  The experimental group will receive advice to stay active associated with neural management, and the control group will receive advice to stay active alone. We will recruit 105 participants per group, totalizing 210 participants. Advice to stay active - Experimental and control participants will receive the same advice to stay active protocol. Participants will receive advice to stay active in their daily living activities, information on physical activity, imaging tests, and sciatica for 5 biweekly sessions that will be performed in person and individually, lasting 25-30 minutes. Advice to stay active will focus on two themes: potential harmful effects of pain and beneficial effects on pain. Neural management - Participants will receive neural mobilisation techniques and soft tissue mobilisation techniques for 30 minutes per session, 10 weekly sessions, considering the participant’s availability. In each neural management session, participants will receive the following protocol in the affected lower limb, based on a previous study. The protocol include Myofascial release techniques in the piriformis and biceps femoral muscles, a friction technique will be applied in the craniocaudal direction on the sacroiliac joint, hip joint mobilisation, and neural mobilisation in the sciatic nerve.

  O grupo experimental receberá orientação para manter-se ativo adicionada ao gerenciamento do tecido neural e o grupo controle receberá somente o protocolo de orientação para manter-se ativo. Serão recrutados 105 participantes por grupo, totalizando 210 participantes. Orientação para manter-se ativo - O grupo experimental e o grupo controle receberão o mesmo protocolo de orientação para manter-se ativo. Os participantes receberão como intervenção a orientação para manter-se ativo em suas atividades diárias, informações sobre atividades físicas, exames de imagem e informações sobre dor ciática quinzenalmente por 5 sessões de forma presencial e individual, durando aproximadamente 25-30 minutos cada sessão. A orientação para manter-se ativo irá se concentrar em dois temas: (1) Efeitos nocivos para a dor e (2) Efeitos benéficos para a dor. Gerenciamento do tecido neural - Os participantes receberão técnicas de mobilização neural e técnicas de mobilização de tecidos moles por 30 minutos a cada sessão, em 10 sessões de forma semanal, considerando a disponibilidade do participante. Em cada sessão de gerenciamento do tecido neural os participantes receberão o protocolo no membro inferior afetado, baseado em um estudo prévio do nosso grupo de pesquisa. O protocolo inclui técnicas miofasciais nos músculos piriforme e bíceps femoral, uma técnica miofascial sobre a articulação sacroilíaca, técnicas de mobilização da articulação do quadril e técnicas de mobilização do nervo ciático.

  Participants in treatment due to sciatica; participants planning to undergo any other procedures for sciatica treatment in the study period; present known or signs of serious spinal pathology; history of abdominal, lumbar, or lower limb surgery in the past year; rheumatologic disease in the acute inflammatory phase; if they are receiving disability compensation due to LBP; pregnant or puerperal women; self-reported psychiatric diagnosis; history of cancer; and cancer-related pain

  We will evaluate leg pain intensity and functional limitation of patients with chronic sciatica. It is expected that patients with chronic sciatica allocated to the neural management group added advice to remain active will have a statistically significant reduction in pain intensity and improved functionality after the treatment period (short, medium and long term) in a way superior to patients with chronic sciatica allocated in the group of only advice to remain active

  Avaliaremos a intensidade da dor nas pernas e a limitação funcional dos pacientes com ciatalgia crônica. Acreditamos que os pacientes com ciatalgia crônica alocados no grupo gerenciamento do tecido neural adicionado a orientação para manter-se ativo apresentem uma redução estatisticamente significativa da intensidade de dor e melhora da funcionalidade após o período de tratamento (curto, médio e longo prazo) de forma superior aos pacientes com ciatalgia crônica alocados no grupo de somente orientação para manter-se ativo

  We will evaluate neuropathic symptoms, sciatic neurodynamic, pain modulation, and psychosocial factors. It is expected that patients included in the experimental group will improve the sciatic neurodynamic after the treatment period in a superior way compared to the control group. The analysis of the participant's baseline characteristics will provide information on the most relevant aspects of the participant's self-reported pain perception. In addition, the analysis of mediators will make it possible to identify potential factors that explain the clinical improvement of the participant with chronic sciatica submitted to the study interventions.

  Avaliaremos os sintomas neuropáticos, neurodinâmica do nervo ciático, modulação da dor e fatores psicossociais. Espera-se que pacientes incluídos no grupo experimental apresentem uma melhora da neurodinâmica do nervo ciático após o período de tratamento de forma superior quando comparados ao grupo controle. A análise das características iniciais do participante fornecerá informações sobre os aspectos mais relevantes na percepção de dor autorreferida do participante. Ademais, a análise de mediadores possibilitará identificar potenciais fatores que expliquem a melhora clínica do participante com ciatalgia crônica submetido às intervenções do estudo.
