This is a two-arm open-label randomized controlled trial with a total of 80 participants allocated by simple randomization using computer-generated software with the allocation sequence generated in advance and recorded in a numbered list the Intervention Group with 40 participants will receive brachial plexus block via the supraclavicular or axillary approach guided by ultrasound the Control Group with 40 participants will receive local anesthesia by subcutaneous infiltration along the incision the clinical and ultrasonographic assessments including preoperative mapping post-block measurements and postoperative follow-up will be performed by the principal investigator the diameter of the superficial veins of the upper limbs will be measured by ultrasonography in the preoperative period before and after complete onset of the anesthetic effect and participants will be followed for sixty days to assess arteriovenous fistula patency.

Trata-se de ensaio clínico randomizado controlado de dois braços aberto com um total de 80 participantes alocados por randomização simples por meio de programa computadorizado com sequência gerada previamente e registrada em lista numerada Grupo Intervenção com 40 participantes receberá bloqueio anestésico do plexo braquial por via supraclavicular ou axilar guiado por ultrassom Grupo Controle com 40 participantes receberá anestesia local por infiltração subcutânea ao longo da incisão as avaliações clínicas e ultrassonográficas incluindo mapeamento pré-operatório mensurações pós-bloqueio e seguimentos pós-operatórios serão realizadas pelo pesquisador principal o diâmetro das veias superficiais dos membros superiores será mensurado por ultrassonografia no período pré-operatório antes e após a instalação completa do efeito anestésico e os participantes serão acompanhados por sessenta dias para avaliação da perviedade da fístula arteriovenosa.

Age 18 years or older; clinical indication for autologous arteriovenous fistula creation for hemodialysis, in radiocephalic, brachiocephalic or brachiobasilic configuration; agreement to participate with signed informed consent form

Exclusion criteria:

Known allergy to local anesthetics; active infection at the brachial plexus block puncture site; severe coagulopathy; preexisting upper limb neuropathy; inability to attend regular outpatient follow-up

To assess the diameter of the superficial vein cephalic or basilic according to the site planned for the anastomosis by Doppler ultrasound in the preoperative period after onset of the anesthetic block and before the surgical incision.

To assess the functional patency of the arteriovenous fistula at sixty days, as a hypothesis-generating secondary outcome, defined by the criteria of Beathard et al 2018: venous diameter greater than or equal to 6 mm, flow greater than or equal to 500 mL per minute, depth less than or equal to 6 mm from the skin, palpable thrill and absence of stenoses greater than 50 percent with results reported with 95 percent confidence interval.

Avaliar a perviedade funcional da fístula arteriovenosa aos 60 dias, como desfecho secundário gerador de hipóteses, definida pelos critérios de Beathard et al 2018: diâmetro venoso maior ou igual a 6 mm, fluxo maior ou igual a 500 mL por minuto, profundidade menor ou igual a 6 mm da pele, frêmito palpável e ausência de estenoses maiores que 50 por cento com os resultados reportados com intervalo de confiança de 95 por cento.
