Interventions

  The startup Oya Care will engage 1000 patients with ovaries and aged over 18 years, irrespective of social group, skin color/race and ethnic origin, sexual orientation, gender identity, and social status, in the resolution of common gynecological disorders and reproductive and fertility issues. The patients will not be subjected to any interventions; they will only be followed up on an individual basis. By means of telemedicine and virtual follow-up, Oya Care physicians and nurses will collaborate to offer patients access to functionalities, contents, and services, in Portuguese. Telemedicine, phone calls, text messages, and a digital platform will be used to assist the patients, to refer them to additional services, to establish diagnosis and conduct, to verify resolution of the complaint, to redirect the conduct, and to verify patient satisfaction. Selected standardized questionnaires will allow information to be collected for screening, diagnosis, and patient follow-up. Lab tests that are essential for diagnosis and selected on the basis of scientific literature and consensus will be employed. Patient care will start the moment the medical appointment is scheduled via the platform. To ensure that the medical appointment is comfortable and productive, before the appointment the patient will fill in a form about their family, medical, and gynecological history that is available on the digital platform, and the physician will have access to this information. During the medical appointment, the physician will explore the most relevant points on the basis of the information previously provided by the patient and will fill in the electronic medical chart with additional information. During patient screening, patient education and systematic data collection will be established. Routine data analysis is an asset for improving this methodology, which will use standardized questionnaires based on the PROMs (Patient Reported Outcome Measures) approach. The time elapsed between follow-up appointments will be individualized and will follow medical guidance. Lab tests that are essential for the diagnosis will be collected in the patient’s home and tested by partner laboratories. Results will be available on the digital platform within 5 working days. After the medical appointment, a medical report that is easy for the patient to read and which will contain all the information regarding their care will be issued and sent to the patient. Finally, via WhatsApp, the patient will receive questions to verify their satisfaction. Answering the questions is optional, and the answers are anonymous. The data will be analyzed by comparing the parameters of the patients that sought healthcare for the purposes of contraception/fertility and the patients that sought healthcare for common gynecological disorders. Subgroups will also be analyzed. For example, in the group of patients that sought healthcare for common gynecological disorders, we will evaluate the percentage of each diagnosis (e.g., vulvovaginitis, urinary tract infection, etc) among the patients, the percentage of patients that were contacted by nurses after the medical appointment, the percentage of patients that reported regression of symptoms, the percentage of patients that answered the patient satisfaction questionnaire, and the percentage of patients that stated they were happy with the Oya Care protocol. The descriptive data will be expressed as the mean and standard deviation; the frequency of events will be measured as percentage

  A startup Oya Care engajará 1000 pacientes com ovários e acima de 18 anos de idade, pertencentes a qualquer grupo social, cor/raça, etnia, orientação sexual, identidade de gênero e classe, na resolução de doenças ginecológicas comuns e questões de reprodução e fertilidade. Não serão realizadas intervenções e sim acompanhamento individual da(o) paciente. Via atendimento remoto e acompanhamento à distância, médicas(os) e enfermeiras(os) da Oya Care colaborarão para fornecer a pacientes acesso a funcionalidades, conteúdos e serviços, em português. Telemedicina, telefonemas, mensagens de texto e a plataforma digital serão utilizados para atender pacientes, encaminhá-las(los) para serviços adicionais, estabelecer diagnóstico e conduta, verificar a resolução da queixa referida, redirecionar a conduta e verificar a satisfação da(o) paciente. Questionários padronizados criteriosamente selecionados possibilitarão a coleta de informações para a triagem, o diagnóstico e o acompanhamento da(o) paciente. Exames laboratoriais essenciais ao diagnóstico e selecionados com base em literatura científica e consensos serão utilizados. O cuidado da(o) paciente se iniciará no momento do agendamento virtual. Para se garantir que a consulta seja confortável e produtiva, antes do atendimento a(o) paciente preencherá seus dados pessoais e informações sobre antecedentes familiares, médicos e ginecológicos na plataforma digital, e a(o) médica(o) terá acesso a essas informações antes da consulta. No momento da consulta, a(o) médica(o) explorará os pontos mais relevantes com base nas informações previamente fornecidas pela(o) paciente e preencherá o prontuário eletrônico com informações adicionais. Durante a triagem, o processo educativo e a coleta sistemática de informações serão estabelecidos. A análise rotineira de dados será um dos diferenciais para se aperfeiçoar esta metodologia, que utilizará questionários padronizados baseados na abordagem PROMs (Monitoramento do Desfecho Clínico Centrado no Paciente). O intervalo para os contatos pós-consulta serão individualizados e seguirão orientação médica. Exames laboratoriais essenciais ao diagnóstico serão colhidos em ambiente domiciliar e realizados por laboratório parceiro. Os resultados serão disponibilizados em até 5 dias úteis na própria plataforma digital. Após a consulta, um relatório com linguagem acessível contendo informações sobre o atendimento e orientações será emitido e enviado à(ao) paciente. Por fim, via WhatsApp e e-mail, a(o) paciente receberá perguntas para verificar sua satisfação, que poderão ser respondidas de forma anônima e serão opcional. Os dados serão analisados comparando-se os parâmetros de pacientes que procuraram atendimento para fins de reprodução/fertilidade com os parâmetros de pacientes que procuraram atendimento para fins de resolução de doenças ginecológicas comuns. Também serão analisadas subamostras. Por exemplo, na subamostra referente a doenças ginecológicas comuns, avaliaremos qual a porcentagem de cada diagnóstico (por exemplo, vulvovaginite, infecção do trato urinário, etc) entre as(os) pacientes, a porcentagem de pacientes que foi contactada pelas(os) enfermeiras(os) após a consulta, a porcentagem de pacientes que relatou regressão de sintomas, a porcentagem de pacientes que respondeu à pesquisa de satisfação e destes, a porcentagem que se declarou satisfeita com o protocolo Oya Care.
