This is an interventional study. A total of 50 volunteers of both genders and 65 years or older who agree to participate in the research and sign the informed consent form will be evaluated, using ultrasound, in 3 different positions: neutral position, shoulder rotation and lateral rotation of the surgical table. The following will be excluded: those who are allergic to ultrasound gel; who are unable to perform adequate positioning; whose image of the posterior longitudinal ligament has low quality or doubtful measurement; or who has a history of trauma, surgery or abnormality of the spine or shoulders. Neutral position: patient sitting, with flexion of the spine and chin against the chest; Shoulder rotation position: similar to the previous one, but with 45° contralateral rotation of the shoulders in the transverse plane; Position of lateral rotation of the table: similar to the neutral position, but with rotation of the surgical table by 10° in dorsal direction behind the patient. Using a paramedian sagittal oblique ultrasound section, the main researcher will save the images of the posterior longitudinal ligament obtained at the lumbar L3-L4 and thoracic T10-T11 levels in each of these positions, therefore totaling 6 images per patient. An assistant will make a simple draw of the images and send them 1 by 1 to the same main researcher to carry out the measurement, via computer software, of the posterior longitudinal ligament. The files will not contain any identification regarding the patient, level evaluated or positioning used, in order to guarantee blinding in the analysis of the images.

There was a significant increase in LLP with both maneuvers at the lumbar and thoracic levels. The mean differences with the use of the shoulder rotation maneuver were 0.8 mm (95% CI 0.2–1.5; p = 0.014) and 1.3 mm (95% CI 0.7–2.0; p < 0.001) at the L3–L4 and T10–T11 levels, respectively. With dorsal rotation of the surgical table, they were 2.0 mm (95% CI 1.4–2.8; p < 0.001) and 1.8 mm (95% CI 1.0–2.6; p < 0.001) at the L3–L4 and T10–T11 levels, respectively. Once the maneuver was fixed and the lumbar and thoracic levels were compared, there was no statistical difference for the shoulder rotation maneuver (p = 0.668) or the surgical table (p = 0.15). Once the level was fixed, but the different maneuvers were compared, there was a statistical difference between the table tilt and shoulder rotation at the L3-L4 level (p < 0.001), but not at the T10-T11 level (p = 0.176). In the analysis of an increase in the LLP of at least 1 mm, there was statistical significance for the dorsal rotation maneuver of the surgical table at the L3-L4 (p < 0.001) and T10-T11 (p = 0.029) levels; however, the shoulder rotation maneuver did not obtain statistical significance at the L3-L4 (p = 0.797) and T10-T11 (p = 0.166) levels

Results summary:

Elderly patients pose a considerable challenge to neuraxial anesthesia due to the anatomical and physiological changes associated with aging and comorbidities. Multiple puncture attempts, in addition to being uncomfortable, can lead to complications, such as subdural hematoma. To facilitate the technique, dorsal rotation maneuvers of the surgical table (table tilt) and shoulder rotation can increase the neuraxial window in young patients, but data on aged are still scarce. The present study aimed to evaluate whether table tilt and shoulder rotation can increase the size of posterior longitudinal ligament (PLL) visualized on ultrasound at L3-L4 and T10-T11 levels in elderly subjects and whether this increase would be a minimum of 1mm. This cross-sectional study was conducted at Hospital das Clínicas de Pernambuco and Hospital da Restauração, with 50 elderly volunteers, from whom ultrasound images of the posterior longitudinal ligament were collected at the L3-L4 and T10-T11 levels in three positions: sitting with spinal flexion (N position); 45º shoulder rotation (R position); and 10º surgical table rotation (T position). Four patients were excluded from the study due to their inability to tolerate the positions, and one volunteer was excluded due to loss of images. At the L3-L4 level, the dimensions of the posterior longitudinal ligament were 15.2 mm, 16.4 mm (p = 0.014), and 17.9 mm (p < 0.001) for the N, R, and T positions, respectively. At the T10-T11 level, the results were 13.0 mm, 14.3 mm (p < 0.001), and 14.8 mm (p < 0.001) for the N, R, and T positions, respectively. Table tilt was superior to shoulder rotation at L3-L4 level (p < 0.001), but not at T10-T11 level (p = 0.176) where both showed similar efficacy. There was no difference in the increase in posterior longitudinal ligament size between the lumbar and thoracic levels for either shoulder rotation (p = 0.688) or table tilt (p = 0.15). The analysis revealed a statistically significant correlation between an increase in posterior longitudinal ligament size of at least 1 mm and table tilt at L3-L4 (p < 0.001) and T10-T11 (p = 0.029) levels; However, shoulder rotation did not achieve statistical significance at L3-L4 (p = 0.797) and T10-T11 (p = 0.166) levels. Shoulder rotation and table tilt have been demonstrated to enhance the acoustic window in the lumbar (L3-L4) and thoracic (T10-T11) regions of elderly patients. Moreover, table tilt was found to augment the posterior longitudinal ligament by a minimum of 1 mm, a finding that was not observed in the case of shoulder rotation.
