This is a longitudinal, prospective, randomised, open clinical study. Total patients included: 60 patients coming out of hospitalisation for severe COVID-19. Randomisation to group 1: 30 patients who will take part in a pulmonary telerehabilitation programme with exclusively remote supervision, or group 2: 30 patients who will take part in a pulmonary telerehabilitation programme with both remote and face-to-face supervision. Duration of the programme: 12 weeks, with sessions held at least three times a week. Evaluation procedures for the two groups, pre and post telerehabilitation programme: six-minute walk test, SF-36 quality of life questionnaire, and assessment of respiratory muscle strength and handgrip strength.

Patients of both sexes; greater than or equal to 18 years old; diagnosis of severe COVID-19 who have been hospitalised; post-discharge outpatient assessment, have dyspnoea at rest; or a distance covered in the six-minute walk test of less than or equal to 400 metres and or 80% of that predicted using a predictive reference equation; or a drop in peripheral oxygen saturation of greater than or equal to 4 points

The study analysed baseline data from 67 participants, including 30 who completed the pulmonary telerehabilitation programme (13 in group 1 and 17 in group 2), and 37 who did not undergo any rehabilitation (control group). The participants' baseline characteristics were analysed, along with the results of the functional assessment using the 6-minute walk test (6MWT) and the SF-36 questionnaire at the first visit after discharge. There were no statistically significant differences between the groups in terms of age (p=0.830), gender (p=0.802), prevalence of the most frequent comorbidities such as systemic arterial hypertension (SAH) (p=0.986) and type 2 diabetes mellitus (DM2) (p=0.284), length of hospitalisation (p=0.162), and mechanical ventilation (MV) (p=0.123). However, there was a statistically significant difference in the length of stay in the ICU (p=0.039). The control group, which did not participate in rehabilitation, had fewer days in the ICU than group 1 (p=0.008), but no difference compared to group 2 (p=0.058). There were no significant differences between the groups in the distance covered during the baseline 6-minute walk test (6MWD) in metres (p=0.214) or in the results of the SF-36 questionnaire, including the physical component summary (p=0.296) and the mental component summary (p=0.366)

Out of the 246 patients assessed, only 114 met the criteria for inclusion in the Pulmonary Telerehabilitation Programme (PTRP). Among the 114 eligible patients, only 49 started PTRP and were randomly assigned to receive either fully remote supervision (G1, n=28) or hybrid supervision (G2, n=21). The reason for not including all eligible patients was the unavailability of places on the rehabilitation programme due to pandemic-related limitations. The aim of this study was to analyse the effect of the Pulmonary Telerehabilitation Programme (PTRP) in two supervision modalities: G1, with exclusively remote supervision, and G2, with hybrid supervision. The variables of the six-minute walk test (6MWT) and the SF-36 questionnaire were used before and after rehabilitation. The groups were similar in terms of distance in the 6MWT pre-rehabilitation. After PTRP, both groups showed improvement and remained similar to each other. The effect of time was significant (p<0.001). The study analysed the quality of life by considering the physical and mental summary components of the SF-36 questionnaire. Before rehabilitation, groups G1 and G2 were similar (p=0.148 and p=0.327, respectively), and both showed improvement after PTRP (time effect, p=0.024 and p=0.050, respectively). The intervention resulted in an improvement in functional capacity (p<0.001), physical aspects (p=0.014), and social aspects (p=0.014). In the pain domain, there was a significant difference between G1 and G2 prior to rehabilitation (p=0.038), which became non-significant after the intervention (p=0.820), indicating improvement with the intervention. To analyse the results of the comparison between the intervention group (IG) and the control group (CG), we used the variable distance travelled in the 6-minute walk test carried out at baseline and after 6 months. The IG included all the patients who took part in the PTRP intervention (G1 and G2), while the CG did not take part in PTRP. The first six-minute walk test (6MWT) was conducted on average 2.78 ± 2.07 months after hospital discharge in the intervention group (IG) and 3.38 ± 1.95 months in the control group (CG) (p=0.124). The results show no significant difference in the 6MWD between the IG and CG groups at baseline, but a difference was observed after 6 months. The intervention group (IG) demonstrated a significant improvement in the six-minute walk distance (6MWD) from 387.1±113.1m to 527.6±106.5m, whereas the control group (CG) did not show any significant improvement between baseline (422.0±110.3m) and 6 months later (438.7±106.8m) (p=0.317). The IG showed an increase of 140.53m in the 6MWD, while the CG only showed an increase of 16.8m (p=0.002)

A qualidade de vida foi analisada considerando os componentes físico e mental resumido do questionário SF-36, e os grupos G1 e G2 eram semelhantes antes da reabilitação (p=0,148 e p=0,327, respectivamente), e ambos melhoraram após o PTRP (efeito do tempo, p=0,024 e p=0,050, respectivamente). Relativamente aos domínios analisados separadamente, a intervenção resultou numa melhoria da capacidade funcional (p<0,001), dos aspectos físico (p=0,014) e do aspectos sociais (p=0,014). No domínio dor, houve diferença entre G1 e G2 pré reabilitaçao (p=0,038), que se tornou semelhante após a intervenção (p=0,820), indicando melhora com a intervenção. Introduction: The COVID-19 pandemic has accelerated the deployment of telehealth services to treat patients recovering from the disease. Objectives: To evaluate and compare the effectiveness of a Pulmonary Telerehabilitation Programme (PTRP) with fully remote versus hybrid (remote and face-to-face) supervision. Methods: This was a prospective, interventional, randomised clinical study carried out in a university hospital (Brazil) and approved by the institution's ethics committee. The study was carried out with patients who were hospitalised with severe COVID-19 and who, 90 days after hospital discharge, had a 6-minute walk test of <400m or a reduction in SpO2≥4% throughout the test. Participants were randomised to PTRP with fully remote supervision (G1) or hybrid supervision (G2). The efficacy parameters were the 6-minute walk distance (6MWD) and quality of life (QoL) (SF-36). PTRP included structured exercises under remote supervision for 12 weeks, 3 sessions per week. Patients were assessed every 3 weeks, at the end and after 3 months of PTRP. The SF-36 was performed at the beginning and end of the PTRP. Results: Of the 246 patients assessed, 114 were eligible for PTRP; of these, only 49 started the programme and were randomised to receive either fully remote supervision (G1, n=28) or hybrid supervision (G2, n=21). The reason for not including all eligible patients was the unavailability of places on the PTRP due to pandemic-related limitations. There was a significant discontinuation rate, with only 13 patients from G1 and 17 from G2 completing the PTRP. The 37 eligible patients who were not included in the PTRP were re-evaluated and constituted a control group (CG). The initial 6MWD was similar between G1 (389m) and G2 (380m) (p=0.92). Both groups improved their QoL scores (physical component, p<0.02; emotional component, p<0.05); the 6MWD also increased in both groups (p<0.001) with increases of similar magnitude (G1=126m, G2=135m), p=0.8815. The 6MWD was used to compare the intervention group (IG) made up of all those who took part in the PTRP (G1+G2) and the control group (CG): there was no significant difference in the 6MWD between IG and CG at the pre-PTRP moment, but there was at the end of 6 months, when the IG had a significantly higher 6MWD than the CG group (p<0.001). GI increased the 6MWD by 140.5m, and CG by only 16.8m (p=0.002). Conclusions: PTRP was effective in improving the participants' functional capacity and QoL, with no difference between the supervision modalities. Severe post-COVID patients with reduced functional capacity showed a significant and lasting increase in 6MWD after PTRP, when compared to patients in clinical follow-up who received usual care

Objetivos: Avaliar e comparar a eficácia de um Programa de Telereabilitação Pulmonar (PTRP) com supervisão totalmente remota versus híbrida (remota e presencial). Métodos: Estudo clínico, prospetivo, intervencional, randomizado, realizado em um hospital universitário (Brasil) e aprovado pelo comitê de ética da instituição. O estudo foi realizado com pacientes que foram hospitalizados com COVID-19 grave e que, após 90 dias da alta hospitalar apresentavam, no teste de caminhada de 6 minutos uma (DTC6) <400m ou redução na SpO2≥4% ao longo do teste. Os participantes foram randomizados para PTRP com supervisão totalmente remota (G1) ou supervisão híbrida (G2). Os parâmetros de eficácia foram a DTC6 e a qualidade de vida (QV) (SF-36). O PTRP incluiu exercícios estruturados sob supervisão remota durante 12 semanas, 3 sessões por semana. Os pacientes foram avaliados a cada 3 semanas, no final e após 3 meses do PTRP. O SF-36 foi realizado no início e no final do PTRP. Resultados: Dos 246 pacientes avaliados, 114 eram elegíveis para o PTRP; destes, apenas 49 iniciaram o programa e foram aleatorizados para receber supervisão totalmente remota (G1, n=28) ou supervisão híbrida (G2, n=21). O acesso ao projeto no REDCap é restrito à coordenadora do projeto e aos envolvidos na coleta e inserção dos dados. Os dados individuais dos participantes não identificados poderão ser compartilhados mediante autorização da coordenadora do projeto, após checagem dos motivos de solicitação e da conformidade com preceitos éticos e dos objetivos do projeto original. Os dados estarão disponíveis na plataforma digital (em nuvem) por até 5 anos, após o que será feito download para armazenamento off line em um local acessível em computador sem necessidade de conexão com a internet.
