This is a randomized clinical study, with parallel, prospective and monocentric treatments. Carried out with 28 women who survived breast cancer and had lymphedema after unilateral mastectomy. Participants will be examined, selected and randomly divided into 2 groups. The determination of which group would receive the intervention and which would make up the control will be made using a random technique and stratified by conglomerates according to the days of the week (Mondays, Tuesdays, Wednesdays and Thursdays) and the shifts in which they attended the outpatient clinic (morning or late). The intervention group (14) will undergo 8 moxibustion sessions (at acupoints LI14, ID 9, LI 13, TA 5) over 4 weeks, in addition to conventional treatment (compressive bandaging and manual lymphatic drainage). The control group (14) will receive only the conventional treatment (compressive bandaging and manual lymphatic drainage). After 4 weeks of intervention, the lymphedema will be evaluated again and the groups compared.

Trata-se de um estudo clínico randomizado, com tratamentos em paralelo, prospectivo e monocêntrico. Realizado com 28 mulheres sobreviventes do câncer de mama e portadoras de linfedema pós-mastectomia unilateral. As participantes serão examinadas, selecionadas e aleatoriamente divididas em 2 grupos. A determinação de qual grupo receberia a intervenção e qual comporia o controle será feita por meio de técnica aleatória e estratificada por conglomerados de acordo com os dias da semana (segundas, terças, quartas e quintas) e os turnos em que frequentavam o ambulatório (manhã ou tarde). O grupo intervenção (14) passará por 8 sessões de moxabustão (nos acupontos IG14, ID 9, IG 13, TA 5) no decorrer de 4 semanas, além do tratamento convencional (enfaixamento compressivo e drenagem linfática manual). O grupo controle (14) receberá apenas o tratamento convencional (enfaixamento compressivo e drenagem linfática manual). Após as 4 semanas de intervenção, o linfedema será novamente avaliado e os grupos comparados.

Inclusion criteria:

Women aged 30 years or older, diagnosed with breast cancer and who underwent unilateral mastectomy at least six months ago, allowing comparison between the two limbs; Undergoing neoadjuvant and/or adjuvant radiotherapy and chemotherapy, which may increase the risk of developing lymphedema; Proof of lymphedema through perimetry of the affected arm ≥3 cm than that of the contralateral arm.

Exclusion criteria:

Diseases related to the vascular system, coagulopathies, skin infections or any condition that contraindicates moxibustion; Limb skin trauma, ulceration or skin disease, as such conditions can hinder venous and lymphatic return; Patients who have breathing problems (allergy to subtle smells), since they can be aggravated by the technique; Women who are receiving other treatments for lymphedema other than those established by the service, in order to guarantee homogeneity in the therapeutic protocol and allow comparisons; Previous contact with any modality of moxibustion, as it will make it impossible for the participant to be blinded; Pregnancy, as there is controversy regarding the safety of moxa in early pregnancy.

The average age of the participants was 58.9 years, the majority being non-white (75%), with a low level of formal education (53.6%) and practicing Christianity (92.9%). As for clinical data, the significant majority were overweight (82.1%) and had undergone total mastectomy (50%), with an average of 11.9 lymph nodes removed (±7.8). All had undergone radiotherapy and chemotherapy. The highest mean difference in perimetry between arm with lymphedema vs. normal arm was 5.3 cm (±2.8 ).

33 women were invited to participate in the study (16 for the intervention group and 17 for the control group). In the intervention group, 2 were excluded for missing more than 20% of the physiotherapy sessions. In the control group, 3 were excluded, as 1 had to change the therapeutic protocol and 2 missed more than 20% of the physiotherapy sessions. Thus, in the end, each group consisted of 14 women who completed the proposed treatment.

The objective was to evaluate the effect of moxibustion as an adjuvant therapy in the control of lymphedema in women with mastectomies. This is an intervention study, carried out at the Hospital do Câncer de Pernambuco during August 2021 and July 2022. Its target audience was women aged ≥30 years, with lymphedema related to the treatment of unilateral breast cancer and who attended the outpatient clinic to perform compressive bandaging and manual lymphatic drainage. Participants were divided into two groups using a random technique and stratified by clusters, where women who attended the outpatient clinic on Mondays and Wednesdays in the morning would make up the intervention group, and those who attended on Tuesdays and Thursdays in the afternoon, the control group. All participants were assessed for the degree of lymphedema, and the perimetry of the upper limbs was recorded at five points. The intervention group was offered (in addition to the standard treatment) eight moxibustion sessions that took place in four weeks with a maximum duration of 20 minutes, while the control group followed only the standard treatment of the institution. Finally, after the intervention period, the lymphedema was again evaluated and the Effective Index for Upper-limb Lymphedema was calculated to determine the progression or regression of the condition. The normality and homogeneity of data variance were evaluated using the Shapiro-Wilk and Levene tests, respectively. Generalized estimation equation models were used considering time (pre vs. post) and group (intervention vs. control) factors. When there was statistical significance for interaction, Bonferroni's post-hoc was used in order to identify where differences occurred. The team members responsible for assessing lymphedema were blinded during the survey, as well as the statistician responsible for data analysis and shift randomization. Resolution 466/2012 of the National Health Council of the Ministry of Health was complied with, receiving a favorable assessment from the Research Ethics Committee of the Hospital do Câncer, with Opinion Number: 4,849,199. In all, 28 women participated in the study, 14 in the intervention group and 14 in the control group. The mean age was 58.9 years, most of whom were non-white, with a low level of education. As for clinical data, the expressive majority were overweight and had undergone mastectomy, with an average of 11.9 lymph nodes removed (±7.8). Women who removed more than 12 lymph nodes had greater perimetry in the region corresponding to 7 cm below the elbow line. Regarding the Effective Index for Upper-limb Lymphedema, the intervention group had an improvement of 23.0% after treatment with moxibustion, while the control group improved by 5.3% (p=0.016). It is concluded that moxibustion presented satisfactory results for the reduction of lymphedema in women submitted to mastectomies due to breast cancer. A simple, low-cost and easy-to-perform therapy can contribute to the management of iatrogenesis, but it is admitted that there are still barriers for complementary therapies to be accepted in the hegemonic models of modern medicine. More studies are needed to prove its efficacy and safety, exploring more plural populations that guarantee the extrapolation of results.

Objetivou-se avaliar o efeito da moxabustão como terapia adjuvante no controle de linfedema em mulheres mastectomizadas. Trata-se de um estudo de intervenção, realizado no Hospital do Câncer de Pernambuco durante agosto de 2021 e julho de 2022. Teve como público-alvo mulheres com ≥30 anos, portadoras de linfedema relacionado ao tratamento do câncer de mama unilateral e que frequentavam o ambulatório para realizar enfaixamento compressivo e drenagem linfática manual. As participantes foram divididas em dois grupos por meio de técnica aleatória e estratificada por conglomerados, onde as mulheres que frequentavam o ambulatório nas segundas e quartas pela manhã comporiam o grupo intervenção, e as que frequentavam nas terças e quintas a tarde o grupo controle. Todas as participantes foram avaliadas quanto ao grau do linfedema, sendo registrado a perimetria dos membros superiores em cinco pontos. Ao grupo intervenção foi ofertado (além do tratamento padrão) oito sessões de moxabustão que ocorreram em quatro semanas com duração máxima de 20 minutos, enquanto o grupo controle seguiu apenas com o tratamento padrão da instituição. Por fim, após o período da intervenção, o linfedema foi novamente avaliado e o Effective Index for Upper-limb Lymphedema foi calculado para determinar a progressão ou regressão do quadro. A normalidade e homogeneidade de variância dos dados foram avaliadas por meio dos testes de Shapiro-Wilk e Levene, respectivamente. Modelos de equações de estimativas generalizadas foram utilizados considerando como fatores o tempo (pré vs. pós) e o grupo (intervenção vs. controle). Quando houve significância estatística para interação, o post-hoc de Bonferroni foi empregado a fim de identificar onde ocorreram as diferenças. Os membros da equipe responsáveis por avaliar o linfedema foram cegos durante a pesquisa, bem como o estatístico responsável pela análise dos dados e à randomização dos turnos. Respeitou-se a resolução 466/2012 do Conselho Nacional da Saúde do Ministério da Saúde, recebendo apreciação favorável do Comitê de Ética em Pesquisa do Hospital do Câncer, com Número do Parecer: 4.849.199. Ao todo, 28 mulheres participaram do estudo, sendo 14 locadas no grupo intervenção e 14 no grupo controle. A média de idade foi de 58,9 anos, sendo a maioria não-branca, com baixo nível de escolaridade. Quanto aos dados clínicos, a expressiva maioria estava com sobrepeso e tinha sido submetida a mastectomia, sendo em média removidos 11,9 linfonodos (±7,8). Mulheres que removeram mais de 12 linfonodos apresentaram maior perimetria na região correspondente a 7cm abaixo da linha do cotovelo. Com relação ao Effective Index for Upper-limb Lymphedema, o grupo intervenção obteve uma melhora de 23,0% após o tratamento com moxabustão, enquanto o grupo controle evoluiu com 5,3% (p=0,016). Conclui-se que a moxabustão apresentou resultados satisfatórios para a redução do linfedema em mulheres mastectomizadas por câncer de mama. Uma terapia simples, com baixo custo e de fácil execução pode contribuir com o manejo da iatrogenia, mas admite-se que ainda existem barreiras para que terapias complementares sejam aceitas nos modelos hegemônicos da medicina moderna. São necessários mais estudos que comprovem sua eficácia e segurança, explorando populações mais plurais que garantam a extrapolação dos resultados.
