This is a double-blind, two-arm controlled clinical trial that evaluated weight-bearing capacity in the operated limb following total knee arthroplasty. Participants were randomized by simple randomization after an initial assessment, using an opaque envelope containing the two intervention options. Randomization was performed by the interventionist researcher, who was unaware of the initial assessment data. The evaluator was blinded to this allocation. Control group (12 participants): The intervention in the control group included exercises traditionally used in postoperative rehabilitation following knee arthroplasty, namely: a) isometric quadriceps contraction in the supine position, pressing the popliteal fossa against the stretcher (5 sets of 20 seconds); b) hip flexion SLR to the patient’s full range of motion (3 sets of 10 repetitions); c) hip abduction SLR in the lateral decubitus position (3 sets of 10 repetitions); d) Standing full-range extension (3 sets of 10 repetitions); e) Lateral-lateral weight-shifting training (3 sets of 10 repetitions). No external load was applied; the resistance of gravity was sufficient, since the interventions were performed in the immediate postoperative period. The exception was the standing full extension, in which resistance was applied manually. Experimental group (16 participants): The intervention in the experimental group included exercises based on the PNF concept using a combination of isotonic techniques (except for the exercise identified as “j,” in which rhythmic stabilization was used), as follows: f) Flexion-abduction-external rotation of the contralateral upper limb in the supine position (2 sets of 10 repetitions); g) Flexion-abduction-internal rotation with knee flexion of the contralateral lower limb in the supine position (2 sets of 10 repetitions); h) Flexion-adduction-external rotation with elbow extension of the ipsilateral upper limb in the lateral decubitus position (2 sets of 10 repetitions); i) Trunk extension while seated (2 sets of 10 repetitions); j) Rhythmic stabilization of the upper trunk while standing (2 sets of 5 cycles); Regarding resistance, the PNF Concept recommends the application of “optimal manual resistance,” which is a sufficient amount of resistance to challenge the individual to the extent that they can perform the movement through its full range of motion and with good body alignment. This was the resistance applied in all the proposed exercises.

Trata-se de um estudo clínico controlado de dois braços duplo-cego no qual foi analisada a capacidade de descarga de peso no membro operado após artroplastia total de joelho. Os participantes foram randomizados por meio de sorteio simples, após avaliação inicial, utilizando envelope opaco, no qual constava as duas opções de intervenção. A randomização foi realizada pelo pesquisador intervencionista, que desconhecia os dados da avaliação inicial. O avaliador foi cego para esta alocação. Grupo controle ( 16 participantes):a intervenção no grupo controle incluiu exercícios tradicionalmente utilizados na reabilitação pós operatória de artroplastia de joelho, sendo eles: a) contração isométrica do quadríceps em decúbito dorsal pressionando a fossa poplítea contra a maca (5 séries de 20 segundos); b) SLR de flexão de quadril na amplitude que o paciente for capaz (3 séries de 10 repetições); c) SLR abdução de quadril em decúbito lateral (3 séries de 10 repetições); d) Extensão terminal em pé (3 séries de 10 repetições); e) Treino de descarga de peso látero-lateral (3 séries de 10 repetições).Não houve imposição de carga externa, bastando a resistência da gravidade, visto que as intervenções foram realizadas em um pós-operatório imediato. A exceção foi a extensão terminal em pé, na qual a resistência foi imposta manualmente. Grupo experimental (16 participantes): A intervenção no grupo experimental incluiu exercícios baseados no conceito de PNF, utilizando uma combinação de técnicas isotônicas (exceto para o exercício identificado como “j”, no qual foi utilizada estabilização rítmica), conforme descrito a seguir: f) Flexão-abdução-rotação externa do membro superior contralateral em decúbito dorsal (2 séries de 10 repetições); g) Flexão-abdução-rotação interna com flexão do joelho do membro inferior contralateral em decúbito dorsal (2 séries de 10 repetições); h) Flexão-adução-rotação externa com extensão do cotovelo do membro superior ipsilateral em decúbito lateral (2 séries de 10 repetições); i) Extensão do tronco na posição sentada (2 séries de 10 repetições); j) Estabilização rítmica do tronco superior na posição em pé (2 séries de 5 ciclos);Em relação à resistência, o conceito PNF recomenda a aplicação de uma “resistência manual ideal”, que consiste em uma resistência suficiente para desafiar o indivíduo a ponto de ele conseguir executar o movimento em toda a sua amplitude e com bom alinhamento corporal. Essa foi a resistência aplicada em todos os exercícios propostos.

The characteristics by intervention group allow for comparison between the control (traditional exercises) and experimental (PNF) groups. The gender distribution is balanced in the control group (41.6% female and 58.3% male), while in the experimental group there is a significant predominance of women (91%). This difference in composition should be considered in the interpretation of the results, since anthropometric and hormonal characteristics can influence functional performance and musculoskeletal responses to the intervention. Regarding age, the groups presented similar averages (69.6 years in the control group and 67.9 years in the experimental group). The average height was slightly higher in the control group (1.65 m) compared to the experimental group (1.56 m), which may be associated with the higher proportion of women in the latter. Average weight was similar between the groups (78.9 kg in the control group and 82.1 kg in the experimental group),however, the average BMI was higher in the experimental group (33.95 kg/m2) compared to the control group (28.93 kg/m2). Waist and hip circumference measurements followed the same pattern, showing slightly higher values in the experimental group (114 cm and 107 cm, respectively) than in the control group (105 cm and 106 cm). On the WOMAC questionnaire, the control group scored an average of 53.5 points, while the experimental group scored 57.39 points. On the Tampa scale, the control group scored an average of 48 points while the experimental group scored 43 points. Overall, the descriptive results indicate that the groups have comparable anthropometric and clinical characteristics, with small differences in body composition and sex distribution. These variations should be controlled for or considered in subsequent inferential analysis, especially in the interpretation of the effects of interventions on functional (TUG) and baropodometric (weight bearing) outcomes. Thus, descriptive statistics confirm the adequacy of the sample to the study objective and provide the necessary context for comparison between the applied outcomes.

Thirty-two participants were recruited for the study. Of these, 9 did not meet the inclusion criteria. The remaining 23 volunteers were randomly allocated, through a draw conducted by the study itself, between the control group (12 volunteers) and the experimental group (11 volunteers). In the interval between the initial postoperative evaluation (T1) and the final postoperative evaluation (T2), 2 volunteers, both from the experimental group, stopped participating in the study (reason: hemodynamic complications). And between the final postoperative evaluation (T2) and the follow-up (T3), one volunteer from the control group stopped participating in the study (reason: early discharge). An intention-to-treat analysis of these patients was performed using the multiple imputation method.

Introduction: Knee osteoarthritis is a chronic, progressive disease with high prevalence and increasing incidence, being one of the main causes of disability in the world population. The clinical picture is characterized by pain, stiffness, and consequently, loss of functionality. Due to its progressive and disabling characteristics, some patients progress to surgical treatment, undergoing total knee arthroplasty. Intrinsic consequences of the postoperative period are pain, swelling, and arthrogenic muscle inhibition, resulting in difficulty bearing weight on the operated limb. Weight-bearing is an essential ability for some basic functional activities, such as standing, sitting, and walking. Therefore, it is essential to restore this ability as soon as possible. The physiotherapy approach based on the Proprioceptive Neuromuscular Facilitation (PNF) Concept is a treatment option for improving weight-bearing that may be more effective than the traditional physiotherapy approach due to its philosophy, principles, procedures, and techniques. Objectives: To quantitatively analyze whether or not there is an advantage in using the Proprioceptive Neuromuscular Facilitation (PNF) concept to improve weight-bearing capacity on the operated limb in patients undergoing total knee arthroplasty in the immediate postoperative period (2 and 10 days after surgery). Methods: Participants in this study were individuals between 55 and 85 years old who were about to undergo total knee arthroplasty surgery on one leg. Volunteers were evaluated at four time points (before surgery, immediately before the first intervention, immediately after the last intervention, and 7 to 10 days after the last intervention) and received two sessions of physiotherapy interventions based on traditional exercises or exercises based on the Proprioceptive Neuromuscular Facilitation (PNF) concept on the first and second postoperative days. Results: Twenty-three patients who met the inclusion criteria and had no exclusion criteria participated in the study, and only three were lost to follow-up during the study. Overall, the results indicate that there was a statistically significant difference between the groups for the time variable in the Timed Up and Go test, but not for the weight-bearing variable in baropodometry. Furthermore, both groups improved over time.

Introdução: A osteoartrite de joelho é uma doença crônica progressiva com alta prevalência e crescente incidência, sendo uma das principais causas de incapacidade na população mundial. O quadro clínico é caracterizado por dor, rigidez e consequentemente perda de funcionalidade. Por suas características progressiva e incapacitante, uma parte dos pacientes evolui para o tratamento cirúrgico, sendo submetidos a artroplastia total de joelho. Consequências intrínsecas ao pós-operatório são dor, inchaço e inibição muscular artrogênica, que resultam em dificuldade para descarregar peso sobre o membro operado. A descarga de peso é uma capacidade essencial para algumas atividades funcionais básicas, como levantar, sentar e andar. Portanto, é imprescindível restaurar esta capacidade tão logo seja possível. A abordagem fisioterapêutica baseada no Conceito Facilitação Neuromuscular Proprioceptiva (PNF) é uma opção de tratamento para melhora da descarga de peso que pode ser mais efetiva que a abordagem fisioterapêutica tradicional por conta de sua filosofia, princípios, procedimentos e técnicas. Objetivos: Analisar de forma quantitativa se há vantagem ou não em utilizar o Conceito Facilitação Neuromuscular Proprioceptiva (PNF) para melhorar a capacidade de descarregar peso no membro operado em pacientes submetidos à artroplastia total de joelho no pós-operatório imediato (2 e 10 dias após a cirurgia). Métodos: participaram deste estudo indivíduos entre 55 e 85 anos, que estavam prestes a serem submetidos a cirurgia de artroplastia total de joelho em uma das pernas. Os voluntários foram avaliados em 4 momentos (antes da cirurgia, imediatamente antes da primeira intervenção, imediatamente após a última intervenção, e 7 a 10 dias após a última intervenção, e receberam duas sessões de intervenções fisioterapêuticas baseadas em exercícios tradicionais ou exercícios baseados no conceito facilitação Neuromuscular Proprioceptiva (PNF) no primeiro e segundo dia de pós-cirúrgico. Resultados: Participaram do estudo 23 pacientes que preenchiam os critérios de inclusão e não tinham critérios para exclusão, e apenas 3 perderam segmento ao longo do estudo. De modo geral, os resultados indicam que houve diferença estatisticamente significativa entre os grupos para a variável tempo no Timed Up and Go test, mas não para a variável descarga de peso na baropodometria. Além disso, ambos os grupos melhoraram ao longo do tempo.
