This is a randomized and controlled clinical trial. We included 30 infants (28 days of age to 24 months of age) on invasive mechanical ventilation for a period greater than or equal to 12 hours through an orotracheal cannula, diagnosed with atelectasis by a pediatric intensive care physician through clinical and imaging examination (radiography of chest and lung ultrasound).Patients who met the inclusion criteria were electronically randomized (https://www.random.org/lists/) into two groups, namely: control group (CG) and intervention group (IG). The control group consisted of 15 infants undergoing routine respiratory physiotherapy at the hospital, while the intervention group, consisting of 15 infants, underwent the Structured Respiratory Physiotherapy Protocol (PEFR) developed for this study (postural drainage, mechanical chest vibration, hyperinflation manual with self-inflating bag, stretching of the accessory respiratory muscles and functional positioning). In both groups, the respiratory pattern (Wood Downes Score) and the lung ultrasound image (Lung Ultrasound Score) were evaluated before and after physiotherapy care. Pulmonary ultrasound images were recorded for analysis by an evaluator (a pediatric intensive care physician certified to perform ultrasound for more than eight years, with more than 150 exams performed in this period) blind to the groups and patient history

Trata-se de um ensaio clínico randomizado e controlado. Foram incluídos 30 lactentes (28 dias de vida a 24 meses de vida) em ventilação mecânica invasiva por período maior ou igual a 12 horas através de cânula orotraqueal, diagnosticadas com atelectasia por um médico intensivista pediátrico através de exame clínico e de imagem (radiografia de tórax e ultrassonografia pulmonar). Os pacientes que preencheram os critérios de inclusão foram randomizados eletronicamente (https://www.random.org/lists/) em dois grupos, sendo eles: grupo controle (GC) e grupo intervenção (GI). O grupo controle foi composto por 15 lactentes submetidos a fisioterapia respiratória de rotina do hospital, enquanto o grupo intervenção, composto por 15 lactentes foi submetido ao Protocolo Estruturado de Fisioterapia Respiratória (PEFR) desenvolvido para este estudo (drenagem postural, vibração torácica mecânica, hiperinsuflação manual com bolsa auto inflável, alongamentos da musculatura respiratória acessória e posicionamento funcional). Em ambos os grupos foi avaliado o padrão respiratório (Escore de Wood Downes) e a imagem da ultrassonografia pulmonar (Lung Ultrasound Score) antes e após o atendimento da fisioterapia. As imagens da ultrassonografia pulomonar foram gravadas para que fossem analisadas por um avaliador (médico intensivista pediátrico com certificação para realizar ultrassonografia há mais de oito anos, com mais de 150 exames realizados neste período) cego para os grupos e história dos pacientes

Patients with bilateral atelectasis; any type of air leak syndrome (such as: subcutaneous emphysema, pneumothorax, pneumatocele, pneumomediastinum); pulmonary hemorrhage; diseases that cause bone fragility (costal osteoporosis and osteomyelitis); chest and/or lung bruises; subcutaneous pacemaker; treatment with anticoagulants for more than 72 continuous hours; hemodynamic instability; thrombocytopenia (<50,000 platelets); using a chest tube; with underlying neuromuscular or cardiac diseases and spinal deformities (kyphoscoliosis)

Outcome found 1: Significant differences were observed when comparing CG and GI before versus after intragroup respiratory physiotherapy. The median (IQ25-75%) age of patients in the control group (CG) was 7 (2-17) months and in the intervention group (IG) it was 4 (2 -13.5) months, in the CG it was 6 female patients and 9 male patients and in the GI 4 female patients and 11 male patients. The average weight of patients in the CG was 7.7 kg, while in the GI it was 7.3 kg. In both groups, 73.3% of patients were diagnosed with acute viral bronchiolitis.

During the period studied, 845 children were admitted to the pediatric ICU, 224 of which underwent orotracheal intubation and 57 developed pulmonary atelectasis. Of these children, 40 met the inclusion criteria due to having atelectasis, with 10 patients being excluded (reasons: one with scoliosis, two with pneumothorax, three with hemodynamic instability and four with thrombocytopenia), with 30 children being eligible (see Figure 5). After selecting eligible patients, it was not necessary to interrupt interventions in any of the groups.

No período estudado, foram internadas 845 crianças na UTI pediátrica, sendo que 224 foram submetidas à intubação orotraqueal e 57 desenvolveram atelectasias pulmonares. Destas crianças, 40 preencheram os critérios de inclusão devido a apresentarem atelectasia, sendo 10 pacientes excluídos (motivos: um com escoliose, dois com pneumotórax, três com instabilidade hemodinâmica e quatro com plaquetopenia), sendo elegíveis 30 crianças (vide Figura 5). Após a seleção dos pacientes elegíveis, não foi necessário interromper as intervenções em nenhum dos grupos.

No patient presented unplanned extubation, displacement of the orotracheal cannula, tissue trauma, air leak syndrome (pneumothorax or pneumomediastinum) or need to increase ventilation parameters after application of respiratory physiotherapy interventions in both groups.

The result of the intervention was evaluated by the magnitude of the effect using the Hedges'g test (interpretation: small (0.2 < Hedges' g < 0.5), moderate (0.5 < Hedges' g < 0.8) and large effect (Hedges' g > 0.8)). A moderate effect was observed in reducing the Lung Ultrasound Score in the control group after respiratory physiotherapy (Hedges' g = -0.64, 95% CI: -1.35 to 0.08), and an effect 2.9 times higher in the intervention group (Hedges' g = -1.88, 95% CI: -1.01 to -2.73), characterizing a large effect in reducing the score. We observed a large effect in reducing the Wood Downes score in the control group after physiotherapy (Hedges' g = -1.53, 95% CI: -3.1 to -1.29), and a 1.4 times greater effect in the intervention group (Hedges' g = -2.2, 95% CI: -2.32 to -0.71).

Introduction: The developing lungs are predisposed to collapse. Children are at greater risk of atelectasis due to their anatomophysiological particularities. Interventions used to treat pulmonary atelectasis in intubated children are limited and include respiratory physiotherapy. Several respiratory physiotherapy techniques and interventions are routinely used to treat atelectasis, but there are only four studies that cite different respiratory physiotherapy methods aimed at resolving atelectasis in pediatric patients on invasive mechanical ventilation (IMV). Objectives: To evaluate the application of a Structured Respiratory Physiotherapy Protocol (SRPP) for lower airway clearance and lung reexpansion for infants on IMV with a clinical and imaging diagnosis of unilateral pulmonary atelectasis. Methods: This is a randomized and controlled clinical trial, including 30 infants with a mean age of 8.9 + 8.0 months; 7.5 + 3.0 kg; BMI 15.8 + 1.6 kg/cm2 and IMV time 7.7 + 4.3 days. The sample was randomized into a control group (CG), which underwent routine respiratory physiotherapy and an intervention group (IG), which underwent PEFR (postural drainage, mechanical chest vibration, manual hyperinflation with a self-inflating bag, stretching of the accessory respiratory muscles and functional positioning) . Both groups were evaluated immediately before and after respiratory physiotherapy regarding respiratory effort using the Wood Downes score (WD) and lung aeration using lung ultrasound (images classified using the Lung Ultrasound Score - LUS). The result of the intervention was evaluated by the magnitude of the effect using the Hedges'g test (interpretation: small (0.2 < Hedges' g < 0.5), moderate (0.5 < Hedges' g < 0.8) and large effect (Hedges' g > 0.8)); being: WD with an important effect on reducing the CG score after physiotherapy (Hedges’ g = -1.53) and a greater effect on the GI (Hedges’ g = -2.2). Results: Regarding LUS, there was a moderate effect on reducing the score in the CG after physiotherapy (Hedges’ g = -0.64) and a much greater effect in the GI (Hedges’ g = -1.88). Conclusion: The application of PEFR resulted in a significant reduction in the degree of respiratory distress and pulmonary atelectasis compared to the CG in this study.

As intervenções utilizadas no tratamento da atelectasia pulmonar em crianças intubadas são limitadas e incluem fisioterapia respiratória. Diversas técnicas e intervenções de fisioterapia respiratória são utilizadas de rotina para o tratamento das atelectasias, mas existem apenas quatro estudos que citam métodos distintos de fisioterapia respiratória visando a resolução das atelectasias em pacientes pediátricos em ventilação mecânica invasiva (VMI). Objetivos: Avaliar a aplicação de um Protocolo Estruturado de Fisioterapia Respiratória (PEFR) para desobstrução de vias aéreas inferiores e reexpansão pulmonar para lactentes em VMI com diagnóstico clínico e de imagem de atelectasia pulmonar unilateral. Métodos: Trata-se de um ensaio clínico randomizado e controlado, incluindo 30 lactentes com média idade de 8,9 + 8,0 meses; 7,5 + 3,0 kg; IMC 15,8 + 1,6 Kg/cm2 e tempo de VMI 7,7 + 4,3 dias. A amostra foi randomizada em grupo controle (GC), submetido fisioterapia respiratória de rotina e grupo intervenção (GI), submetido ao PEFR (drenagem postural, vibração torácica mecânica, hiperinsuflação manual com bolsa auto inflável, alongamentos da musculatura respiratória acessória e posicionamento funcional). Ambos os grupos foram avaliados imediatamente antes e após a fisioterapia respiratória quanto ao esforço respiratório pelo escore Wood Downes (WD) e quanto a aeração pulmonar pela ultrassonografia pulmonar (imagens classificadas pelo Lung Ultrasound Score - LUS). O resultado da intervenção foi avaliado pela magnitude do efeito pelo teste Hedges’g (interpretação: efeito pequeno (0,2 < Hedges’ g < 0,5), moderado ( 0,5 < Hedges’ g < 0,8) e grande (Hedges’ g > 0,8)); sendo: WD com um efeito importante na redução do escore no GC após a fisioterapia (Hedges’ g = -1,53) e um efeito maior no GI (Hedges’ g = -2,2). Resultados: Quanto ao LUS houve efeito moderado na redução do escore no GC após a fisioterapia (Hedges’ g = -0,64) e um efeito muito maior no GI (Hedges’ g = -1,88). Conclusão: A aplicação do PEFR resultou em uma redução significativa no grau de desconforto respiratório e atelectasia pulmonar comparativamente ao GC neste estudo.
