This is a prospective, double-blind, split-mouth and randomized study. The Department of Surgery and Traumatology of the State University of Maringá, Brazil, conducted all surgical procedures from July 2023 to April 2024 in 20 patients. Patients, surgeons, and observers did not know the drug used in the procedures. The Research Randomizer, version 4.0 (available from http://www.randomizer.org), randomized the side of the first approach and the drug for application. One of the researchers (G.M.F.) conducted randomization, informing the surgical site and delivering the drug to the patient at the time of surgery. Patients took the capsule containing Valeriana or Passiflora orally 60 minutes before the procedure. They also received a preemptive single dose of 4mg dexamethasone orally to prevent hyperalgesia and control postoperative edema. The surgical procedure started with intra- and extraoral antisepsis with an aqueous solution of 0.12% and 2% chlorhexidine digluconate (Dermatológica Ltd, Maringá, PR, Brazil), respectively. Local anesthesia was performed in the lower alveolar, buccal, and lingual nerve with a maximum volume of 5.4 ml (equivalent to three tubes) of anesthetic solution containing 2% mepivacaine and 1:100,000 epinephrine. A single surgeon with extensive experience in this intervention conducted all mandibular third molar procedures, consistently following the same protocol. Relaxing incisions were made in the mesiolingual region of the second molar, followed by a conventional incision for third molar extraction, using a #15 scalpel blade. After lifting the soft tissue to expose the surgical cavity, osteotomy and odontosection were performed with a #702 carbide bur mounted on a high-rotation (350,000 rpm) handpiece under constant irrigation with 0.9% sterile saline solution (Eurofarma, São Paulo, Brazil). A sterile Seldin elevator aided the extraction, followed by careful curettage, bone leveling, and surgical site cleaning with abundant saline irrigation. The incision was sutured with interrupted stitches using a 4-0 silk suture. After surgery, the patients received written instructions about local hemostatic measures, diet, surgical area hygiene, physical activity restrictions, and other routine recommendations for this intervention. A 0.12% chlorhexidine digluconate mouthwash was recommended as an antiseptic agent after tooth extraction every 12 hours for seven days. The patients also received 12 500mg dipyrone pills to take every six hours in case of pain. The sutures were removed on the seventh postoperative day. A minimum interval between the first and second interventions (observation period) was 15 days, as recommended12. The second surgery was scheduled for the same weekday and around the same time to prevent chronopharmacological variations

Trata-se de um estudo clínico prospectivo, duplo-cego, boca dividida e randomizado realizado em 20 pacientes. Todos os procedimentos cirúrgicos foram conduzidos no Departamento de Cirurgia e Traumatologia da Universidade Estadual de Maringá, Brasil, de Julho 2023 a Abril 2024. Os pacientes, operador e observador não tinham conhecimento da droga utilizada durante os procedimentos. O lado a ser primeiramente abordado e a droga a ser utilizada foram randomizadas utilizando o Research Randomizer, versão 4.0 (disponível em: https://www. ranomizer.org). Um dos pesquisadores (G.M.F) foi responsável por conduzir a randomização, informando o sítio cirúrgico, e entregando a droga ao paciente na hora da cirurgia. Foi entregue aos pacientes a cápsula contendo Valeriana ou Passiflora, via oral, 60 minutos antes do procedimento. Para prevenir hiperalgesia e controlar edema pós-operatório, os pacientes também receberam uma única dose de Dexametasona 4mg, via oral, de forma preemptiva. No início do procedimento cirúrgico, antissepsia intrabucal e extrabucal foi realizada com solução aquosa de digluconato de clorexidina 0.12 e 2% (Dermatológica Ltd, Maringá, Brasil), respectivamente. Anestesia local do nervo alveolar inferior, bucal e lingual foi alcançada com o volume máximo de 5.4ml (equivalente a 3 tubetes) de solução anestésica contendo mepivacaína 2% com epinefrina 1:100,000. Todos os procedimentos de terceiros molares mandibulares foram conduzidos por um único cirurgião de ampla experiência com esse tipo de intervenção e sempre seguindo o mesmo protocolo. Incisões relaxantes na região mesiolingual do segundo molar, seguida por uma incisão convencional para extração de terceiros molares foram realizadas com lâmina de bisturi No. 15. Após o tecido mole ter sido levantado para exposição da loja cirúrgica, osteotomia e odontossecção foram realizadas com a broca carbide 702 montada na peça de mão de alta rotação (350,000 rpm), sob constante irrigação com solução salina estéril 0.9% (Eurofarma, São Paulo, Brasil). A extração foi realizada com o auxílio de um elevador Seldin estéril, seguido pela cuidadosa curetagem, regularização óssea e limpeza do sítio cirúrgico com abundante irrigação sorológica. A incisão foi suturada com pontos descontínuos utilizando fio seda 4-0 ome. Após a cirurgia, os pacientes receberam instruções escritas acerca de medidas hemostáticas locais, alimentação, higienização da área cirúrgica, restrições de atividades físicas e outras recomendações rotineiras para este tipo de intervenção. Para o dia seguinte a extração dentária, foi recomendado bochecho com digluconato de clorexidina 0,12% a cada 12 horas por 7 dias, como agente antisséptico. Os pacientes também receberam 12 pílulas de dipirona 500mg para tomar a cada 6 horas em caso de dor. As suturas foram removidas no sétimo dia pós-operatório. O intervalo mínimo entre a primeira e a segunda intervenção (período de observação) foi de 15 dias, como recomendado previamente. Na segunda cirurgia, os pacientes foram agendados para o mesmo dia da semana e aproximadamente no mesmo horário, com o objetivo de prevenir variações cronofarmacológicas

Found Outcome 1: The consistency of physiological outcomes does not suggest differences between the phytotherapeutic drugs, apparently confirmed by the information on the self-assessment reports, in which eight patients (40%) preferred Passiflora, seven (35%) favored Valeriana, and five (25%) claimed no difference between both. However, these findings do not necessarily indicate that Valeriana inefficiently provided comfort to anxious patients during surgery. This difference more likely indicates each patient's subjectivity, as there were no significant mean differences. Sedation levels did not show statistically significant differences between Passiflora incarnata and Valeriana officinalis drugs.

The oxygen saturation was within normal limits in most cases (97.75% and 97.88%), and no statistically significant difference was observed between the 2 drugs (p < .05). The respiratory rate did not show significant effects for the statistical analysis (p < .05). Regarding the heart rate, there were differences in means between the medications at times T1 and T3 (p-value: 0.0420), T3 and T5 (p-value: < 0.001), T3 and T6 (p-value: < 0.001), T6 and T9 (p-value: < 0.001). Regarding blood pressure, both systolic and diastolic, there was no statistically significant difference. Regarding the level of sedation, recorded by the researcher at the end of the surgery, with the use of passionflower, 2 patients were classified as anxious during the surgery (10%), 14 patients were classified as cooperative (70%), and 4 as drowsy (20%). Similarly, with the use of Valerian, 2 patients were classified as anxious (10%), 15 as cooperative (75%), and only 3 patients were classified as drowsy (15%). There were no statistically significant differences regarding the level of sedation between the medications Passiflora incarnata and Valeriana officinalis.

The objective of the present research is to compare the effect of Passiflora incarnata with that of Valeriana officinalis L in controlling anxiety in patients undergoing extraction of impacted lower third molars, through a clinical trial that is controlled, randomized, double-blind, and crossover, conducted with 20 volunteer patients from the Dentistry Department of the State University of Maringá, from September 2023 to August 2024. The patients underwent extractions of impacted lower third molars. The same patient should have dental elements with identical extraction difficulties. One side was operated on after the patient was medicated with Passiflora incarnata one hour before the procedure, and the other side, at another surgical moment, with the patient medicated with Valeriana. To compare the control of anxiety provided by the two herbal remedies, measurements of heart rate, respiratory rate, and saturation were conducted. In order to compare the anxiety control provided by the two herbal remedies, measurements of heart rate, respiratory rate, oxygen saturation, and blood pressure of the patients were taken before, during, and after the surgical procedure, and at the end, the patients themselves were subjected to a questionnaire reporting their experiences. The data obtained were tabulated and statistically analyzed (Two-Factor Analysis of Variance with Repeated Measures (ANOVA-2-FACTOR-MR)), where it was observed that there was no significant difference between the medications (p < .05), and the only variable vital sign in this comparison was heart rate, in which there were mean differences among the times T1 and T3 (p-value: 0.0420), T3 and T5 (p-value: < 0.001), T3 and T6 (p-value: < 0.001), T6 and T9 (p-value: < 0.001); however, such changes do not refer to the effects of the drugs, being independent of the group (Passiflora and Valerian). O objetivo da presente pesquisa é comparar o efeito da Passiflora incarnata com o da Valeriana officinalis L no controle da ansiedade em pacientes submetidos a exodontias de terceiros molares mandibulares inclusos, por meio de um ensaio clínico, controlado, randomizado, duplo-cego, cruzado, realizado com 20 pacientes voluntários do Departamento de Odontologia da Universidade Estadual de Maringá, no período de Setembro de 2023 a Agosto de 2024. Os pacientes foram submetidos exodontias de terceiros molares inclusos mandibulares. O mesmo paciente deveria possuir elementos dentários com idênticas dificuldades de extração. Um dos lados foi operado após o paciente ser medicado com passiflora incarnata uma hora antes do procedimento, e o outro lado, em outro momento cirúrgico, com o paciente medicado com Valeriana. Com o objetivo de comparar o controle da ansiedade que os dois fitoterápicos proporcionaram, foram realizadas medições de frequência cardíaca, frequência respiratória, saturação de oxigênio e pressão arterial dos pacientes antes, durante e após o procedimento cirúrgico e ao final o próprio paciente foi submetido a um questionário relatando suas experiências. Os dados obtidos foram tabulados e analisados estatisticamente (Variância de Dois Fatores com Medidas Repetidas (ANOVA-2-FATORES-MR)), onde observou-se não haver diferença significativa entre os medicamentos (p < .05), e o único sinal vital variável nesta comparação foi a frequência cardíaca, na qual houveram diferenças de médias dentre os tempos T1 e T3 (p-valor: 0,0420), T3 e T5 (p-valor: < 0,001), T3 e T6 (p-valor: < 0,001), T6 e T9 (p-valor: < 0,001), contudo, tais alterações não referem-se aos efeitos das drogas, sendo independentemente do grupo (Passiflora e Valeriana).
