857), second (p=0.262) and eighth (p=0.566) weeks there was no difference. Of the total number of women evaluated, 39.5% required insulin therapy, 44.1% in the control group and 36.2% in the intervention group, with no differences between the groups (p value=0.498), despite a slightly lower percentage of women in the intervention group having required drug treatment. No woman was treated with oral antidiabetic medication. At the end of the follow-up, there were no differences in caloric intake (p=0.178), nor in carbohydrate consumption (p=0.20). There was greater consumption of proteins (p=0.021), lipids (p<0.001) and fiber (p=0.02) in the intervention group at the end of the follow-up.

The management of gestational diabetes mellitus focuses on dietary interventions, physical activity, and pharmacological therapy. Strategies that enhance adherence to the diet could play a pivotal role in managing this population. The use of digital tools has been gaining im-portance as an accessible tool to help control the disease. The objective of the study was to compare the effectiveness of outpatient nutritional guidance complemented by digital media with outpatient nutritional guidance in pregnant women with gestational diabetes mellitus. Randomized, patient-blind clinical trial. The pregnant women were randomly allocated into two groups: the Control Group (CG) received exclusively outpatient nutritional treatment and the Intervention Group (IG) received outpatient nutritional guidance and reminders via WhatsApp. Food intake (calories, carbohydrates, lipids, proteins and fiber) was assessed using 24-hour dietary recalls (24-hour R). Glycemic control and the need for pharmacological treatment were also assessed. 81 women were included, 34 allocated to the CG and 47 to the GI. Patients were followed for a period of 4 to 8 weeks. Few differences were observed in glycemic control between the two groups throughout the follow-up period. There were no statistical differences in pharmacological therapy (p=0.498). A statistically significant increase in lipid intake was observed in the IG throughout the follow-up period compared to the CG (p<0.001). There were no differences for insulin therapy, but glycemic control was better in the GI group. Furthermore, a increase in protein, fiber and lipid intake was noted in the intervention group.

O objetivo do trabalho foi comparar a efetividade da orientação nutricional ambulatorial complementada por mídia digital com a orientação nutricional ambulatorial em gestantes com diabetes mellitus gestacional. Ensaio clínico randomizado, cego para pacientes. As gestantes foram alocadas aleatoriamente em dois grupos: o Grupo Controle (GC) recebeu tratamento nutricional exclusivamente ambulatorial e o Grupo Intervenção (GI) recebeu orientação nutricional ambulatorial e lembretes via WhatsApp. A ingestão alimentar (calorias, carboidratos, lipídios, proteínas e fibras) foi avaliada por meio de recordatórios alimentares de 24 horas (R24 horas). Também foram avaliados o controle glicêmico e a necessidade de tratamento farmacológico. Foram incluídas 81 mulheres, sendo 34 alocadas no GC e 47 no GI. Os pacientes foram acompanhados por um período de 4 a 8 semanas. Poucas diferenças foram observadas no controle glicêmico entre os dois grupos ao longo do período de acompanhamento. Não houve diferenças estatísticas na terapia farmacológica (p=0,498). Observou-se aumento estatisticamente significativo na ingestão lipídica no GI ao longo do período de seguimento em comparação ao GC (p<0,001). Não houve diferenças para a insulinoterapia, mas o controle glicêmico foi melhor no GI. Além disso, notou-se aumento na ingestão de proteínas, fibras e lipídios no GI
