This is an experimental study using a non-randomized controlled clinical trial design, involving 50 hemiparetic patients with stroke who are in the chronic phase of the condition (at least 6 months post-stroke). The following parameters will be assessed: cognitive level (Mini-Mental State Examination), balance (Berg Balance Scale), functional mobility (Timed Up and Go Test), muscle strength (dynamometry), and perceived quality of life (Nottingham Health Profile). Patients will be divided into three groups: an experimental group (EG), control group 1 (CG1), and control group 2 (CG2). The EG will undergo treatment based on an exercise program using the "MyBalance" serious game; CG1 will undergo treatment based on an exercise program using the "Stabball" serious game; and CG2 will undergo treatment based on a conventional exercise program. All groups will undergo the same treatment period—10 consecutive weeks with two sessions per week, totaling 20 sessions. Both interventions aim to improve patient balance. The serious game was developed specifically for stroke patient rehabilitation, involving the creation of dedicated software and hardware; consequently, it accounts for the patients' limitations, ensuring greater safety during application. For this experiment, the serious game settings will remain consistent for all patients. It should be noted that if a patient experiences any discomfort, malaise, or intense fatigue during training, the exercise will be stopped immediately. The tests and therapeutic procedures outlined in this project will be administered by the researchers themselves at the Neurorehabilitation Research Center (NUPEN), at Faculdade Guilherme Guimbala (FGG) in Joinville, Santa Catarina.

Trata-se de um estudo experimental, com delineamento do tipo Ensaio Clínico Controlado Não Randomizado, envolvendo 50 pacientes hemiparéticos com Acidente vascular cerebral (AVC), na fase crônica da doença (mínimo 6 meses pós-AVC). Serão avaliados: nível cognitivo (Mini-Exame do Estado Mental), equilíbrio (Escala de Equilíbrio de Berg), mobilidade funcional (Timed Up and Go Test), força muscular (Dinamometria) e percepção de qualidade de vida (Perfil de Saúde de Nottingham). Os pacientes serão divididos em 3 grupos: grupo experimental (GE - n=20), grupo controle 1 (GC1 - n=15) e grupo controle 2 (GC2 - n=15). O GE receberá tratamento baseado em um programa de exercícios utilizando o Jogo Sério "MyBalance", o GC1 receberá o tratamento baseado em um programa de exercícios utilizando o Jogo Sério "Stabball", enquanto o GC2 receberá o tratamento baseado em um programa exercícios convencionais. Ambos os grupos serão submetidos ao mesmo período de tratamento, que será de 10 semanas consecutivas, com frequência de duas sessões semanais, totalizando 20 sessões. Ambas intervenções possuem como objetivo melhorar o equilíbrio dos pacientes. O Jogo Sério foi desenvolvido especificamente para reabilitação de pacientes com AVC, sendo desenvolvidos software e hardware específicos. Portanto, leva em consideração as limitações apresentadas pelos pacientes, o que traz maior segurança na aplicação. Para este experimento, serão mantidas as mesmas configurações do JS para todos os pacientes. Cabe salientar que, caso o paciente sinta qualquer tipo de desconforto ou mal estar durante o treinamento, bem como fadiga intensa, o exercício será encerrado imediatamente. A aplicação dos testes e procedimentos terapêuticos previstos neste projeto serão realizados pelos próprios pesquisadores no Núcleo de Pesquisas em Neuroreabilitação (NUPEN), anexo ao Ambulatório de Reabilitação Neurológica do Curso de Fisioterapia, da Faculdade Guilherme Guimbala (FGG) em Joinville, Santa Catarina.

In the present study, baseline comparison tests across the three groups revealed that they were similar in all variables (age, time since stroke, Berg Balance Scale, Timed Up and Go Test, paretic quadriceps femoris strength, and Nottingham Health Profile), except for non-paretic quadriceps femoris strength (where a difference was observed). Thus, it is possible to establish a comparative overview of the effects of the different interventions performed with each group. Impaired balance following a stroke limits the performance of activities of daily living, social participation, and quality of life. Although conventional physical therapy is effective, digital rehabilitation technologies are increasingly being explored to boost patient motivation and active engagement. Serious Games are developed specifically for therapeutic purposes and incorporate rehabilitation principles; however, evidence regarding this type of resource remains limited. Therefore, this study aimed to compare the effects of balance training on an unstable board using a rehabilitation-specific Serious Game against those achieved via another Serious Game using a stable board and via conventional physical therapy in individuals with chronic post-stroke hemiparesis. Twenty-eight post-stroke hemiparetic patients were non-randomly assigned to three groups: the experimental group (EG) (n=12), control group 1 (CG1) (n=8), and control group 2 (CG2) (n=8). The EG underwent balance training using the myBalance Serious Game; CG1 underwent balance training using the Stabball Serious Game; and CG2 underwent training based on conventional kinesiotherapy. All groups received the same treatment duration: 10 consecutive weeks, totaling 20 sessions. Assessments covered balance, functional mobility, muscle strength, and quality of life. Balance training using the myBalance Serious Game yielded the best results, showing significant improvements with large effect sizes in balance and all other variables. Similarly, the group receiving the Stabball Serious Game intervention also showed significant improvements with large effect sizes across all variables, except for muscle strength in the paretic and non-paretic limbs, which showed small and moderate effect sizes, respectively. Finally, the group receiving conventional training showed significant improvements with large effect sizes only in the balance variable. The findings indicate that interventions based on the use of serious games hold beneficial potential for the rehabilitation of chronic post-stroke patients. O comprometimento do equilíbrio após o Acidente Vascular Cerebral (AVC) limita a realização das atividades de vida diária, a participação social e a qualidade de vida. Embora a fisioterapia convencional seja eficaz, as tecnologias digitais de reabilitação têm sido cada vez mais exploradas para aumentar a motivação e o envolvimento ativo dos pacientes. Os Jogos Sérios são desenvolvidos especificamente para fins terapêuticos e incorporam princípios da reabilitação, porém ainda são limitadas as evidências sobre esse tipo de recurso. Portanto, este estudo teve como objetivo comparar os efeitos de um treinamento de equilíbrio em prancha instável baseado em um Jogo Sério específico para reabilitação com aqueles obtidos por meio de outro Jogo Sério que utiliza uma prancha estável e por meio de fisioterapia convencional em indivíduos com hemiparesia crônica pós-AVC. 28 pacientes hemiparéticos pós-AVC foram divididos de forma não randomizado em 3 grupos: o grupo experimental (GE) (n=12) grupo controle 1 (GC1) (n=8) e o grupo controle 2 (GC2) (n=8). O GE recebeu um treinamento de equilíbrio baseado no Jogo Sério MyBalance, o GC1 recebeu um treinamento de equilíbrio baseado no Jogo Sério Stabball e o CG2 recebeu um treinamento baseado em cinesioterapia convencional. Ambos os grupos receberam o mesmo período de tratamento, 10 semanas consecutivas totalizando 20 sessões. Foram avaliados: equilíbrio, mobilidade funcional, força muscular e qualidade de vida. O treinamento do equilíbrio baseado no Jogo Sério MyBalance foi a intervenção que apresentou melhoras resultados, sendo encontradas melhorias significativas e com grande tamanho do efeito no equilíbrio e em todas as demais variáveis. De forma semelhante, o grupo que recebeu a intervenção com o Jogo Sério Stabball também apresentou melhoras significativas e com grande tamanho do efeito em todas as variáveis, exceto na força muscular no membro parético e não parético, com tamanho do efeito pequeno e moderado, respectivamente. Por fim, o grupo que recebeu o treinamento convencional apresentou melhoras significativas e com grande tamanho do efeito apenas na variável equilíbrio. Os achados indicam que as intervenções baseadas na utilização de Jogos Sérios possuem potencial benéfico na reabilitação de pacientes crônicos pós-AVC. A efetividade na melhora de múltiplos desfechos funcionais sugere que esse tipo de abordagem pode constituir uma estratégia terapêutica viável que pode servir como alternativa ou um recurso complementar à fisioterapia convencional, com potencial para aumentar o envolvimento dos pacientes e tornar o processo de reabilitação mais atrativo e prazeroso.
