This is a quasi-experimental, one-arm, open-label, before-and-after study. The comparison of the warfarin adherence score and knowledge about treatment before and after the application of an educational intervention, based on the Educational Support System of Orem's Systems Theory, was evaluated. The intervention was guided by a script developed by the authors to promote self-care in anticoagulated patients. Twenty-five patients on chronic warfarin use participated in the study, followed for an average time of 38 days, with weekly telephone contacts and reinforcement of guidelines through electronic messages. The AA was applied through the 5A's: Assessment; Counseling; Agreement; Assistance and Monitoring.

SOCIODEMOGRAPHIC AND CLINICAL PROFILE The study included 25 patients (single arm) who were chronically using warfarin. Most of the patients were female, with an average age of 58 years, non-white, and living in a marital union. Although some patients lived in the interior of the state of Pernambuco, the majority lived in the capital or metropolitan region. Regarding occupation, almost all participants did not have a formal employment relationship, but were retired or received some form of social security benefit. Low levels of education were also prevalent, as was a per capita family income of less than half the minimum wage. Regarding the cost of treatment, most participants paid for their own transportation to the outpatient clinic and for the anticoagulant medication. The clinical information provided by the participants shows that the main indications for the use of the anticoagulant medication were atrial fibrillation and heart valve replacement. All participants used warfarin as a coumarin anticoagulant, with a greater use of the medication with the commercial name Marevan. The median treatment time was 2 years, with a lower percentage of patients using it for longer than this period. Regarding the history of previous complications during treatment, 48% mentioned at least one episode of hemorrhagic or thromboembolic complications. Previous events of discontinuing treatment on their own were reported by 24% of participants. Although many patients reported having received guidance from health professionals about their treatment with OAC, 36% of the sample still had doubts about the treatment in question. Regarding the health profile of the participants, the diagnosis of multiple comorbidities was common in patients, with a greater predominance of arterial hypertension, dyslipidemia and diabetes mellitus. In this scenario, a consequent use of other medications to control other chronic conditions was found

PERFIL SOCIODEMOGRÁFICO E CLÍNICO Foram acompanhados durante o estudo 25 pacientes (braço único) em uso crônico de varfarina, dentre os quais foi preponderante indivíduos do sexo feminino, com idade média de 58 anos, raça não branca e que vivem em união conjugal. Embora alguns pacientes residam no interior do Estado de Pernambuco, a maioria residia na capital ou região metropolitana. Quanto à ocupação, quase todos os participantes não possuíam vínculo empregatício, no entanto eram aposentados ou recebiam algum benefício previdenciário. Ainda foi predominante uma baixa escolaridade, assim como uma renda per capita familiar inferior à meio salário-mínimo. No tocante ao custeio com o tratamento, a maioria dos participantes financiavam os custos com o transporte até o ambulatório e com a medicação anticoagulante. As informações clínicas dos participantes, demonstram que as principais indicações para uso do medicamento anticoagulante foram a fibrilação atrial e a troca de válvula cardíaca. Todos os participantes faziam uso da varfarina como anticoagulante cumarínico, com destaque para uma maior utilização do medicmento com nome comercial Marevan. O tempo mediano de tratamento foi de 2 anos, com um menor percentual de pacientes em uso superior a esse período. Sobre o histórico de complicações prévias no tratamento, 48% mencionou ao menos um episódio de complicação hemorrágica ou tromboembólica. Eventos anteriores de interrupção do tratamento por conta própria foi relatado por 24% dos participantes. Apesar de muitos pacientes afirmarem ter recebido orientações de profissionais de saúde acerca do seu tratamento com ACO, 36% da amostra ainda tinham dúvidas sobre o referido tratamento. Com relação ao perfil de saúde dos participantes, foi comum o diagnóstico de múltiplas comorbidades nos pacientes, com maior predominância de hipertensão arterial, dislipidemia e diabetes mellitus. OUTCOMES: KNOWLEDGE AND ADHERENCE With regard to knowledge about warfarin treatment, before the intervention, a mean score of 5.24 (standard deviation 1.38) was observed. In the categorization, 36 percent of patients were classified as having insufficient knowledge, 64 percent as having regular knowledge, and no participant had a satisfactory level of knowledge at baseline. After the educational intervention, the knowledge score increased to 7.74 (standard deviation 1.24), with a positive migration between the knowledge categories being observed, with no patient having a deficit of knowledge about their treatment, 56 percent with regular knowledge, and 44 percent with satisfactory knowledge. With regard to medication adherence to warfarin, it was observed that at the beginning of the study (before the educational intervention), the participants had a mean score of 5.24 (standard deviation 0.74). In the distribution between the levels of adherence, 76 percent and 24 percent were classified as having low and medium adherence, respectively. No patient achieved satisfactory adherence at the beginning of the study. After the nursing intervention, the mean adherence score increased to 7.31 (standard deviation 0.74) and the low, medium and high adherence categories were seen among 8 percent, 52 percent and 40 percent of the participants, respectively. This difference in scores presented in both outcomes studied demonstrated statistical significance (p value 0.000) verified by the Wilcoxon test

Introduction: Treatment with oral anticoagulants includes monitoring the International Normalized Ratio (INR), cessation of alcohol consumption and monitoring food and pharmacological interactions. Such measures, added to medication adherence, constitute relevant aspects of self-care. Knowledge about the treatment constitutes an important path towards adopting the recommended behaviors. Using educational strategies such as Supported Self-Care (AA) can bring benefits to treatment. General objective: To evaluate the effect of AA via teleconsultation on knowledge and adherence to treatment with oral anticoagulants. Methods: A quasi-experimental study was carried out to compare warfarin knowledge and adherence scores before and after the remote application of an educational intervention, based on the Orem Systems Theory Support Educational System. The intervention was guided by a script prepared by the authors to promote self-care for anticoagulated patients, which was submitted to content validation by 14 experts, using the Delphi technique. The study included 25 patients recruited from the oral anticoagulation outpatient clinic of the Pernambuco Cardiological Emergency Room, between April 2022 and March 2023. The average follow-up time was 38 days, with weekly telephone contacts and reinforcement of guidance via electronics messages. The application of AA occurred through the 5A's: Assessment; Counseling; Agreement; Assistance and Monitoring. In data collection, sociodemographic and clinical questionnaires, therapeutic knowledge questionnaires and the Morisky adherence scale with 8 items were used. The study was carried out after approval from the Human Research Ethics Committee. Data analysis included descriptive statistics and the Wilcoxon test for paired samples, Mann-Whitney and Kruskal Wallis with statistical significance with p value lower 0.005. Results: The average age was 58.04 (standard deviation 11.7), with a predominance of women, mixed/black ethnicity, married, from Recife and the Metropolitan Region, with no work activity, with low education and per capita income. Before the intervention, there was a lack of knowledge about the side effects of the medication, the care required for treatment, the therapeutic value of the INR, factors that interfere with the stability of the INR, the consequence of not using the medication, as well as the clinical indication. Non-adherence attitudes were generally unintentional and related to forgetfulness. It was observed that after conducting the AA, there was an increase in knowledge about the treatment and medication adherence (p=0.000), confirming the proposed thesis. Participants with better knowledge about the treatment achieved greater coagulation stability after the intervention (p value of 0.019). Better knowledge scores were associated with individuals under 60 years of age (p value of 0.030), who paid for their INR exams (p value of 0.045) and without reports of bleeding complications (p value of 0.039). No significant associations were found between adherence and the other variables studied. The difficulties most reported by patients in achieving agreed self-care goals were personal and professional/institutional. The study showed that supporting self-care for anticoagulated patients, with individualized interventions, makes it possible to obtain satisfactory clinical results

O conhecimento sobre o tratamento, configura um caminho importante para a adoção dos comportamentos recomendados. Utilizar estratégias educacionais como o Autocuidado Apoiado (AA), pode trazer benefícios ao tratamento. Objetivo geral: Avaliar o efeito do AA por teleconsulta no conhecimento e na adesão ao tratamento com anticoagulantes orais. Métodos: Realizou-se um estudo quase-experimental para comparar os escores do conhecimento e da adesão à varfarina antes e após a aplicação remota de uma intervenção educativa, fundamentada pelo Sistema Educativo de Apoio da Teoria dos Sistemas de Orem. A intervenção foi guiada por um roteiro elaborado pelas autoras para a promoção ao autocuidado de pacientes anticoagulados, o qual foi submetido à validação de conteúdo por 14 especialistas, utilizando-se a tecnica de Delphi. Participaram do estudo 25 pacientes recrutados no ambulatório de anticoagulação oral do Pronto Socorro Cardiológico de Pernambuco, entre os meses de abril de 2022 e março de 2023. O tempo médio de seguimento foi de 38 dias, com contatos telefônicos semanais e reforço das orientações por mensagens eletrônicas. A aplicação do AA ocorreu por meio dos 5A's: Avaliação; Aconselhamento; Acordo; Assistência e Acompanhamento. Na coleta dos dados, utilizou-se os questionários sociodemográfico e clínico, de conhecimento terapêutico e a escala de adesão de Morisky com 8 itens. A realização do estudo se deu após a aprovação do Comitê de ética em Pesquisa com seres humanos. A análise dos dados contou com a estatística descritiva e os testes de Wilcoxon para amostras pareadas, Mann-whitney e Kruskal Wallis com significância estatística com p valor inferior a 0,005. Resultados: A média de idade foi de 58,04 (desvio padrão 11,7), com predominância de mulheres, etnia parda/negra, casadas, procedente de Recife e Região Metropolitana, sem atividade laboral, com baixa escolaridade e renda per capita. Antes da intervenção, havia um déficit do conhecimento sobre os efeitos colaterais da medicação, cuidados necessários ao tratamento, valor terapêutico do INR, fatores que interferem a estabilidade do INR, consequência da não utilização do medicamento, assim como sobre a indicação clínica. As atitudes de não adesão geralmente foram não intencionais e estavam relacionadas ao esquecimento. Observou-se que após a condução do AA, houve um aumento do conhecimento sobre o tratamento e da adesão medicamentosa (p valor de 0,000), confirmando a tese proposta. Os participantes com melhor conhecimento sobre o tratamento obtiveram maior estabilidade na coagulação após a intervenção (p valor de 0,019). Melhores escores de conhecimento foram associados aos indivíduos com menos de 60 anos (p valor de 0,030), que custeavam seus exames do INR (p valor de 0,045) e sem relatos de complicações hemorrágicas (p valor de 0,039). Não foram encontradas associações significativas entre a adesão e as outras variáveis estudadas. As dificuldades mais relatadas pelos pacientes para o cumprimento das metas de autocuidado pactuadas, foram de aspecto pessoal e profissional/institucional. O estudo evidenciou que o suporte ao autocuidado dos pacientes anticoagulados, com intervenções individualizadas, possibilita a obtenção de resultados clínicos satisfatórios
