This is a two-arm, observer-blind, randomized controlled clinical trial. A total of 40 participants who meet the eligibility criteria will be randomized using sequentially numbered, opaque, sealed envelopes. Group I will undergo application of the 1640 Nd:YAG laser and group II application of the 1640 Nd:YAG laser combined with 2940 Er:YAG. In addition, Group II participants will receive 8% ciclopirox olamine solution (topical nail route), applying a thin layer covering the entire surface of the nail plate and periungual region, once daily, at night, throughout the study period, according to medical guidance. All participants with subungual hyperkeratosis greater than two millimeters will undergo prior filing of the nail plate with a dermabrasor, immediately before each laser session, aimed at reducing thickness and facilitating energy delivery and therapeutic penetration. Experimental group one: 20 participants will undergo the use of the 1064 nm Nd:YAG laser with a fluence of 14 J/cm², pulse duration of 0.6 ms, spot size of 5 mm, and frequency of 3 Hz. Irradiation will be performed over the entire surface of the nail plate, including the region corresponding to the germinal matrix, through longitudinal and transverse passes, for 3 minutes per nail, maintaining a target temperature of 40–42 °C, totaling 4 sessions with a 21-day interval between applications. Experimental group two: 20 participants will undergo the 1064 nm Nd:YAG laser with parameters and technique identical to group one, combined with the use of the 2940 nm Er:YAG laser after the first and third sessions. The 2940 nm Er:YAG laser will be applied with a fluence of 30 J, pulse duration of 3 ms, spot size of 1 mm, and frequency of 1 Hz to perform micro-perforations in the nail plate with the purpose of increasing the penetration of the topical antifungal, which will be applied topically daily by the participant throughout the study period, as prescribed, totaling 4 sessions with a 21-day interval between applications.

Trata-se de um estudo clínico randomizado controlado de dois braços, observador cego. Um total de 40 participantes que preencham os critérios de elegibilidade serão randomizados usando envelopes opacos e selados com numeração sequencial. O grupo I será submetido a aplicação do Nd:YAG laser 1640 e o grupo II aplicação do Nd:YAG laser 1640 associado com Er:YAG 2940. Além disso, os participantes do Grupo II receberão solução de ciclopirox olamina a 8% (via tópica ungueal), aplicando uma fina camada cobrindo toda a superfície da lâmina ungueal e região periungueal, uma vez ao dia, à noite, durante todo o período do estudo, conforme orientação médica.Todos os participantes com hiperqueratose subungueal maior que dois milímetros serão submetidos a lixamento prévio da lâmina ungueal com dermoabrasor, imediatamente antes de cada sessão de laser, com o objetivo de reduzir a espessura e facilitar a entrega de energia e penetração terapêutica. Grupo experimental um: 20 participantes serão submetidos ao uso do laser Nd:YAG 1064 nm com fluência de 14 J/cm², duração de pulso de 0,6 ms, spot de 5 mm e frequência de 3 Hz. A irradiação será realizada em toda a superfície da lâmina ungueal, incluindo a região correspondente à matriz germinativa, por meio de passadas longitudinais e transversais, durante 3 minutos por unha, mantendo temperatura alvo de 40–42 °C, totalizando 4 sessões com intervalo de 21 dias entre as aplicações. Grupo experimental dois: 20 participantes serão submetidos ao laser Nd:YAG 1064 nm com parâmetros e técnica idênticos ao grupo um, associado ao uso do laser Er:YAG 2940 nm após a primeira e a terceira sessões. O laser Er:YAG 2940 nm será aplicado com fluência de 30 J, duração de pulso de 3 ms, spot de 1 mm e frequência de 1 Hz para realizar microperfurações na lâmina ungueal com a finalidade de aumentar a penetração do antifúngico tópico, o qual será aplicado via tópica diariamente pelo participante durante todo o período do estudo, conforme prescrição, totalizando 4 sessões com intervalo de 21 dias entre as aplicações.

Outcome 2: A nail was classified as having complete cure when it achieved both mycological cure and clinical cure. According to this criterion, of the 22 nails evaluated, five achieved an OSI score of zero; thus, 22.7% of nails showed clinical cure (95% CI: 7.8%–45.4%). Among the five nails with clinical cure, three were treated with 1064 (37.5%; 95% CI: 8.5%–75.5%) and two with 1064+2940 (14.3%; 95% CI: 1.8%–42.8%). Using Fisher’s exact test, there was no statistically significant difference between treatments regarding clinical cure (p=0.309) at 20 months after the start of the intervention.

Among the twelve patients included in the study, mycological cure was observed in 11, corresponding to 91.7% (95% CI: 61.5%–99.8%). The only patient who did not achieve mycological cure belonged to the 1064+2940 group and contributed two nails; therefore, mycological cure occurred in 20 of the 22 nails evaluated (90.9%; 95% CI: 70.8%–98.8%). Among the 8 nails treated with 1064, all achieved mycological cure (100%). In contrast, among the 14 nails treated with 1064+2940, 85.7% achieved mycological cure (95% CI: 57.2%–98.2%).

Expected outcome 2: It is expected to analyze whether, after achieving clinical and mycological cure, the treatment efficacy will persist for a period of eighteen months after the end of the interventions, in addition to studying the physiological and morphological changes of the fungus resulting from the application of the Nd:YAG 1064 nm laser.

A total of 116 patients with clinical suspicion of onychomycosis were assessed for eligibility. Of these, 104 patients (89.7%) were excluded for not presenting culture-confirmed persistent mycosis (eligibility criterion), leaving 12 eligible patients (10.3%; 95% CI: 5.5%–17.4%).
The 12 eligible patients were randomized into the two treatment groups:
Group 1 (Nd:YAG 1064 nm alone): 5 patients allocated, totaling 8 affected nails.
Group 2 (Nd:YAG 1064 nm + Er:YAG 2940 nm + topical 8% ciclopirox olamine): 7 patients allocated, totaling 14 affected nails.
All 12 randomized patients (100%) completed the treatment protocol in full (4 laser sessions, 21 days apart) and the 18-month follow-up period (15 scheduled visits, V1 to V15). There was no loss to follow-up in either group.
Therefore, the 12 patients (22 nails) randomized were the same 12 patients (22 nails) included in the final outcome analysis, with no post-randomization exclusions.pt-br
Foram avaliados 116 pacientes com suspeita clínica de onicomicose para elegibilidade no estudo. Destes, 104 pacientes (89,7%) foram excluídos por não apresentarem micose persistente confirmada por cultura (critério de elegibilidade), restando 12 pacientes elegíveis (10,3%; IC95%: 5,5%–17,4%).
Os 12 pacientes elegíveis foram randomizados nos dois grupos de tratamento:
Grupo 1 (Nd:YAG 1064 nm isolado): 5 pacientes alocados, totalizando 8 unhas acometidas.
Grupo 2 (Nd:YAG 1064 nm + Er:YAG 2940 nm + ciclopiroxolamina 8% tópica): 7 pacientes alocados, totalizando 14 unhas acometidas.
Todos os 12 pacientes randomizados (100%) completaram integralmente o protocolo de tratamento (4 sessões de laser, com intervalo de 21 dias entre elas) e o período de acompanhamento de 18 meses (15 visitas programadas, V1 a V15). Não houve nenhuma perda de seguimento em nenhum dos dois grupos.
Dessa forma, os 12 pacientes (22 unhas) randomizados foram os mesmos 12 pacientes (22 unhas) incluídos na análise final dos desfechos, sem exclusões pós-randomização. -
Adverse events:

During the study, only mild, local, and transient adverse events related to the laser procedure were observed, with no need for treatment interruption in any case. There were no serious adverse events or deaths during the follow-up period.

All events were classified as mild, local, and transient, resolving spontaneously after the laser session, with no need for additional intervention or treatment discontinuation. No serious adverse events, systemic reactions, or study-related deaths were observed in either group.pt-br
Durante o estudo foram observados apenas eventos adversos leves, locais e transitórios, relacionados ao procedimento com laser, sem necessidade de interrupção do tratamento em nenhum caso. Não houve nenhum evento adverso grave nem óbitos durante o período de acompanhamento.

Mycological cure (defined as negative direct microscopy and culture) occurred in 11 of the 12 patients evaluated (91.7%; 95% CI: 61.5%–99.8%), corresponding to 20 of the 22 nails evaluated (90.9%; 95% CI: 70.8%–98.8%); by group, in Group 1 (Nd:YAG 1064 nm alone) all 8 nails (100%) achieved mycological cure, while in Group 2 (Nd:YAG 1064 nm + Er:YAG 2940 nm + 8% ciclopirox olamine) 12 of 14 nails (85.7%; 95% CI: 57.2%–98.2%) achieved mycological cure, with the single patient without mycological cure belonging to Group 2 and contributing 2 nails to the study. Clinical cure (defined as an OSI score of zero, i.e., 100% nail clearance) occurred in 5 of the 22 nails evaluated (22.7%; 95% CI: 7.8%–45.4%), with 3 nails in Group 1 (37.5%; 95% CI: 8.5%–75.5%) and 2 nails in Group 2 (14.3%; 95% CI: 1.8%–42.8%), with no statistically significant difference between groups (Fisher's exact test, p=0.309). Total cure (simultaneous mycological and clinical cure) occurred in only 1 of the 22 nails evaluated (4.5%). As secondary outcomes, the OSI score showed a statistically significant reduction from baseline to post-treatment in both groups (p<0.05), with a mean reduction of 12.4 ± 4.5 points in Group 1 (from 22.1 ± 4.3 to 9.7 ± 4.3; p=0.006) and 10.1 ± 3.4 points in Group 2 (from 15.1 ± 3.4 to 5.0 ± 3.4; p=0.003), with no significant difference between groups at baseline (p=0.194) or post-treatment (p=0.382), and no interaction effect between time and treatment group by repeated-measures analysis of variance (p=0.681), indicating a similar OSI score trajectory over time in both groups; regarding the reduction in OSI classification by severity category, 6 of 8 nails in Group 1 (75.0%) and 9 of 14 nails in Group 2 (64.3%) showed a reduction of at least one category, a difference that was not statistically significant between groups (Fisher's exact test, p>0.999). The statistical tests used were Fisher's exact test and the Wilcoxon signed-rank test for categorical variables and repeated-measures analysis of variance (ANOVA) for the OSI score over time, with a significance level of p<0.05 and 95% confidence intervals for all proportion estimates.

A cura micológica (definida como exame micológico direto e cultura negativos) ocorreu em 11 dos 12 pacientes avaliados (91,7%; IC95%: 61,5%–99,8%), correspondendo a 20 das 22 unhas avaliadas (90,9%; IC95%: 70,8%–98,8%); por grupo, no Grupo 1 (Nd:YAG 1064 nm isolado) todas as 8 unhas (100%) apresentaram cura micológica, enquanto no Grupo 2 (Nd:YAG 1064 nm + Er:YAG 2940 nm + ciclopirox olamina 8%) 12 das 14 unhas (85,7%; IC95%: 57,2%–98,2%) apresentaram cura micológica, sendo que o único paciente sem cura micológica pertencia ao Grupo 2 e contribuiu com 2 unhas para o estudo. A cura clínica (definida como escore OSI igual a zero, ou seja, 100% de melhora/clareamento da unha) ocorreu em 5 das 22 unhas avaliadas (22,7%; IC95%: 7,8%–45,4%), sendo 3 unhas no Grupo 1 (37,5%; IC95%: 8,5%–75,5%) e 2 unhas no Grupo 2 (14,3%; IC95%: 1,8%–42,8%), sem diferença estatisticamente significante entre os grupos (teste exato de Fisher, p=0,309). A cura total (cura micológica e clínica simultâneas) ocorreu em apenas 1 das 22 unhas avaliadas (4,5%). Como desfechos secundários, o escore OSI apresentou redução estatisticamente significante do momento pré-tratamento para o pós-tratamento em ambos os grupos (p<0,05), sendo a redução média de 12,4 ± 4,5 pontos no Grupo 1 (de 22,1 ± 4,3 para 9,7 ± 4,3; p=0,006) e de 10,1 ± 3,4 pontos no Grupo 2 (de 15,1 ± 3,4 para 5,0 ± 3,4; p=0,003), sem diferença significante entre os grupos no escore basal (p=0,194) ou pós-tratamento (p=0,382), e sem efeito de interação entre momento de avaliação e grupo de tratamento pela análise de variância com medidas repetidas (p=0,681), indicando comportamento semelhante do escore OSI ao longo do tempo em ambos os grupos; quanto à redução da classificação OSI por categoria de gravidade, 6 das 8 unhas do Grupo 1 (75,0%) e 9 das 14 unhas do Grupo 2 (64,3%) apresentaram redução de pelo menos uma categoria, diferença não estatisticamente significante entre os grupos (teste exato de Fisher, p>0,999). Os testes estatísticos utilizados foram o teste exato de Fisher e o teste de postos com sinais de Wilcoxon para variáveis categóricas e a análise de variância (ANOVA) com medidas repetidas para o escore OSI ao longo do tempo, adotando-se nível de significância de p<0,05 e intervalo de confiança de 95% para todas as estimativas de proporção.

Onychomycosis is a common fungal nail infection that is challenging to treat due to the long treatment period required, poor patient adherence, and high recurrence rates, with systemic treatment being the most effective option, though often contraindicated due to side effects and drug interactions, while topical therapy alone has a low cure rate, making laser therapy a selective, non-invasive, and safe alternative. The objective of this study was to compare the efficacy of Nd:YAG 1064 nm laser alone versus its combination with fractional ablative Er:YAG 2940 nm laser and topical antifungal (8% ciclopirox olamine) in the treatment of onychomycosis. This was a prospective, randomized, comparative, therapeutic intervention, observer-blind clinical study in which, of 116 patients assessed, 12 (10.3%) had culture-confirmed persistent mycosis and were randomized into two groups — Group 1 (Nd:YAG 1064 nm alone, n=5 patients/8 nails) and Group 2 (Nd:YAG 1064 nm combined with Er:YAG 2940 nm and daily topical 8% ciclopirox olamine, n=7 patients/14 nails) — undergoing four laser sessions 21 days apart, with 18 months of clinical follow-up, assessing mycological cure, clinical cure (OSI score of zero), and OSI score progression over time. All 12 randomized patients completed the study, with no loss to follow-up; there was a statistically significant reduction in OSI score in both groups (Group 1: p=0.006; Group 2: p=0.003), with no significant difference between groups (p=0.194 at baseline; p=0.382 post-treatment); mycological cure occurred in 91.7% of patients (100% of nails in Group 1 and 85.7% in Group 2) and clinical cure in 22.7% of nails evaluated, with no statistically significant difference between groups (p=0.309); only mild, transient adverse events were reported (pain, needle-prick sensation, and warmth sensation), with no need for treatment interruption and no serious adverse events or deaths. It is concluded that both treatment protocols — Nd:YAG 1064 nm alone and its combination with Er:YAG 2940 nm and topical antifungal — proved effective and safe in the treatment of onychomycosis, with a significant reduction in disease severity and high mycological cure rates, with no statistically significant difference between the two approaches.

A onicomicose é uma infecção fúngica ungueal comum, de tratamento desafiador devido ao longo período necessário, à baixa aderência dos pacientes e à alta taxa de recorrência, sendo o tratamento sistêmico o mais eficaz, porém frequentemente contraindicado por efeitos colaterais e interações medicamentosas, enquanto a terapia tópica isolada apresenta baixo índice de cura, o que torna a laserterapia uma alternativa seletiva, não invasiva e segura. O objetivo deste estudo foi comparar a eficácia do laser Nd:YAG 1064 nm isolado versus sua associação com o laser fracionado ablativo Er:YAG 2940 nm e antifúngico tópico (ciclopirox olamina 8%) no tratamento da onicomicose. Trata-se de um estudo clínico prospectivo, randomizado, comparativo, de intervenção terapêutica e observador cego, no qual, de 116 pacientes avaliados, 12 (10,3%) apresentaram micose persistente confirmada por cultura e foram randomizados em dois grupos — Grupo 1 (Nd:YAG 1064 nm isolado, n=5 pacientes/8 unhas) e Grupo 2 (Nd:YAG 1064 nm associado a Er:YAG 2940 nm e ciclopirox olamina 8% tópica diária, n=7 pacientes/14 unhas) —, submetidos a 4 sessões de laser com intervalo de 21 dias e acompanhamento clínico por 18 meses, avaliando-se cura micológica, cura clínica (escore OSI zerado) e evolução do escore OSI ao longo do tempo. Todos os 12 pacientes randomizados completaram o estudo, sem perdas de seguimento; houve redução estatisticamente significante do escore OSI em ambos os grupos (Grupo 1: p=0,006; Grupo 2: p=0,003), sem diferença significante entre os grupos (p=0,194 no início; p=0,382 ao final); a cura micológica ocorreu em 91,7% dos pacientes (100% das unhas no Grupo 1 e 85,7% no Grupo 2) e a cura clínica em 22,7% das unhas avaliadas, sem diferença estatística entre os grupos (p=0,309); foram relatados apenas eventos adversos leves e transitórios (dor, sensação de picada e de aquecimento), sem necessidade de interrupção do tratamento e sem nenhum evento adverso grave ou óbito. Conclui-se que ambos os protocolos de tratamento — Nd:YAG 1064 nm isolado e sua associação com Er:YAG 2940 nm e antifúngico tópico — mostraram-se eficazes e seguros no tratamento da onicomicose, com redução significativa da gravidade da doença e altas taxas de cura micológica, sem diferença estatisticamente significante entre as duas abordagens.
