Usually, the corneal deepithelization, which is one of the steps of refractive surgery called photorefractive keratectomy (PRK) is made circular in shape. In this study, with a randomized manner in each of the 43 patients undergoing simultaneous bilateral PRK, an eye was deepithelized the circular shape and the other in the oval shape, forming two study groups ( eyes submitted to scarification circular and oval eyes submitted to scarification ). For this, were used the circular and oval markers. The oval marker was developed specifically for this study with the following characteristics: greater diameter 9.5 mm, lower diameter 7.5 mm and 5 mm high with a handling rod into the lateral delimiting an internal area 55, 93 mm2 [radius 1 (4.75) x-ray 2 (3.75) x (3,14)]. The circular marker used was the conventional optical marking zone with 9.5 mm diameter, 6.5 mm height and the lateral rod, delimiting an internal area of 70.86 mm2 [radius (4.75) x ( 3.14)]. On the sixth day after surgery (D6) were evaluated three items: the subjective perception, reported by the patient, for the quality of vision compared between the eyes; visual acuity of each eye measured with the Snellen decimal and biomicroscopy evaluation of the cornea, conducted by the medical examiner, on slit lamp, comparing subjectively epithelial stacking. In some patients, where the difference in the appearance of re-epithelialization between the eyes was clearly visible to the examination slit lamp, photographic documentation showing the corneal stacking in the right eye (OD) and left eye was made (OE).

Habitualmente, a desepitelização corneana, que é um dos passos da cirurgia refrativa denominada ceratectomia fotorrefrativa (PRK), é feita de forma circular. Neste estudo, de forma randomizada em cada um dos 43 pacientes submetidos à PRK bilateral simultânea, um olho foi desepitelizado no formato circular e o outro no formato oval, formando 2 grupos de estudos (olhos submetidos à desepitelização circular e olhos submetidos à desepitelização oval). Para tal, foram usados os marcadores circular e oval. O marcador oval foi desenvolvido especificamente para o presente estudo com as seguintes características: maior diâmetro com 9,5 mm, menor diâmetro com 7,5 mm e altura de 5 mm com uma haste de manuseio na lateral, delimitando uma área interna de 55,93 mm2 [raio 1 (4,75) x raio 2 (3,75) x (3,14)]. O marcador circular usado foi o convencional de marcação de zona óptica com 9,5 mm de diâmetro, 6,5 mm de altura e haste na lateral, delimitando uma área interna de 70,86 mm2 [raio (4,75) x (3,14)]. No sexto dia de pós-operatório (D6) foram avaliados três ítens: a percepção subjetiva, informada pelo paciente, quanto à qualidade de visão comparativamente entre os olhos; a acuidade visual de cada olho medida com a tabela de Snellen em decimal e a avaliação biomicroscópica da córnea, realizada pelo médico examinador, em lâmpada de fenda, comparando de forma subjetiva o empilhamento epitelial. Em alguns pacientes, em que a diferença no aspecto da reepitelização entre os olhos foi nitidamente perceptível ao exame na lâmpada de fenda, foi feita documentação fotográfica mostrando o empilhamento corneano em olho direito (OD) e em olho esquerdo (OE).

Volunteers of both gender ; aged between 18 and 50 years; formal indication for bilateral PRK with preoperative visual acuity of 1.0 in both eyes and the difference between the spherical equivalent refractive error of the eye of each less than 1 diopter .

Patients with known or suspected keratoconus ; with cataracts ; people with corneal diseases ; ametropies greater than 8 negative diopters or greater than 4 positive diopters or greater than 4 cylindrical diopters ; people with amblyopia ; people with strabismus, prior eye surgery history.

Improvement of visual acuity in decimal with the Snellen chart in the sixth postoperative day , which ranged between 0.30 and 0.80 in patients undergoing circular scarification technique and from 0.30 to 1.00 in the subject oval technique.

In 43 patients , it was observed by the subjective analysis questioning each patient, 48.8 % of patients did not notice difference between the eyes and 51.2 % noticed a difference ( 41.9 % perceived better quality of vision in the eye of scarification oval and 9.3 % perceived better quality of vision in the eye of the circular scarification ) .
