This is a randomized, controlled, seven-arm parallel-group clinical trial with masking of participants and outcome assessors. A total of 105 participants will be included and randomly allocated to seven groups, with 15 participants in each group. Randomization will be performed using a previously established drawing procedure, and the allocation sequence will be maintained by a researcher who is not involved in outcome assessment. Participants will not be informed of the intervention to which they have been allocated, and the assessors responsible for outcome collection will remain blinded to participants’ group allocation. The researcher responsible for administering the stimulation may be aware of the allocation in order to ensure correct electrode placement and device programming but will not participate in outcome assessments. The experimental protocol will be performed at three time points: baseline, immediately after the intervention, and 30 minutes after the intervention. At each time point, muscle electrical activity will be assessed by surface electromyography, muscle strength by isometric dynamometry, and pressure pain threshold by algometry. Participants will remain comfortably seated in an armchair with cervical support in a room maintained at approximately 22 degrees Celsius. Muscle electrical activity will be assessed using surface electromyography with two bipolar electrodes positioned over the muscle belly of the right first dorsal interosseous muscle. Electromyographic signals will be amplified and filtered using a 10-millisecond time constant, a 20 to 500 hertz band-pass filter, and a 60 hertz notch filter. The sampling rate will be 2 kilohertz, with signal transmission to a computer and processing using specific software for data acquisition. Muscle strength will be assessed using manual isometric dynamometry. The participant will be instructed, using a previously recorded verbal command, to perform maximal force during abduction of the second digit of the right hand against the dynamometry device held by the researcher. Three maximal isometric contractions will be performed, each lasting six seconds. For analysis, the highest force value obtained during the third or fourth second of the contraction will be considered. Muscle strength assessment will be performed simultaneously with the acquisition of muscle electrical activity. Pressure pain threshold will be assessed using algometry over the muscle belly of the right first dorsal interosseous muscle. The algometer will be positioned perpendicular to a previously determined and marked point on the participant's skin, and pressure will be progressively increased. The participant will be instructed to say “stop” when the sensation of pressure becomes painful. At that moment, pressure application will be immediately discontinued, and the recorded value will be considered the pressure pain threshold. After the baseline assessment, participants will undergo the electrical stimulation protocol corresponding to the group to which they were randomized. The six active intervention groups will receive stimulation for 20 minutes at an intensity of 1 milliampere, using electrodes with an area of 35 square centimeters. The electrode configurations will consist of anodal stimulation of the primary motor cortex and cathodal stimulation of the contralateral brachial plexus in the supraclavicular region; cathodal stimulation of the primary motor cortex and anodal stimulation of the contralateral brachial plexus in the supraclavicular region; anodal stimulation of the primary motor cortex and cathodal stimulation of the contralateral supraorbital region, corresponding to the FP2 position of the international 10-20 system; cathodal stimulation of the primary motor cortex and anodal stimulation of the contralateral supraorbital region, corresponding to the FP2 position of the international 10-20 system; anodal stimulation of the brachial plexus and cathodal stimulation of the contralateral olecranon region; and cathodal stimulation of the brachial plexus and anodal stimulation of the contralateral olecranon region. The seventh group will undergo the sham condition, using electrodes with an area of 35 square centimeters positioned over the same anatomical regions used in the configuration involving the left primary motor cortex and the right supraorbital region, corresponding to the FP2 position of the international 10-20 system, with a sham stimulation protocol and a total duration of 20 minutes. After completion of the intervention, the assessments will be repeated immediately and 30 minutes after stimulation, following the same procedures performed at baseline. At the end of the intervention, participants will be asked which intervention they believe they received, with the purpose of assessing the effectiveness of masking.

Trata-se de um ensaio clínico randomizado, controlado, de sete braços paralelos, com mascaramento dos participantes e dos avaliadores dos desfechos. Serão incluídos 105 participantes, distribuídos aleatoriamente em sete grupos, com 15 participantes em cada grupo. A randomização será realizada por meio de sorteio previamente estabelecido, e a sequência de alocação será mantida sob responsabilidade de pesquisador não envolvido na avaliação dos desfechos. Os participantes não serão informados sobre a intervenção para a qual foram alocados e os avaliadores responsáveis pela coleta dos desfechos permanecerão cegos quanto à alocação dos participantes. O pesquisador responsável pela aplicação da estimulação poderá ter conhecimento da alocação para possibilitar o correto posicionamento dos eletrodos e a programação do equipamento, não participando, entretanto, das avaliações dos desfechos. O protocolo experimental será realizado nos momentos linha de base, imediatamente após a intervenção e 30 minutos após a intervenção. Em cada momento serão avaliadas a atividade elétrica muscular por meio de eletromiografia de superfície, a força muscular por meio de dinamometria isométrica e o limiar de dor à pressão por meio de algometria. Os participantes permanecerão sentados confortavelmente em uma poltrona, utilizando apoio cervical, em sala com temperatura controlada em aproximadamente 22 graus Celsius. A atividade elétrica muscular será avaliada por meio de eletromiografia de superfície, utilizando dois eletrodos bipolares posicionados sobre o ventre muscular do primeiro músculo interósseo dorsal da mão direita. Os sinais eletromiográficos serão amplificados e filtrados com constante de tempo de 10 milissegundos, filtro passa-banda de 20 a 500 hertz e filtro notch de 60 hertz. A taxa de aquisição será de 2 quilohertz, com transmissão dos sinais para computador e processamento por software específico para aquisição dos dados. A força muscular será avaliada por meio de dinamometria isométrica manual. O participante será orientado, mediante comando verbal previamente gravado, a realizar força máxima durante o movimento de abdução do segundo quirodáctilo da mão direita contra o equipamento de dinamometria mantido pelo pesquisador. Serão realizadas três contrações isométricas máximas, com duração de seis segundos cada. Para análise, será considerado o maior valor de força obtido durante o terceiro ou quarto segundo da contração. A coleta da força muscular será realizada simultaneamente à aquisição da atividade elétrica muscular. O limiar de dor à pressão será avaliado por meio de algometria no ventre muscular do primeiro músculo interósseo dorsal da mão direita. O algômetro será posicionado perpendicularmente ao ponto previamente determinado e marcado na pele do participante, sendo aplicada pressão de forma progressiva. O participante será orientado a dizer “pare” quando a sensação de pressão se tornar dolorosa. Nesse momento, a aplicação da pressão será interrompida e o valor registrado será considerado o limiar de dor à pressão. Após a avaliação de linha de base, os participantes serão submetidos ao protocolo de estimulação elétrica correspondente ao grupo para o qual foram randomizados. Os seis grupos de intervenção ativa receberão estimulação durante 20 minutos, com intensidade de 1 miliampere e eletrodos com área de 35 centímetros quadrados. As configurações dos eletrodos consistirão na estimulação anódica do córtex motor primário e estimulação catódica do plexo braquial contralateral, na região supraclavicular; estimulação catódica do córtex motor primário e estimulação anódica do plexo braquial contralateral, na região supraclavicular; estimulação anódica do córtex motor primário e estimulação catódica da região supraorbital contralateral, correspondente à posição FP2 do sistema internacional 10-20; estimulação catódica do córtex motor primário e estimulação anódica da região supraorbital contralateral, correspondente à posição FP2 do sistema internacional 10-20; estimulação anódica do plexo braquial e estimulação catódica na região do olécrano contralateral; e estimulação catódica do plexo braquial e estimulação anódica na região do olécrano contralateral. O sétimo grupo será submetido à condição simulada, utilizando eletrodos com área de 35 centímetros quadrados posicionados nas mesmas regiões anatômicas utilizadas na configuração do córtex motor primário esquerdo e da região supraorbital direita, correspondente à posição FP2 do sistema internacional 10-20, com protocolo de estimulação simulada e duração total de 20 minutos. Após o término da intervenção, as avaliações serão repetidas imediatamente e 30 minutos após a estimulação, seguindo os mesmos procedimentos realizados na avaliação de linha de base. Ao final da intervenção, será perguntado ao participante qual intervenção ele acredita ter recebido, com o objetivo de avaliar a eficácia do mascaramento.
