Interventions

  This is a randomized clinical trial with a control group and an intervention group, with 39 patients with advanced gynecological and/or breast cancer in each group, totaling a sample of 78 participants. The clinical trial will be divided into three stages: baseline, intervention and post-intervention. Patients will be randomized between groups prior to their contact with the study through configuration on the RedCap platform. Baseline- Application of the TCLE, application of the questionnaires and guidance on the Go Wish Cards Game (GWCG). All patients, from both groups, will be recruited at the inpatient unit Barretos Cancer Hospital. The procedures will be performed by researcher 1 (B) and by researcher 2 (Research Group) to avoid bias, data impairment or even embarrassment of patients in their responses. After fulfilling the eligibility criteria, they will be invited to participate in the study, being guided about its procedures and the GWCG's objectives about their values and desires in relation to the end of life (this only for the Intervention Group) by researcher 1, followed by application of the TCLE. In addition to responding to the sociodemographic characterization questionnaire and the Meaning in Life Scale (meaning of life), optimism, understanding about their own treatment, ESAS Br (multiple symptoms with the addition of inner peace and spiritual pain), HADS (anxiety symptoms and depression) and FACIT Sp 12 (religiousness and spirituality) by researcher 2. The presentation of the letters, for patients in the intervention group, will initially be carried out with researcher 1, with a brief reading of each letter as a way of clarifying doubts about vocabulary and semantics. When there are no more doubts, the patient will keep the letters and will be instructed to reflect on each desire expressed as a way to identify priorities and for the subsequent fractionation of the letters in envelopes marked with three categories: “Very important” (green envelope) , “more or less important” (yellow envelope) and “not important” (red envelope). The control group will receive the standard procedures of the daily routine of clinical oncology. Intervention – GWCG The intervention will take place within 24 hours after the patients receive the letters and will be carried out by researcher 1. The patients will be encouraged by the researcher, in an active listening process, to expose the “Very important” letters already separated in the envelopes described above. After proper permissions, every dialogue will be recorded. At this stage, each letter will be addressed separately and the main function of the researcher is to guide the speech, seeking to get the patient to describe the meaning of each letter. This recording stage will also meet the specific objective of understanding the meaning of the GWCG for the women who will participate in the intervention, the lines presented at that time will be essential for the expression of values, desires and life plans. At this stage, all 10 cards chosen by the patient will be discussed one by one, aiming to understand the meaning of each card, record the wishes of this patient for later realization, address any doubts and needs that may arise during the speeches. It is hoped that with this intervention of the game of cards and desires, the patient can be stimulated to open up to a more personal dialogue, which, for many times, does not happen during the daily routine with the professionals.The function of the intervention is also to identify the feasibility of playing cards during the care of patients with gynecological and/or advanced breast cancer. After discussing all the letters, researcher 1 will inform the patient about the list of 10 wishes and the planning for their realization. Thus, a qualitative analysis of the 39 patients' speeches will be put into practice according to Bardin's methodology, which refers to a series of technical, systematic and objective procedures that seek to analyze the content described in the communications. Post-intervention - Telehealth - 30±3 days after intervention. Patients will already be oriented in advance on the application of the TCLE and after the intervention they will receive a notification through the instant messaging platform of the Hospital de Câncer de Barretos called ihelp, with the date and time schedule for participation in the post-intervention stage. On the scheduled day and time, an access link will be sent to the patient so that a video call can be made with the researcher 2. Patients will participate remotely through the voice and video call platform Anclinic, an online system for managing medical clinic and telemedicine. For this, the data are imported by the TASY agenda, the information management system used by the health institution. Patients will respond to the same assessment instruments applied at baseline: Meaning in Life Scale (meaning of life), optimism, understanding about their own treatment, ESAS Br (multiple symptoms with the addition of inner peace and spiritual pain), HADS (symptoms of anxiety and depression) and FACIT Sp 12 (religiousness and spirituality). Beyond the question, "What did playing cards mean to you?" In this same period, patients in the control group will also respond to the same assessment instruments applied at baseline: Meaning in Life Scale (meaning of life), optimism, understanding about their own treatment, ESAS Br (multiple symptoms with the addition of inner peace and pain spiritual), HADS (symptoms of anxiety and depression) and FACIT Sp 12 (religiousness and spirituality). Regarding the patients' wishes, they will be identified through the GWCG card game, the meaning of playing it and a set of information collected with an interview and the other questionnaires. In this way, it will be possible to identify the prognosis of each patient, their most important wishes, and the resources to carry them out, which will have the support of the hospital, the clinical unit, caregivers and family members, and especially the research group. Desires can sometimes be simple and without many difficulties to fulfill them, but a possible diversity should be considered since the research proposes a subjective and particular attention to each patient. Desires that will not be possible to fulfill due to their complexity, scarcity of resources and/or incompatibility of health due to the moment the patient is experiencing will be discussed with them to align possible adjustments so that the value and meaning of their desire is maintained. The time for fulfilling wishes can go beyond the period of hospitalization, taking into account that there will be a second stage of conversation with the patients and contact with them so that it is also possible to talk about the wishes listed in the game with the cards. Even so, it is possible that some wish(s) that the patient has chosen may not be fulfilled, but every effort will be made so that we can fulfill them

  Trata-se de um ensaio clínico randomizado com grupo controle e grupo intervenção, com 39 pacientes com câncer avançado ginecológico e/ou de mama em cada grupo, totalizando uma amostra de 78 participantes. O ensaio clínico será divido em três etapas: baseline, intervenção e pós-intervenção. As pacientes serão randomizadas entre os grupos anteriormente ao seu contato com o estudo através de configuração na plataforma RedCap. Baseline- Aplicação do TCLE, aplicação dos questionários e orientação sobre o Go Wish Cards Game (GWCG). Todas as pacientes, de ambos os grupos, serão recrutadas na unidade de internação Hospital de Câncer de Barretos. Os procedimentos serão realizados pela pesquisadora 1 (B) e pela pesquisadora 2 (Grupo de Pesquisa) para evitar viés, comprometimento de dados ou ainda constrangimento das pacientes em suas respostas. Preenchendo os critérios de elegibilidade, as mesmas serão convidadas a participar do estudo, sendo orientadas acerca dos procedimentos deste e objetivos do GWCG sobre seus valores e desejos em relação ao fim de vida (este apenas para o Grupo Intervenção) pela pesquisadora 1, seguida da aplicação do TCLE. Além de responderem ao questionário de caracterização sociodemográfica e aos questionários Meaning in Life Scale (sentido de vida), otimismo, compreensão sobre o seu próprio tratamento, ESAS Br (múltiplos sintomas com adição de paz interior e dor espiritual), HADS (sintomas de ansiedade e de depressão) e FACIT Sp 12 (religiosidade e espiritualidade) pela pesquisadora 2. A apresentação das cartas, para as pacientes do grupo intervenção, será realizada inicialmente junto da pesquisadora 1, com leitura breve de cada carta como forma de esclarecer dúvidas sobre vocabulário e semântica. Quando não demonstrarem mais dúvidas, a paciente ficará com a posse das cartas e será orientada a refletir sobre cada desejo expresso como forma de identificar prioridades e para o posterior fracionamento das cartas em envelopes sinalizados com três categorias: “Muito importante” (envelope verde), “mais ou menos importante” (envelope amarelo) e “não importante” (envelope vermelho). O grupo controle receberá os procedimentos padrões da rotina diária da oncologia clínica. Intervenção – GWCG: A intervenção acontecerá até 24 horas após as pacientes receberem as cartas e será realizada pela pesquisadora 1. As pacientes serão estimuladas pela pesquisadora, em processo de escuta ativa, para expor as cartas “Muito importantes” já separadas nos envelopes descritos anteriormente. Depois de devidas permissões, todo diálogo será gravado. Nesta etapa, cada carta será abordada separadamente e a principal função da pesquisadora é nortear o discurso, buscando levar a paciente a descrever o significado de cada carta. Essa etapa de gravação atenderá também ao objetivo específico de compreender o significado do GWCG para as mulheres que participarão da intervenção, as falas apresentadas nesse momento serão primordiais para a expressão de valores, desejos e planejamentos da vida. Nesta etapa, todas as 10 cartas escolhidas pela paciente serão discutidas uma a uma, visando compreender o significado de cada carta, registrar os desejos desta paciente para posterior realização, direcionar algumas dúvidas e necessidades que possam surgir durante os discursos. Espera-se que com essa intervenção do jogo de cartas e desejos possa ser estimulado que a paciente se abra para um diálogo mais pessoal e que, por muitas vezes, não acontece durante a rotina diária com os profissionais. A função da intervenção é identificar também a viabilidade de jogar as cartas durante o cuidado às pacientes com câncer ginecológico e/ou de mama avançado. Após as discussões de todas as cartas, a pesquisadora 1 comunicará à paciente quanto a lista dos 10 desejos e o planejamento para a realização dos mesmos. Dessa forma, uma análise qualitativa dos discursos das 39 pacientes será colocada em prática atendendo a metodologia de Bardin, que se refere a uma série de procedimentos técnicos, sistemáticos e objetivos que buscam analisar o conteúdo descrito das comunicações. Pós-intervenção - Telesaúde - 30±3 dias após intervenção: As pacientes já serão orientadas previamente na aplicação do TCLE e após a intervenção receberão uma notificação pela plataforma de mensagens instantâneas do Hospital de Câncer de Barretos denominada ihelp, com a programação de data e horário para a participação na etapa de pós-intervenção. No dia e horário agendados, um link de acesso será enviado para a paciente para que seja efetivada uma vídeo chamada com a pesquisadora 2. As pacientes participarão de forma remota por meio da plataforma de chamadas de voz e vídeo Anclinic, sistema online para gestão de clínica médica e telemedicina. Para isso, os dados são importados pela agenda do TASY, sistema de gestão de informação utilizada pela instituição de saúde. As pacientes responderão aos mesmos instrumentos de avaliação aplicados no baseline: Meaning in Life Scale (sentido de vida), otimismo, compreensão sobre o seu próprio tratamento, ESAS Br (múltiplos sintomas com adição de paz interior e dor espiritual), HADS (sintomas de ansiedade e de depressão) e FACIT Sp 12 (religiosidade e espiritualidade). Além da pergunta: "O que significou para você jogar as cartas?" Neste mesmo período, pacientes do grupo controle também responderão aos mesmos instrumentos de avaliação aplicados no baseline: Meaning in Life Scale (sentido de vida), otimismo, compreensão sobre o seu próprio tratamento, ESAS Br (múltiplos sintomas com adição de paz interior e dor espiritual), HADS (sintomas de ansiedade e de depressão) e FACIT Sp 12 (religiosidade e espiritualidade). Em relação aos desejos das pacientes, eles serão identificados por meio do jogo de cartas GWCG, do significado de jogá-lo e de um conjunto de informações coletadas com uma entrevista e os demais questionários. Dessa forma, será possível identificar o prognóstico de cada paciente, seus desejos mais importantes, e os recursos para realizá-los que contará com o suporte do hospital, da unidade clínica, dos cuidadores e familiares, e especialmente do grupo de pesquisas. Os desejos por vezes poderão ser simples e sem muitas dificuldades para realizá-los, mas, deve-se considerar uma possível diversidade já que a pesquisa propõe uma atenção subjetiva e particular de cada paciente. Os desejos que não serão possíveis de se realizar por sua complexidade, escassez de recursos e/ou incompatibilidade de saúde pelo momento vivido pela paciente serão conversados com elas para alinhar possíveis ajustes para que o valor e significado de seu desejo seja mantido. O tempo de realização dos desejos pode ir além do período de internação levando-se em consideração que existirá uma segunda etapa de conversa com as pacientes e o contato com as mesmas para que seja possível também conversar sobre os desejos elencados no jogo com as cartas. Mesmo assim, é possível que algum (s) desejo (s) que a paciente tenha escolhido possa não ser realizado, mas todos os esforços serão feitos para que possamos realizá-lo (s)
